C.laurentii is rare of the non neoformans Cryptococci that can cause human infections. Incidence of human infections with Cryptococcus laurentii and albidus has increased over last couple of decades. The risk factors for C. laurentii infections usually include diminished Cell mediated immunity and presence of invasive devices, while mortality risks increases with CNS involvement and age above 45 years. Clinical manifestations are indistinguishable from other invasive yeast infections. High index of suspicion with any yeasts associated with raised inflammatory biomarkers and presence of neutrophils in the specimen’s stained smear is important in the diagnosis and prompt initiation of appropriate treatment since C. laurentii shows low susceptibility to fluconazole. We report here a case of lower respiratory infection due to Cryptococcus laurentii in a 57 years old female patient with bronchiectasis. The patient was admitted with complaints of fever, dyspnoea with productive cough. She was on antibiotic treatment. Her chest x-ray report revealed infiltrative opacities at right upper mid zone of the lungs and pleural effusion at left lower zone. {"title":"Evaluating the impact of audit interventions on accidental removal of critical care devices in the intensive care unit - Clinical Audit Project","authors":"A. The study aimed to understand possible causes of such events and identify interventions that reduced their occurrence. The researchers conducted a single-center audit by collecting patient data and bundle forms for accidental device removal across two consecutive periods; they retrospectively reviewed the data from the first period (August 1, 2019 to January 31, 2020) and prospectively analyzed the data from the bundle forms obtained in the second (February 1, 2020 to July 31, 2020). From the findings of the first period, the researchers designed an intervention comprising nurses’ adherence to a care bundle checklist and an educational campaign for the care-taking team and applied it in the second period. Patients either accidentally removed the central venous lines secondary to agitation (47%), or it happened by loss of catheter securement (21%), or during daily care (17%) or patient transfer (13%). Such inadvertent incidents resulted in reinsertion with another central venous line (69%), agitation due to sedation interruption (47%), development of hemodynamic instability because of interruption of inotrope administration (30%), significant bleeding that required intervention (21%), and no complications (39%). The overall nurses’ compliance to the care bundle checklist improved from 87% to 97% after introduction of the intervention and the number of devices found in place increased. Diabetes care services are presently unavailable in PHC facilities, except the screening programs that are carried out by non-governmental organizations. This implies there is issue of behavioural change wheel (BCW) of the stakeholders including the quartet of ministry of health, hospital management board, healthcare professionals (HCP) and diabetes patients. This narrative review investigates perception of stakeholders towards scaling-up of a diabetes context of PHC agenda in Delta State Nigeria. The study utilized data from ongoing studies including opportunistic and purposive sampling designs. Data from published literatures on Nigeria were also reviewed. The various stakeholders’ perceptions is that motivation is poor across board. The ministry of health as well as the hospitals management board, but not patients, have the capacity. Suggested ways of improving BCW in the state include increased allocation for the ministry of health, and retraining healthcare professionals. Others improvement measures including tackling socio-cultural beliefs, and taboos are suggested. Scaling-up and sustaining diabetes care program at the PHC level in Delta State is agreed to be the responsibility of all stakeholders. Even their basic human rights are not provided to them; these resulted several psychological and physiological health problems. The purpose of this paper is to narratively review the health problems/issues inherent in forced marriage and enunciate the roles of the public health care in ameliorating them. There was review of the health effects of early marriage grouped into psychological, pathophysiological, antenatal malaria and socio-cultural injustice, recommendations on how the public health-care roles can be useful tools to combating these unhealthy practices fostered by obsolete traditional beliefs and gross ignorance from both the victims and their parents. From the review of the previous studies, though no previous study has been documented in my locations for the study, there were great negative impactful health effects of early marriage on women, it was also obvious that the public health care providers can be vital in controlling or reducing these age-long anomalies. The grave dangers of early marriage on women were elucidated, its prevalence, the adverse consequences on the women as obviously observed were critically examined with enthusiasm and concerns. The agency for PHC in Nigeria is the National Primary Health Care Development Agency (NPHCDA). The objective of this narrative review is to identify the role of PHC providers in promoting the use of ITNs among pregnant mothers and care givers of children under five years. Method was by review of 30 published literatures on PHC; as it relates to use of ITNs. Findings indicate that most pregnant mothers and care-givers of children under five years in Nigeria are aware that the best option to prevent malaria infection is by use of ITNs. Healthcare providers participate in mass distribution of ITNs. There is increase nets ownership in households. Supervisory guidance influences use of ITNs. However, there is evidence of slow uptake of ITNs due to resistance to behavioural change. Also, the knowledge of pregnant women concerning ITNs use was good, but the attitudes and practices were poor. The implication is that health promotion need to be enhanced among both PHC workers and community leaders on use of ITNs.
