Our case highlights the imperative of considering catheter-based minimally invasive therapy for the increasing number of adults with congenital heart disease who may present multiple risks factors for surgery. Because of advancements in medical and surgical treatment, there has been a decline in mortality from congenital heart defects over the past several decades [1]. As a result, the number of patients with adult congenital heart disease has increased over time [2,3]. Most patients with adult congenital heart disease underwent several surgeries during childhood. In some cases, surgical or catheter interventional treatment in adulthood is recommended because of sequelae or remote-term complications [4,5]. In some cases, catheter intervention is preferable to surgery because the patient is considered a high-risk candidate. The fundamental biologic relationship of the QT interval-heart rate relationship has rarely been studied across different species. Methods: The literature on the QT and heart rate of 32 different species was examined specifically in the whales, dolphins, Asian elephants, rhinoceros, camels, brown bears, grizzly bear, lions, tigers, sea lions, seals, donkeys, ponies, horses, cows, sheep, goats, llamas & alpacas, swine, Minipigs, deer, dogs, cats, baboons, apes, monkeys, prairie dogs, marmot, rabbit, hamsters, guinea pigs and mice. Results: An inverse relationship between QT interval and heart rate was apparent across species. The relationship was poorly fit by a linear formula. The Bazett approach (square root of heart rate or RR interval) or the Fridericia approach (cubed root of the heart rate or RR interval) produced slight improvements. Second, third and fourth order polynomials produced a progressive greater improvement in the goodness of the fit of the relationship. The best fit of the relationship was attained by a nonlinear two-phase relationship identifying that there is one QT-heart rate relationship at slower heart rates and one at faster heart rates. Conclusion: The Bazett, Fridericia and linear formulae approaches to QT interval correction for the impact of heart are far from ideal. Methods: 254 patients affected by stenosis or stenoinsufficiency of the aortic valve received a Sorin Pericarbon Freedom (SPF) aortic bioprosthesis at our institution between 2003 and 2010. Out of the entire cohort, 184 patients, alive and capable of sustaining an exercise test 7 years after surgery, were selected and enrolled in the study (mean age: 69.7 ± 10.9 years, 51.6% male, mean EF: 53.0 ± 7.1). All patients were evaluated by transthoracic echocardiography in order to assess hemodynamics at the time of hospital discharge and after 7 years. Followup control consisted of clinical examination and rest and exercise echocardiographic evaluation using a semisupine bicycle with a workload of 25W every two minutes, increasing up to 100W. Results: Comparing rest to exercise, mean gradient increased from 7.8 ± 3.3 mmHg to 11.6 ± 4.0 mmHg (p < 0.05) and peak gradient from 14.9 ± 5.2 mmHg to 21.6 ± 6.5 mmHg (p < 0.05) at maximum stress, without significant differences between valve sizes. Concurrently EOA and EOAi raised from 1.86 ± 0.69 cm2 to 1.96 ± 0.71 cm2 (p < 0.0001), and from 1.05 ± 0.37 cm2 to 1.10 ± 0.38 cm2 (p < 0.0001) respectively, without significant differences between the implanted valve sizes. Intraaortic regurgitation was absent or trivial in 98% of patients, and did not change with exercise. Conclusions: Use of exercise echocardiography to assess changes in hemodynamics and function of aortic bioprostheses can be helpful in the choice of appropriate aortic valve substitutes. SPF pericardial stentless valve presents favourable and durable hemodynamic characteristics at rest and on exercise, similar to native aortic valve. Speckle Tracking Echocardiography (STE) is a novel technology to assessing modifications of myocardial deformation with higher accuracy than the simple visual of regional wall motion. Purpose: The study was to evaluate, using STE applied to dobutamine stress echocardiography (DSE), changes in echo parameters before and after successful recanalization of a CTO. Methods: Eleven patients with subacute or chronic coronary syndromes and angiographic evidence of CTO, scheduled for reopening by percutaneous coronary angioplasty, underwent DSE, using standard protocol, before and 3 months after CTO recanalization. The acquisition of Echo images was performed at baseline and peak stress. Left ventricular ejection fraction (EF), volumes, wall motion score index, parameters of diastolic flow, tissue velocities at mitral annulus, global longitudinal strain (GLS), strain rate (SR), systolic and diastolic longitudinal functional reserve (SLR and DLR respectively) were obtained at baseline and at peak stress before and after percutaneous coronary angioplasty (PCI). Results: The mean follow-up after PCI was 3.09 ± 1 months. Statistical analysis showed significant improvement after PCI in EF, comparing to baseline (P < 0.03) and stress peak (P < 0.001) in systolic SR from the 4 chamber-view (P < 0.02), in DLR (P < 0.01), but there were no significant changes for SLR, GLS and SR. It is caused by conscious or unconscious manipulation at the implantation site by the patient with the result of device malfunction. It results in lead dislodgment, diaphragmatic stimulation, and loss of capture. In a pacemaker dependent patient, it can be a lethal complication. It usually occurs in the first year following pacemaker implantation. The treatment involves readmission, repositioning of the dislodged leads and suture fixation of the lead and pulse generator within its pocket. Although femoral artery aneurysm is usually caused by atherosclerosis or chronic inflammation, aneurysm formation in sites of anastomosis, grafts or endarterectomy territory may occur at late term. A 57-yearsold man presenting with stable angina pectoris was admitted for coronary angiography with documented ischemia in exercise stress test. The patient had a history of left femoral thromboendarterectomy and venous patch plasty five years ago. Physical examination revealed a pulsatile mass in the left groin. Coronary angiography revealed a distal circumflex artery lesion and follow up with optimal medical treatment was planned for coronary artery disease. Afterwards, a lower extremity arteriography was performed which indicated a true aneurysm of the left common femoral artery (CFA). The aneurysm was at the site of previous endarterectomy and patch plasty pointing out an iatrogenic etiology. A multidetector computed tomography (MDCT) angiography was performed to rule out any other arterial aneurysms and confirm the relation of the aneurysm and deep femoral artery bifurcation. Objective: We evaluate the metabolic profile from risk factors associated with the development of atherosclerosis in patients with CKD. Method: Biological markers for renal function and associated with atherosclerosis were analyzed from serum samples of 23 patients before undergoing hemodialysis procedure. Results: We observed a correlation of HDL with creatinine. Moreover, a positive correlation was observed between serum albumin levels and the values of VLDL and triglycerides, concomitant with a decrease in HDL.
