This study aims to identify the failure risk of the most dominant type of defect in Polyethylene (PE) Film products and identify the significant factors that cause the most dominant defects so that a proposal for improvement is obtained. The Fuzzy Failure mode and effect analysis (FFMEA) method is applied to reduce the risk of product defects in the PE Film production process. The steps with FFMEA are: a). Identify the type of defect of the Polyethylene film product, b). investigate the potential causes of each mode of failure and their impact, c). Identify the scale and linguistic terms for the input factors S, O, and D. d). Create a membership function appropriate for S, O, and D input factors, e). Change the input factor values S, O, and D to linguistic variables, f). establish an IF-Then rule, create a fuzzy risk priority number (FRPN) membership function, g). defuzzification. The buildup of material can cause the production material transfer system less efficient. The research aims to identify waste in the production material transfer process using current state value stream mapping (VSM), recommend improvement strategies using future state VSM, and design a more efficient production material transfer system. First, we develop a causal loop diagram because it can map the problem. Then, we identify and fix the problem through value stream mapping for more efficient use of resources. The waste identified in the current state value stream mapping consists of waste of over-processing, waste of motion, and waste of useless transportation. In the future-state VSM, the lead time of the material transfer process is reduced by 840 seconds or 14 minutes, the cycle time of the material transfer process is reduced by 731 seconds or 12 minutes, the area needed is reduced by 43.2- meter square, and the total workforce reduced to two people. Based on the interviews with factory owners, there have been several work accidents, and the production room has very minimal lighting and air ventilation. Based on these problems, the method used is the Hazard and Operability Study (HAZOP), which aims to identify sources of hazards and potential work accidents in the production process to prevent work accidents. The risk matrix analysis shows as much as 24% of the risk is classified as extreme, 65% is classified as high risk, and 12% is classified as moderate risk. In addition, 17 sources of danger were found, namely knives, slippery floors, smoke, hot water, drying roofs, fire, smoke, hot water, ash flakes, hot tanks, wooden board, chopper, loads carried by workers, ovens, hot oil, hot frying pan, hanging boards, and cramped workplaces. According to Top Brand Award data for 2020, most of Wardah's products experienced a decline in buying interest. The decrease in the percentage of interest in buying Wardah skincare products can be caused by the shift of consumers to other brands. Buying interest is how likely a consumer is to buy a brand and service or how likely a consumer is to move from one brand to another, so this study aimed to analyze different perceptions between two skincare consumers, Avoskin and Wardah, using the variables of halal certification, brand ambassador, and beauty vlogger review. The sampling technique used in this study was purposive sampling, taken from the respondents. This study used the Mann-Whitney test as the data analysis method. The company also produces machines used to process palm oil, such as threshers, screw conveyor belts, and sterilizers. This company found 15 cases of work accidents occurred from 2017-2020. The purpose of this study is to analyze the causes of frequent accidents and analyze potential hazards and assess the risk of hazards in the company so that it can provide recommendations for appropriate control for the company using the hazard identification risk assessment and risk control (HIRARC) method. There are 5 types of accidents that have occurred frequently and 13 potential accident hazards in the company today. Then for the risk level in the risk assessment, there is 1 event with a high-risk rating (in red), 10 events with a medium level risk rating (yellow), and 11 events with a low-risk rating result (in green). The risk of accidents that should be a priority is the risk of gas cylinder leakage accidents.
