This analytical path is oriented to identify the critical issues and possible strategies aimed not only to overcome them, but also to implement the current management through organizational tools and alternative management models. In this evolution, an important role is played by the funding deriving from the PNRR intended to implement both territorial medicine and digital innovation, as main axes of the path to improving therapeutic adhesion. The conclusions concern the application of organizational models ( one health, change management, value based health) able to channel these resources towards this area of therapeutic adherence. In the current phase of planning and financing from PNRR mission 6, italian institutions such as AGENAS and the Ministry of Health have initiated the reform of the D.M. n. 70/2015 through outline the lines of development of social protection to support social assistance adherence therapeutic. Their symptoms vary from being asymptomatic and incidentally found to non-specific, presenting as lower abdominal pain, nausea and vomiting, constipation and diarrhea. Clinically, these abdominal masses may be palpable in more than 50% of patients. The diagnosis of these lesions can be made accurately radiologically through abdominal ultrasound and CT. The treatment of choice is complete surgical resection. Methods. Clinical photography a digital camera was used Olympus SP570UZ with master software 2.0. The images obtained both in the initial phase without treatment and at the 72 hours of treatment were transferred and stored on a 4-core Sansung computer. The auto-dial adjustment option was selected. In this way, the camera selects the optimal way to take the photo shot. Analysis of the affected skin with scabs. Image J software was used to measure the area selected for the study in both cases. This measurement was made in pixels for the photographic region under study. Results. The image j software program measures the areas in pixels, to decrease in measurement error there was no calibration. That is, the measurement of the areas in pixels was maintained. The percentage ratios of the affected or non-affected areas for both left and right legs are presented in Tables. It is possible to observe the reduction of the affected area. The doubtful areas represent for the left leg 5.21% and for the right leg 30.08% after treatment. Several ABC-transporters as “P-glycoprotein (P-gp), Multidrug resistance proteins (MRP-1) and breast Cancer Resistant Protein (BCRP)” are up-regulated in human epileptogenic brain lesions of RE, however they have not been explored in Transmantle cortical dysplasia (TMCD). We describe a 13 years old boy with Refractory Epilepsy (RE) and abnormal Magneic Resonance Image (MRI) (T1, FLAIR and T2) compatible with TMCD. Clinical follow-up, images and pathologic studies were developed by routinely methods. Epilepsy surgical treatment included total lesion resection with complete seizures remission at date. Deeper brain areas related with images findings, showed features of TMCD with abnormal ballooned neurons with high accumulation of PAS+, sudanophilic and autoflourescent lipopigment (LP). Immunohistochemistry, using primary monoclonal antibodies for P-gp, MVP and BCRP proteins, showed high expression of BCRP in several ballooned-LP+ cells. In contrast, P-gp and MVP were negative and MRP-1 has not been investigated. There is always hair loss with varying degrees of scaling and erythema [1-3]. Objectives: To identify the role of soil, plant & sewage in transmission of ringworm of scalp infection. This study was carried out on Kosti basic school students who suffering from ringworm of scalp (fungal infection) in White Nile state at Sudan in 2020. Methodology, Data Analyses and Results: Due to preservation of this fungal infection among school children in Kosti basic school students, a total of fifty specimens collected from different sources to determine the source of this infection in the area of study, twenty five students who suffering from ring worm of scalp and twenty five from soil, sewage, water and trees in the period between January – December -2020.The samples collected by scraping the scalp by surgical blade and removing the hair with needle and forceps from the peripheral lesion and gathered in filter paper and closed tightly,10% KOH was used to clarify and keratinized tissues. Trees samples were collected by washing the tree’s leaves and with all soil, sewage, water were collected into container. All samples were cultured on sabouroad dextrose agar medium for one week at room temperature. After growth, lactophenol cotton blue staining was used to identify depending on the colonial morphology under microscope and tolook for color, septate, aseptate hyphae, macro, micro conidia and spores. The results showed that there was 68% of isolates from soil, sewage, water and trees were matched to scalp isolates. The soil is significant one of these when Chi-square (X2) was used (P value = 0.027) that means, the source of infection among students may be due to soil transmission. Conclusion and Recommendation: Our results highlight the epidemiology of fungal infection of Kosti basic school students. Prostate specific antigen (PSA) represents a milestone in the diagnosis and screening of prostate cancer since its introduction even considering its limitations in term of sensitivity and specificity. The widespread use of PSA often led to unnecessary biopsies and to the diagnosis of indolent cancers that do not require treatment, therefore in the era of tailored personalized medicine there is a strong need for new markers that overcome PSA and that can help to identify the patients that have clinically significant disease that must be treated. To date different urinary and serum biomarkers have been proposed in the diagnostic setting with promising results in terms of sensitivity and specificity, however, none of them have been routinely introduced in clinical practice. In this review we reported the latest evidence for prostate cancer diagnosis in terms of urinary and blood biomarkers.
