However, the bacterial microflora of the skin, especially Staphylococcus aureus, plays an important role in its the pathogenesis. Continuously, new therapies are sought which will block factors exacerbating the disease and significantly support the AD treatment. The aim of the study was to determine the effect of sodium hypochlorite baths on the eradication of St. aureus from the skin and to assess their effect on AD severity. Material and methods: The study was conducted among 56 patients suffering from AD. A half of the patients were randomly assigned to the group treated with sodium hypochlorite baths (study group) in addition to the standard therapy of AD, the remaining patients were treated with conventional treatment (control group). The severity of the disease and the intensity of the itch were assessed in each patient at the time of enrollment into the study and 6 weeks later. A skin smear was taken for microbiological examination with pathogen identification and semi-quantitative evaluation of the isolated bacteria strains. Results: Patients using baths in diluted sodium hypochlorite showed a greater improvement of AD severity compared to those using conventional treatment only (relative EASI improvement: 42.0% ± 32.9% vs. –33.1% ± 88.6%, p < 0.01; relative SCORAD improvement: 33.5 ± 25.9% vs. –6.7 ± 36.8%, p < 0.001). Additionally, a greater reduction of itch was observed in the study group compared to the control group (ΔNRS: 1.8 ± 2.2 vs. 0.7 ± 2.2, p = 0.04) as well as significant improvement of quality of life was noted (study group: mean DLQI before treatment: 12.7 ± 6.7 points, mean DLQI after treatment: 7.3 ± 5.4, p < 0.001; control group: mean DLQI before treatment: 10.8 ± 7.5 points, mean DLQI after treatment: 8.7 ± 5.6, p < 0.82). However, the relationship between the applied therapy and the eradication of St. aureus has not been confirmed. Conclusions: Bathing in diluted sodium hypochlorite allowed to reduce the intensity of AD and to improve the quality of life, while ensuring good tolerability and safety of the therapy. However, no significant effect of the therapy was observed on the reduction of the number of St. aureus colonies on the skin.","PeriodicalId":112619,"journal":{"name":"Forum Dermatologicum","volume":"25 21","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-06-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"120842400","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Topical or systemic corticosteroids, cysclosporine or hydroxychlorquina are the treatments usually proposed. We describe the effectiveness of intralesional triamcinolone acetonide in concentration of 8 mg/mL with oral acitretin 10 mg/day combined with topical 0.05% clobetasol propionate ointment in a patient with GLPLS. After 2 months of treatment, reduction of perifollicular erythema and follicular hyperkeratotic papules were observed with no new area of alopecia. We believe that intralesional triamcinolone acetonide could be an effective treatment by limiting the progression of the disease.","PeriodicalId":112619,"journal":{"name":"Forum Dermatologicum","volume":"38 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-12-21","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"132418806","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

During the last decades, scientists have engaged in research that has increased our understanding of different mechanisms of aging of a hair follicle, however many aspects of this process remain unclear. Progressive hypopigmentation of the hair shaft has been associated with decrease in number of melanocytes and tyrosinase hypoactivity in the hair bulb along with the depletion of melanocyte stem cells (MSCs) in the bulge area. Pathogenesis of graying may involve oxidative damage and incompetent MSCs maintenance. Moreover, other factors such as defective melanosomal transfer as well as insufficient neuroendocrine stimulation of MSCs and melanogenesis are hypothesized to play an important role.","PeriodicalId":112619,"journal":{"name":"Forum Dermatologicum","volume":"105 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-12-21","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"114605370","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It is estimated that even 70–80% of patients do not require systemic treatment or light therapy, and topical medications effectively control the symptoms of the disease. Moreover, topical medications are also an essential treatment option in addition to systemic therapy for moderate to severe psoriasis. The most common medications used in local therapy are glucocorticosteroids and vitamin D analogues. These substances show the highest efficiency among all drugs available on the market, while maintaining a favorable toxicity profile. A significant advantage of these medications is also the ease of use, which significantly affects the compliance with medical recommendations and regular use of the preparation by patients. The different mechanism of action of the two drugs makes this unique combination of calcipotriol and betamethasone dipropionate particularly effective at any stage of the pathogenesis of psoriasis. Due to their strong antiproliferative, immunomodulating and immunostatic effects, this duo is currently recommended by the Polish Dermatological Society as a first-line preparation in the local treatment of smooth skin, hands and feet. The ointment ensures rapid penetration of active substances, strongly moisturizing, reducing skin atrophy, which is an additional benefit for patients with psoriasis. The study analyzes the effectiveness, safety and mechanism of action of medications containing calcipotriol with betamethasone dipropionate in the local therapy of psoriasis in the light of current literature data, with particular emphasis on the form of ointment.","PeriodicalId":112619,"journal":{"name":"Forum Dermatologicum","volume":"57 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-12-21","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"115111819","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
