Since the mandibular nerve block is mostly performed by dentists, this complication is mostly seen after an intraoral mandibular nerve block. The mandibular nerve is the third division of the trigeminal nerve. It is the most caudal and lateral part of Gasser's ganglion. It arises from the middle cranial fossa through the foramen ovale. In this region, a block method, which is performed by passing through the coronoid process, has been defined. This block, usually made using anatomical markers, is used in the treatment of trigeminal neuralgia. A 42-year-old female patient was admitted to our department for a maxillary and mandibular block with a diagnosis of trigeminal neuralgia. Immediately after the administration of the local anesthetic, the patient described a complete loss of vision. The complaint of vision loss lasted for about 1 minute, after which the patient's complaint of diplopia continued for 2 hours and 10 minutes. {"title":"Treatment of fibromyalgia among patients with mastalgia and fibromyalgia improved mastalgia.","authors":"Tonguç Utku Yılmaz, Murat Inanır, Saffet Çınar, Sertaç Ata Güler, Nihat Zafer Utkan","doi":"10.14744/agri.2022.58219","DOIUrl":"10.14744/agri.2022.58219","url":null,"abstract":"<p><strong>Objectives: </strong>The aim of this study is to examine the effect of fibromyalgia (FM) treatment on mastalgia by performing fibromyalgia screening in patients who applied for mastalgia and whose underlying cause could not be found.</p><p><strong>Methods: </strong>Patients who applied to Kocaeli University General Surgery Outpatient Clinic between November 2017 and November 2020 with breast pain were included (n=120). Patients without cancer, systemic disease, previous breast surgery, and breast mass larger than 3 cm (n=30) were referred to the Physical Therapy and Rehabilitation Outpatient Clinic. A total of 13 patients (43%) were diagnosed with FMS. Twelve of them were given selective serotonin-noradrenaline reuptake inhibitor (duloxetine) treatment for 3 months. Turkish version of the Short Form - 36 (SF-36) quality of life scores, Visual Analog Scale (VAS), Cardiff breast pain score before and after treatment were compared. The remaining 17 patients were followed as only mastalgia.</p><p><strong>Results: </strong>Patients with fibromyalgia and mastalgia had similar demographic results. At the end of the 3rd month, the complaints of breast pain completely regressed in all of the patients. Statistically significant changes were detected in VAS score, the number of trigger points, and SF-36 quality of life scores, Cardiff breast pain score after duloxetine treatment.</p><p><strong>Conclusion: </strong>In the presence of unexplained mastalgia, fibromyalgia should be kept in mind. The effects of RF on nerves and the underlying mechanism of these effects are not clearly understood. The aim of this study is to show the effects of Pulsed (P-RF) and Continuous (C-RF) RF in light and electron microscopy, and to determine the differences between them.</p><p><strong>Methods: </strong>In this study, a total of 60 Rattus norvegicus rats were used in 6 groups. No procedure was performed on the control group. In the Sham group, the electrodes were placed but no current was applied. P-RF for 120 seconds, P-RF for 240 seconds, C-RF for 120 seconds, and C-RF for 240 seconds at 42 °C were applied respectively to the other groups. Sections obtained from sciatic nerves were examined with light and electron microscopy.</p><p><strong>Results: </strong>Examinations of the Sham, P120, and C120 groups were normal. In P240, some morphological changes were observed, but when all samples were examined, these abnormalities were evaluated as negligible. In C240, severe deformation of both myelinated and non-myelinated nerve fibers was observed under an electron and light microscope. Dramatic structural deformities in Schwann cells were observed.</p><p><strong>Conclusion: </strong>P120, P240, and C120 treatments did not produce any deformities in the sciatic nerve. {"title":"Effect of erector spinae plane block on postoperative intravenous morphine consumption in open subcostal nephrectomy: A prospective randomized clinical trial.","authors":"Erkan Atıcı, Zehra İpek Arslan, Hasan Yılmaz","doi":"10.14744/agri.2022.71602","DOIUrl":"10.14744/agri.2022.71602","url":null,"abstract":"<p><strong>Objectives: </strong>We began with the hypothesis that the erector spinae plane block (ESPB) would decrease postoperative morphine consumption in patients scheduled for open subcostal nephrectomy.</p><p><strong>Methods: </strong>After obtaining ethics committee approval and informed patient consent, American Society of Anesthesiologists I-II, 46 patients between the ages of 18 and 65 who were scheduled for elective nephrectomy with an anterior subcostal incision were included in this study. Patients were randomly divided into two groups, the ESPB and the control group, using the sealed envelope technique. In the ESPB group, ESPB was applied with 20 mL of 0.25% bupivacaine at the T10 level at the block corner before being taken to the operating room. Patient-controlled analgesia with intravenous morphine was applied to both the ESPB and the control groups.</p><p><strong>Results: </strong>Intraoperative remifentanil consumption in the ESPB group was statistically significantly less than in the control group (1069.5±211.54 micrograms versus 1471.4±202.21 micrograms) (p<0.001). Postoperative morphine consumption of the patients was also lower in the ESPB group (16.8±4.13 milligrams versus 33.65±6.91 milligrams) (p<0.001). The numeric rating scales of the patients in the ESPB group were lower than in the control group (p<0.001). The additional analgesic requirements of patients were less in the ESPB group (35% vs 95%, p<0.001). Patient satisfaction was higher in the ESPB group compared to the control group (p=0.009). {"title":"The relationship between disability and depression, anxiety, and sleep quality in patients with coccydynia.","authors":"Emel Güler, Hanzade Aybüke Ünal","doi":"10.14744/agri.2023.23865","DOIUrl":"10.14744/agri.2023.23865","url":null,"abstract":"<p><strong>Objectives: </strong>In this study, our aim was to investigate the relationship between sleep quality, anxiety, depression, and disability in patients with coccydynia.</p><p><strong>Methods: </strong>This prospective cross-sectional study evaluated 60 patients diagnosed with coccydynia. Clinical and demographic characteristics of the patients were recorded. Outcome measures included the Visual Analogue Scale (VAS), Oswestry Disability Index (ODI), Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI), and Pittsburgh Sleep Quality Index (PSQI), which were analyzed.</p><p><strong>Results: </strong>Depressive symptoms and anxiety were detected in 47 (78.3%) and 49 (81.6%) of the 60 patients, respectively. Additionally, 46 (76.7%) patients were identified as poor sleepers. Our results demonstrated a statistically significant positive correlation between the quality of sleep and anxiety and depressive symptoms scores (p<0.001); there was also a statistically weak positive correlation with disability index values.</p><p><strong>Conclusion: </strong>Depressive symptoms, anxiety, and poor sleep quality are exacerbated by disability in coccydynia.
