It is now recognized as a potent risk factor for atherosclerotic cardiovascular disease, with emerging associations across a broader spectrum of cardiovascular phenotypes, including myocarditis, pericarditis, arrhythmias, valvular heart disease, and heart failure. The 2024 American Heart Association and American Stroke Association guideline for primary prevention of stroke emphasizes female-specific factors that increase risk for stroke, including adverse pregnancy outcomes, premature and early menopause, endometriosis, and certain hormone therapies (eg, combined hormonal contraceptives). However, losing this much weight remains a challenge for most patients, especially with lifestyle modifications alone. Moreover, not all patients with MASH are overweight or obese. Finerenone reduced the composite risk of hospitalization for heart failure or cardiovascular death by 16% relative to placebo (rate ratio 0.84, <i>P</i> = .007), primarily by lowering admissions for heart failure.
