The authors want to highlight its indications in primigravidae according to Robson's classification. Methods This prospective observational study was carried out on 518 patients in 2019 and 366 patients in 2020 who were nulliparous, single foetus, more than or equal 37 weeks, passed into spontaneous labour and received induction of labour after their admission to Assiut Woman's Health University Hospital, Assiut, Egypt. Results Residence in rural areas, education and placenta previa were significantly higher in 2019 compared with 2020 while residence in urban areas and intrauterine growth restriction were significantly lower in 2019 compared with 2020. Failure of progress in delivery in Robson group 1 was significantly lower in 2019 than in 2020. Intrauterine growth restriction in Robson group 2 was significantly lower in 2019 than in 2020. They are characterized by presence of antinuclear antibodies (ANA). ANA include autoantibodies to extractable nuclear antigens, autoantibodies to histones and deoxyribonucleic acid (DNA). Anti-double stranded DNA (dsDNA) antibodies are recognized as diagnostic markers of systemic lupus erythematosus (SLE) and as indicators of SLE disease activity, especially in lupus nephritis. The significance of anti-dsDNA in SLE diagnosis and in monitoring SLE disease activity has led to increase in this test laboratory requests as well as in the number of commercially available kits. Aim of the work This study aims to evaluate the performance of Alegria anti-dsDNA screen test method in the Laboratory of Clinical Immunology, Assiut University and to compare its results to the results obtained by immunofluorescence method. Materials and methods Evaluation of the performance of Alegria anti-dsDNA screen test as a qualitative test was evaluated through method comparison according to Clinical and Laboratory Standard Institute (CLSI) guidelines. Results obtained by Alegria anti-dsDNA screen kit were compared with that obtained by nDNA Fluoro-Kit indirect fluorescent antibody test. Manufacturer's recommended reference interval of Alegria anti-dsDNA screen was verified according to CLSI guideline. Result Alegria anti-dsDNA screen ELISA kit showed 94.4% positive agreement, 37% negative agreement and 65.7% overall agreement with nDNA Fluorokit. In the verification study of manufacturer's reference interval (negative <25 U/ml), 5/40 specimens (12.5%) from healthy subjects were positive which exceeds the acceptance criteria of 10%. ROC curve methodology was used to analyze results of both methods and cut off value was adjusted. With cut off adjustment, method specificity increased but sensitivity was decreased. Optimal cut off was determined to be 74U/ml with acceptable level of both sensitivity and specificity (72% and 74%, respectively). Conclusion Alegria anti-dsDNA screen method demonstrated good sensitivity and low specificity. With manufacturer's cut off adjustment, specificity was improved. Manufacturer's reference interval of Alegria anti-dsDNA screen was not verified. Clinical audit is the balance that helps us to assess how well we are performing our daily practice against research evidence-based criteria. It is a multidisciplinary tool involving all members of healthcare team. Objectives The aim of this study is to assess how useful criteria-based management audit is in improving medical records and how far it helps in the overall cost of care of psoriatic patients attending the Phototherapy Unit of Dermatology Department, Assiut University Hospital. Patients and methods A rapid review was conducted to 32 psoriatic patients referred to the phototherapy unit and a quasi-experimental design (audit) of our experience with phototherapy in the treatment of psoriasis was performed retrospectively over a 6 months period from May to November 2018. That was followed by a successive prospective 6 months sustained improvement period from December 2018 to May 2019 following the standards defined by the British Association of Dermatologists phototherapy service guidelines 2018. Results Most of the criteria were fulfilled and achieved 100% outcome but the rest were deficient; the most significant were absence of written consent and absent discharge protocol. Patients' records did not include the skin type, minimal erythema dose, minimal phototoxic dose and total cumulative dose which necessary for skin cancer surveillance. Only 62.5% of the patients were subjected to Psoriasis Area Severity Index score measurement. Conclusions The present study demonstrated that the phototherapy unit varied in some issues in their capacity to meet international audit standards of British Association of Dermatologists guidelines in the management of psoriatic patients. By using a hearing aid (HA) in the nonimplanted ear, children can benefit from binaural hearing advantages and auditory deprivation is prevented. The present study aimed to determine the bimodal stimulation advantages in children, especially its effect on speech recognition ability and discover the effect of some factors on bimodal hearing. Materials and methods In this study, the participants were 42 CI children age ranged from 5 to 12 years old who used a HA in their contralateral nonimplanted ear for 6 months at least. The parents of the children signed the informed consent. Early speech perception in Arabic (ESP) test, the developed Arabic word in noise test and Aided audiometry were compared in bimodal fitting condition and CI condition. Results and discussion There was a great improvement in both ESP test scores and word in noise test scores in case of bimodal condition over CI alone condition. While on aided sound field audiometry test, there was no statistically significant improvement in the aided tonal threshold between bimodal and CI alone aided threshold. Factors such as implant age, regularity of HA usage, and amount of residual hearing in the contralateral ear were not affect benefits of bimodal hearing. Although the author now understand this group of complicated tissues better, it is still unclear which surgical procedure is ideal. The author hypothesize that acute repair of cases is enough for posterolateral corner (PLC) injury. Also, Larson's technique is equal to biceps femoris tenodesis reconstruction of posterolateral corner. Methods Case series study of 41 patients included in this series with 14 cases of repair and 27 cases of reconstructions using Larson's technique or biceps femoris tenodesisis. Patients were followed clinically and recorded with IKDC score. Results 41 patients had a minimum clinical follow-up of 24 months. 14 patients underwent acute primary repairs; with 11 having successful results and three (21%) being unsuccessful. 15 patients had reconstructions using the Larson's approach, with 14 successful outcomes and 1 (6.6%) failure. 12 patients underwent biceps tenodesis, with 9 (75%) successful results, and three (25%) being unsuccessful. Clinical evaluation results showed no discernible difference in stability between repairs and reconstructions (P > 0.05). 17 (41.5%) knees were found to be normal, 17 (41.5%) near-normal, 4 (9.8%) abnormal, and 3 (7.3%) severely abnormal according to the IKDC score final assessments. Conclusion When compared with the results from reconstruction, results from repair of soft tissue injury were significantly inferior. Results of repair of avulsion fracture have been significantly superior in comparison with result of soft tissue repair. Aim of the work Accurate measurement of airflow limitation in COPD patients and correlation between standard forced expiratory volume in 1 s (FEV1) and VFL as an accurate measurement of airflow limitation. Patients and methods This study included 50 patients, 40 patients with COPD from outpatient clinic of Chest Department of Asyut University Hospital and 10 control persons were enrolled into our study. Measurement of spirometric data was taken (FEV1, forced vital capacity, FEV1/forced vital capacity) and patients were classified according to GOLD 2017 into mild, moderate, severe, and very severe COPD. Patients in our study were classified into two groups: group I: mild and moderate, group II: severe and very severe COPD. Tidal flow-volume loop was drawn by plotting exercise tidal flow-volume loop inside maximal flow-volume loop. Assessment of airflow limitation was done by measuring VFL: percent from tidal volume (Vt) that expiratory airflows meet or exceed the maximal expiratory flow. End expiratory lung volume (EELV) was measured from exercise tidal flow-volume loop. Results All very severe COPD patients had severe VFL. While severe COPD patients who had severe VFL were only 27.3%. The remaining 72.27% of severe COPD patients had moderate VFL. All mild and moderate COPD patients had mild VFL. Patients were classified into three categories: (a) with mild VFL for mild and moderate COPD grades, (b) with moderate VFL for severe COPD grade, (c) with severe VFL for very severe COPD grade. The increase in EELV during exercise is linked to the occurrence of VFL. EELV is increased to mean value of 618 ml in group II and just 240 ml in group I. Group II had significantly higher (P < 0.0001) VFL and EELV. As a result, phototherapy induced hypocalcemia can be a serious issue in neonates. Aim The study's aim was to determine phototherapy effect on serum calcium levels in neonates having unconjugated hyperbilirubinemia after being exposed to phototherapy for 48 h. Patients and methods This study is an interventional single-arm study conducted on 40 full-term newborns with jaundice (24 of them are males and 16 are females) who received phototherapy for neonatal indirect hyperbilirubinemia (exaggerated physiological jaundice) in neonatal ICU of Children's Hospital at Assiut University in the period between February 2020 to February 2022. Results The total serum Ca levels significantly decreased over admission period from 9.72 ± 0.58 to 9.41 ± 0.47 mg/dl. Similarly, ionized Serum Ca levels significantly decreased over admission period from 1.19 ± 0.11 to 1.07 ± 0.12 mg/dl. Malnutrition is an ongoing problem among children with ESRD. Children are at greater risk due to accelerated growth velocity and brain development. The provision of adequate nutrition and regular evaluation of nutritional status are key points in the management of children with CKD. Objectives To assess the nutritional status of ESRD children on regular hemodialysis attending the Nephrology Unit at Assiut University Children's Hospital (AUCH). Patient and methods A descriptive study included children with ESRD on regular hemodialysis, who attended Assiut University Children's Hospital during the 1-year period from 1st Jan 2020 to 31st December 2020. The patients were assessed after 6 months from enrollment in the study. The patients were carefully assessed for signs of growth failure and malnutrition by measuring weight, height, and BMI putting them on percentile using clinical growth charts according to their genders and ages. There was growth failure if weight and height were less than equal to third percentile and BMI less than fifth percentile. Results This study included 60 patients with ESRD on regular hemodialysis. Their ages ranged from 6months to 18 years old with mean ± SD (9.7 ± 5.2); 40 patients were males and 20 patients were females. Congenital anomalies of the urinary tract system were the most common cause of ESRD in the studied patients. Body weight in 55% of the studied patients was less than the 3rd percentile, height in 62% of them was less than the third percentile, and BMI was less than the 5th percentile in 65% of the patients; 35% of the patients were within normal BMI (5th–84th), all those cases had limitations in nutrients supplementation. Patients and methods A total of 32 eyes of 19 infants with primary congenital glaucoma were enrolled in this study. All eyes underwent combined trabeculotomy-trabeculectomy with mitomycin C under general anesthesia. Follow-up of the surgical outcome was achieved for each patient by measuring the postoperative ocular tension monthly for 3 months and then every 3 months till the end of a year. Results A total of 20 (62.5%) eyes showed successful intraocular pressure (IOP) lowering after the surgery, with mean IOP of 15.3 mmHg, with range between 11 and 19 mmHg measured under general anesthesia using halothane. A total of nine (28.1%) eyes required postoperative antiglaucoma medications to normalize the IOP and three (9.3%) eyes required glaucoma drainage device surgery to normalize the IOP. Pressure control (PC) and volume control (VC) modes are used for the management of moderate and severe ARDS. Each mode has its benefits and drawbacks. Aim The aim of this study was to compare lung-protective strategy (LPS) using PC ventilation with LPS using VC ventilation to get the best benefit for those critically ill ARDS patients and to solve the problem of hypoventilation. Patients and methods This prospective, randomized controlled trial was carried out at the respiratory intensive care unit of university hospital during the period from October 2017 to December 2018. The study included 30 patients diagnosed as ARDS according to Berlin definition. Those patients were categorized into two groups: group I is the PC group (15 patients) using the LPS and group II is the VC group (15 patients) using the LPS. Results The mean age was 48.93 ± 9.91 years for the PC group with female predominance (53.3%), while the mean age was 46.11 ± 7.94 for the VC group with male predominance (53.3%). Arterial blood gases showed a significant higher pH and lower PaCO2 in the VC group. Mechanical parameters showed a significant higher VT and VE in the VC group. Weaning and outcome in both groups were successful, while seven of the patients of PC (46.7%) and four of the patients of VC (26.7%) failed extubation and died. Conclusion The authors conclude that both modes are equally effective in improving oxygenation in patients of ARDS, who were ventilated with LPS, while VC achieved better ventilation, higher pH, and lower PCO2 than PC.
