Some studies have found an association of vitamin-D with the severity of GP and observed remarkable pain improvement after vitamin D supplementation. Objectives: To assess the severity of GP and serum level of vitamin D in children and to determine the association between them. Methods: This cross-sectional study was conducted in the Department of Paediatrics, Bangabandhu Sheikh Mujib Medical University (BSMMU). Seventy children between 6-12 years were included in the study from March 2020 to August 2021. Children who fulfilled the Evans criteria of GP were enrolled and severity of GP was assessed by Wong- Baker Facies Scale in this study. Serum 25-hydroxy-vitamin-D [25(OH)D] levels were measured and subsequently their association with the severity of growing pain were determined. Results: In this study, 97.14% of growing pain patients had hypovitaminosis D and among them majority (87.14%) were vitamin D deficient. Moderate intensity of pain was found in 51.4% of children followed by severe (30%) and mild (18.6%) intensity of pain. Severity of pain was significantly associated with lower serum levels of vitamin D. Conclusion: It may be concluded that most of the children with GP had hypovitaminosis D and severity of pain significantly increased with the decrement of serum vitamin D level. Patients with Waldenstrom macroglobulinemia may experience fatigue, unexplained weight loss, enlarged lymph nodes, abdominal swelling, weakness, nose bleeding, blurring of vision etc. Waldenstrom macroglobulinemia is a disease of elderly individuals. The median age at diagnosis is 65 years, with a slight male predominance. We reported a case of a 35-yearold male who presented with fever, abdominal pain, loose motion and epistaxis. Examination revealed cervical and inguinal lymphadenopathy with hepatosplenomegaly. Investigation revealed diffuse lymphocyte infiltration on bone marrow study . Protein electrophoresis showed monoclonal gammopathy with high serum IgM level. On the basis of clinical features and investigation reports finally we diagnosed that, this was a rare case of Waldenstrom macroglobulinemia in a young patient. The patient was transfused 5 units of whole blood and was treated following standard treatment protocol with dexamethasone, Bortezomib and G-CSF. Unfortunately, patient died 3 weeks after initiation of treatment. It has somewhat greater failure rates than oral contraceptives but is otherwise safe. IUCD is used by 85 million women worldwide. A missing device may indicate unrecognised string expulsion or retraction into the cervix/uterus or IUCD perforation at an extra uterine region. Many problems are observed with IUCD. If IUCD perforates the uterine wall and enters the peritoneal cavity, consequences may be asymptomatic or severe. We report a case of a lost IUCD found trapped in the sigmoid colon wall 4 years after insertion and presenting with chronic constipation in a 23-year-old woman. A CT scan of the abdomen was performed to identify constipation and chronic backache. The IUCD was collected during colonoscopy after bowel preparation. The patient recovered without difficulties. Due to its peculiar presentation, missing IUCD may not require extensive workup. In this rare case, the endoscopic method of retrieving IUCD saved the patient from a main laparotomy. It is quickly progressive, and death occurs within 10 years. It is a glycogen metabolism disorder characterized by the presence of inclusion bodies, known as Lafora bodies within the cytoplasm of the skin, liver, breast and muscle. Lafora disease presents as a neurodegenerative disorder with difficult to control seizures mostly like progressive myoclonic epilepsy. But the disease may also present with multiple types of seizures like generalized tonic clonic, focal with secondary generalization, myoclonic, absence etc. Patient may also present with psychomotor regression with ataxia, dysarthria, dementia, visual hallucination etc. Electroencephalogram (EEG) shows generalized spike/ polyspikes and waves with photosensitivity and background slowing. Diagnosis is further confirmed by presence of inclusion bodies (Lafora body) with typical histological findings on skin biopsy and genetic testing. Here we present a case of Lafora disease that presented with progressive myoclonic epilepsy and generalized tonic clonic seizure. We took proper history, did meticulous clinical examination and investigated her at our institute. We confirmed the patient having Lafora disease with typical histological findings that is presence of Lafora body on skin biopsy taken from axilla. Then treatment was given to the patient accordingly and proper counseling was done about the disease and its prognosis. VATS has been evaluated to replace thoracotomy for various lung resection surgeries. This study aims to determine the efficacy, safety, and outcome VATS for bullectomy. Method: This retrospective study was performed from January 2021 to December 2022 in the department of Thoracic Surgery at National Institute of Diseases of the Chest & Hospital, Dhaka and Green Life Hospital, Dhaka. We reviewed all the patients who underwent bullectomy through VATS within this period. The demographic and operative variables were collected. The safety and efficacy of the procedure in terms of postoperative complications and change in lung function after bullectomy was analysed. Result: Forty patients underwent elective VATS bullectomy due either to giant bulla or pneumothorax. Among them, 36 were male with mean age 47.4 years (range 20-65 years). There was significant improvement in lung function following bullectomy. Approximately one third patients (32.5%) developed postoperative complications, majority having air leak. There was no postoperative mortality within 30 days. Conclusion: VATS resection is a safe and effective treatment for bullous lung disease with significant improvement in symptoms and lung function following surgery. In these situations sometimes the patients are asked to continue category-1 more than six months on the basis repeat fine needle aspiration cytology (FNAC) findings. This study evaluated the cause of persistence or appearance of new lymph node after anti-TB category-1. Methods: This observational study was done from January, 2013 to March, 2014 after getting ethical clearance from Bangabandhu Sheikh Mujib Medical University (BSMMU). Patients who had persistent or new lymphadenitis after completing anti-TB category-1 for six months were selected. After analysis of treatment records, patients were selected for either FNAC or biopsy with local surgical clearance. Cyto/histopathological analysis, AFB stain (LED fluorescent), AFB culture (MGIT and L-J media) and GeneXpert test was done. Follow-up of the patients were done after two weeks and six months of FNA/surgery. Results: Thirty-two patients (M/F=11/21) with mean age 25(±12) years were investigated. Altogether 26 cases (81.25%) Journal of Bangladesh College of Physicians and Surgeons Vol. 41, No. 4, October 2023 were confirmed as tuberculous lymphadenitis and six (18.75%) were non-tuberculous (03 lymphomas, 01 sarcoidosis, 01 reactive lymphadenitis and 01 pyogenic abscess). Among the TB patients, granuloma was present in 23 cases (88.46%) (FNA-07, biopsy-16). Twenty-three (88.46%) cases were GeneXpert positive, comprising 21 rifampicin sensitive and 02 rifampicin resistant. One rifampicin resistant case was confirmed by culture and the remaining 25 were culture negative. One case was AFB stain positive and no NTM was found in AFB culture. After surgery, rapid healing was observed within two weeks among tuberculous lymphadenitis patients. Conclusions: Persistent granuloma after anti-TB category-1 does not indicate active tuberculosis if it is culture negative. It may occur due to immunological reaction against dead bacilli in deep seated sinus. Local surgical clearance results rapid recovery and other diagnosis should be excluded without further continuation of anti-TB category-1. Definitive diagnosis is sometimes difficult to establish because of clinical symptoms similar to other abdominal diseases. We report a 22-year-old woman, nullipara, with acute lower right abdominal pain for 14 hours before coming to an emergency room. On examination, the abdomen was soft but tender over the right iliac fossa and rebound tenderness were present.Ultrasound radiological examination showed that the right ovary is grossly enlarged (volume- 98.4cc) with hyperechoic central stoma. On colour, doppler twisted pedicel was noted. Mild collection is also seen in the pouch of Doglus. During laparoscopy, we found a blackish-red cystic mass of about 12 × 11 cm experiencing torsion. Then salpingooophorectomy was done. This is an interesting case of torsion in an otherwise-normal ovary with a huge hematosalpinx.
