Computerised tomographic imaging revealed an anterior mediastinal mass and a left adrenal mass. At operation, a large mediastinal mass with extensive invasion into surrounding structures, a non-contiguous sternal metastasis, and a left adrenal metastasis were found. Analysis of frozen section confirmed metastatic thymoma. Aggressive surgical resection of primary and metastatic lesions was undertaken, followed by adjuvant chemotherapy with cisplatin, adriamycin and cyclophosphamide. Despite the presence of haematogenous metastatic disease, the patient was free of disease 36 months later. Some sternal breakdown is associated with wires cutting through the sternum. In an attempt to avoid this, we employed broad, strong tapes for sternal closure to determine the efficacy of this technique in our hands. A total of 94 patients were prospectively entered into this study. There were 2 early deaths; the sternums were intact at the time of death. There were 4 complications in the remaining 92 patients. Three patients had would infections superficial to intact sternums. One patient had sternal dehiscence associated with snapping of the tapes. In no case did the tapes cut through the sternum. The second objective was to determine whether the internal thoracic artery (ITA) grafts in patients aged more than 70 years were safe and beneficial. Patients were divided into 2 groups: “elderly” (125 patients aged 70 years or more) or “younger” (765 patients younger than 70 years). Early and long-term results and the incidence of postoperative complications were compared. The elderly group was subdivided into 96 patients in whom ITA grafts were used for CABG (elderly ITA) and 29 patients in whom only saphenous vein grafts (SVG) were used (elderly SVG). Hospital mortality was higher in elderly patients than in younger patients. There was no significant difference in hospital mortality between elderly ITA and elderly SVG patients. The incidence of postoperative complications was higher in elderly patients than younger patients. When comparing postoperative surgical complications between elderly ITA and elderly SVG patients, no significant differences were noted. Although the mortality after CABG was slightly higher in elderly patients than in younger patients, most of the elderly long-term survivors were able to lead ordinary lives. It causes left ventricular inflow obstruction and mimics mitral stenosis. The 2 conditions have rarely been reported concurrently. This report describes the case of a 60-year-old women with rheumatic mitral stenosis and single vessel coronary disease, in whom cor triatriatum was diagnosed coincidentally at echocardiography.
