This modification of the malleolar LRR distinguishes venous healthy from venous patients like the malleolar procedure. By the plantar procedure LRR becomes possible in cases where malleolar LRR is not performable like in larger leg ulcers and lymphedemas. In this process 6 patients out of 3 kindreds were identified additionally. 10 genotypic carriers of the disease were found of 30 examined relatives by a newly developed UV-provocation test. An autosomal-dominant hereditary pattern was proven for sure and with high probability, respectively, in 10 out of 11 kindreds by formal genetic genealogy-examinations. There was an incomplete penetrance in 3 kindreds. A sex-determined heredity of disease can be excluded. Statistically significant increase of HbA2 was established in patients with psoriasis vulgaris placata (restricted forms) as well in patients with psoriatic arthritis. HbF is statistically significant increased in all forms of psoriasis. In PS-patients, the serum levels of both parameters were significantly increased with a high correlation to the stage of the disease and to each other. In patients with silicosis normal serum concentrations of both parameters were found, whereas in all patients suffering from silicosis and PS significantly elevated serum levels were detectable, independent of the stage of PS. This peculiarity can be used for photodynamic destruction of propionibacteria. After repeated irradiations with a blue light-type high pressure lamp (Narva NC 1000-64) both acne and seborrhoea was improved markedly (10 irradiations, each for 10 minutes, cumulative light dose 325 J/cm2). Also porphyrin content inside acne follicles dropped. A possible mechanism of the systemic immunomodulatory effects of ultraviolet light in the organism is the transformation of urocanic acid in the skin from its trans- to the cis-isomer, which acts as a mediator on the immune system. A treatment of graft recipients with cis-, but not trans-urocanic acid also prolonged graft survival. The rejection of secondary grafts and the humoral immune response of mice to sheep erythrocytes were not influenced by cis-urocanic acid. {"title":"[Abnormal hematopoiesis in psoriasis--in vitro studies].","authors":"H Weber,&nbsp;W Diezel,&nbsp;J Greiner,&nbsp;N Sönnichsen","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Using mononuclear cells (MNC) of patients suffering from psoriasis for production of colony-stimulating factors (CSF) an abnormal colony formation in bone marrow of healthy volunteers was detected (colony-forming assay): Number of colonies per 10(5) bone marrow cells after cultivation with 10(6) MNC of patients suffering from psoriasis vulgaris: 65 +/- 14 (n = 8); psoriasis arthropathica: 63 +/- 11 (n = 8). Control values (10(5) bone marrow cells plus 10(6) MNC of healthy donors): 116 +/- 23 (n = 10). Statistical differences: psoriasis vulgaris: p less than 0.001; psoriasis arthropathica: p less than 0.001. The mechanisms responsible for these abnormalities in hematopoiesis are still unknown. Including a monoclonal antibody against the secretory component (SC) of the secretory IgA (SIga), we have tried to characterize the IgA in psoriasis. With help of a polyclonal antiserum we found depots of IgA in the skin in 69% of all investigated patients. There was a correlation between the degree of akanthosis and the amount of intraepidermal IgA.
