The search for painless and minimally invasive alternatives is a challenge in the field of aesthetic medicine. Objective: To evaluate the efficacy and safety of intradermal microinjection of autologous platelet concentrate (APC) in the treatment of perioral skin aging signs. Method: An observational, analytical and longitudinal study was carried out in 60 patients from the Hospital Clínico Quirúrgico: “Hermanos Ameijeiras”, in the period between March 1, 2017 and March 31, 2020. The treatment was applied monthly. For 1 year. The final evaluation was carried out 3 months after the end of the treatment. Results: 60 women with an average age of 45 ± 4.3 years were treated. After treatment, there were significant changes in the Glogau Photo Damage Scale (P = 0.028), in the Global Aesthetic Improvement Scale (P <0.032) and in the Evaluative Scale of the severity range of the nasolabial fold (P = 0.031). The adverse events found were pain, inflammation and ecchymosis. The degree of satisfaction reported by the patients was good (16.7%) and very good (83.8%) (P <0.038). It is currently a versatile therapy in dermatology; however, few studies objectively evaluate its efficacy. {"title":"Why Evaluate the Sunscreen Active Oxybenzone (Benzophenone-3) for Carcinogenicity and Reproductive Toxicology or Consider it Unsafe for Human Use?","authors":"DiNardo Jc, Downs Ca","doi":"10.33425/2690-537x.1005","DOIUrl":"https://doi.org/10.33425/2690-537x.1005","url":null,"abstract":"On February 26, 2019, the Food & Drug Administration (FDA) published an update to the Sunscreen Drug Products for Overthe-Counter Human Use in the Federal Register [1], requesting additional information on 12 petroleum-based sunscreen actives (cinoxate, dioxybenzone, ensulizole, homosalate, meradimate, octinoxate, octisalate, octocrylene, padimate O, sulisobenzone, oxybenzone and avobenzone). This is because the public record does not currently contain sufficient data to support that these drugs are Generally Recognized as Safe and Effective (GRASE). The FDA went on to conduct and publish a small clinical trail [2] to determine whether four of these drugs (avobenzone, oxybenzone, octocrylene, and ecamsule) absorb into the systemic circulation and whether they would exceed the FDA level for toxicological concern (0.5 ng/mL). The data collected demonstrated that all four sunscreen drugs absorbed through the skin and into the blood, and accumulated above the 0.5 ng/mL level within the first day (4 applications) of the study. The perioral region is the most dynamic anatomic area of the face and is subject to complex and dramatic changes during aging. The ideal and effective treatment for perioral rejuvenation and, above all, smoker's lines, has yet to be identified. All proposed techniques and previous studies reported only subjective results. The propose of this study is to evaluate the treatment of the upper lips and the smoker’s lines with a superficial disepithelization with a non-ablative multifrequency electrosurgery and a peeling based on mandelic acid. Patients with upper lip wrinkles were inserted into the protocol. After a light scrub, we proceeded to make a superficial dermabrasion using a multifrequency electrosurgery able to create a voltaic arc. Once the superficial corneal layer of the skin was removed, we made a peeling consisting of mandelic acid combined with lactoferrin until a white Frost was obtained. We treated 25 women with an average age of 60 (Range 52/73). A marked improvement, from 70% to 90%, was observed in 18 patients with class I II wrinkles. Instead a moderate improvement was observed in 7 cases with class II III wrinkles, with percentages ranging from 40% to 60%. Dermabrasion controlled by a voltaic arc, combined with peeling with MA and LF, has proved effective for the marked reduction of perioral wrinkles. The arc acts without coming into contact with the tissues, creating a delicate coagulation. Patients and method: To evaluate the therapeutic efficacy and safety of pegylated interferon, alone or adjuvant for cryosurgery, in the condyloma acuminata an open clinical trial was carried out on 30 patients of the \"Hermanos Ameijeiras\" hospital, who were randomized to receive for 6 weeks (group A) only fortnightly cryosurgery, (group B) subcutaneous pegylated interferon, once a week, associated with fortnightly cryosurgery application or (group C) only subcutaneous pegylated interferon, once a week. The main variable was the percentage of recurrence at one year of follow-up, evaluated quarterly. There was also a rigorous control of adverse events. Results: At the end of the treatment 8/10 (80%) patients from group A, 10/10 (100%) from group B and 9/10 (90%) from group C were left without injuries. During the follow-up, none of the patients who received pegylated interferon, alone or adjuvant to cryosurgery, had recurrences; while 100% of patients who received cryosurgery as exclusive treatment relapsed (p <0.000). The most frequent events were local burning (100%) due to cryosurgery and fever (6.6%), headache (6.6%), myalgia (3.3%) and malaise (3.3%) due to interferon. Arachidonic acid (AA; 20:4n-6), eicosapentaenoic acid (EPA; 20:5n-3) and docosahexaenoic acid (DHA; 22:6n-3) are the most predominant essential fatty acids in human skin [2]. They are precursors for synthesis of mediators that are pro-inflammatory or that actively end this process and named for this specific effect, specialized proresolving lipid mediators (SPMs). As the cause, the involvement of a toxin (called superantigen) produced by Staphylococcus aureus, which is detected from the skin of atopic dermatitis, is presumed. As a measure against Staphylococcus aureus, the use of Povidone-iodine was listed as adjuvant therapy in the Japanese Guidelines for Atopic Dermatitis Treatment 2018.
