The frequency of drug requests and dispensation was evaluated and the consumption of each active ingredient of the preparations was expressed as number of “infant defined daily dose” (iDDD) and of iDDD/100 bed-days.</p></div><div><h3>Results</h3><p>A total of 657 forms were analyzed – a monthly average of 27 pediatric preparations. The neonatal ICU accounted for 69.6% of these requests. Twenty-one drug items were used, of which the most common were folinic acid (88 requests), sulfadiazine (85) and captopril (73). Vieira , Isadora Carolina Krueger Negrelle , Karla Ulaf Webber , Marjorie Gosdal , Sabine Krüger Truppel , Solena Ziemer Kusma","doi":"10.1016/j.rppede.2016.06.003","DOIUrl":"10.1016/j.rppede.2016.06.003","url":null,"abstract":"<div><h3>Objective</h3><p>To evaluate the pediatrician's knowledge regarding the diagnostic and therapeutic approach of childhood functional constipation.</p></div><div><h3>Methods</h3><p>A descriptive cross-sectional study was performed with the application of a self-administered questionnaire concerning a hypothetical clinical case of childhood functional constipation with fecal incontinence to physicians (n=297) randomly interviewed at the 36th Brazilian Congress of Pediatrics in 2013.</p></div><div><h3>Results</h3><p>The majority of the participants were females, the mean age was 44.1 years, the mean time of professional practice was 18.8 years; 56.9% were Board Certified by the Brazilian Society of Pediatrics. Additional tests were ordered by 40.4%; including abdominal radiography (19.5%), barium enema (10.4%), laboratory tests (9.8%), abdominal ultrasound (6.7%), colonoscopy (2.4%), manometry and rectal biopsy (both 1.7%). The most common interventions included lactulose (26.6%), mineral oil (17.5%), polyethylene glycol (14.5%), fiber supplement (9.1%) and milk of magnesia (5.4%). The weight and height of children were measured in the day care centers in 2009, 2010 and 2011. The body mass index <em>z</em>-score (BMIz) was calculated and children were classified as risk of overweight (BMIz≥1 to<2) or excess weight (BMIz≥2). Data analysis was carried out by comparison of proportions, coefficient of correlation and multivariate linear regression.</p></div><div><h3>Results</h3><p>The prevalence of risk of overweight was 18.9% and of excess weight (overweight or obesity) was 9.3%. The median duration of breastfeeding and age of introduction of non-breast milk was 6 months. The child's BMIz showed direct correlation with birth weight (<em>r</em>=0.154; <em>p</em><0.001) and maternal body mass index (BMI) (<em>r</em>=0.113; <em>p</em>=0.002). The correlation was inverse with the total duration of breastfeeding (<em>r</em>=−0.099; <em>p</em>=0.006) and age at non-breast milk introduction (<em>r</em>=−0.112; <em>p</em>=0.002). We analyzed the self-assessed sexual maturation level (prepubertal, pubertal and post-pubertal), the economic level (high and low) through a questionnaire and body fat (normal and high) through triceps and subscapular skinfolds. The 20-meter back-and-forth test was applied to estimate maximum oxygen consumption. Cardiorespiratory fitness was assessed according to reference criteria and considered low when the minimum health criterion for age and sex was not met. Chi-square test and logistic regression were applied, with a significance level of 5%.</p></div><div><h3>Results</h3><p>The prevalence of low cardiorespiratory fitness was 51.3%, being associated with all study variables (<em>p</em><0.001). At the crude analysis, adolescents with high body fat were associated with low cardiorespiratory fitness, when compared to those with normal body fat (<em>OR</em>=2.76; 95%CI: 2.17–3.52). After adjustment by sexual maturation, this association remained valid and showed an effect that was 1.8-fold higher (95%CI: 1.39–2.46) and after adjusting by economic level, the effect was 1.9-fold higher (95%CI: 1.45–2.61).</p></div><div><h3>Conclusions</h3><p>Approximately half of the assessed girls showed unsatisfactory levels of cardiorespiratory fitness for health, which was associated with high body fat, regardless of sexual maturation level and economic level. Location: 31 schools of the Regional Education Board–District of Penha, East Side the city of São Paulo. The children's care needs according to the caregivers were assessed in three areas–self-care, mobility and social function, using the Pediatric Evaluation of Disability Inventory, according to the following score: 5, Independent; 4, Supervision; 3, Minimum Assistance; 2, Moderate Assistance; 1, Maximum Assistance and 0, Total Assistance. Vaccination was considered as the exposure factor and hospitalization for community-acquired pneumonia as the endpoint, using secondary annual data by municipality. The prevalence ratio and its 95% confidence interval (95%CI) were used to verify the association between variables. The double-blind assessment of cough was through the number of observed coughing episodes and intensity score for a period of 10minutes of observation. The decrease of one point in the mean total score was considered as a therapeutic effect.</p></div><div><h3>Results</h3><p>There was a reduction in coughing episodes in both groups, as well as in the cough score after 30minutes of drug or honey administration. The change in clinical score above two points, which could indicate marked improvement, occurred in five patients in the bromelin group and only in one in the placebo group, but without significant difference. There were no adverse events.</p></div><div><h3>Conclusions</h3><p>The immediate improvement rate of irritative cough was similar in patients treated with combination of <em>Ananas comosus</em> extract and honey (Bromelin<sup>®</sup>) compared with the use of honey alone (placebo group). {"title":"Severe protein-calorie malnutrition in two brothers due to abuse by starvation","authors":"Marcela Montenegro Braga Barroso, Luiza Martins Salvador, Ulysses Fagundes Neto","doi":"10.1016/j.rppede.2016.06.002","DOIUrl":"10.1016/j.rppede.2016.06.002","url":null,"abstract":"<div><h3>Objective</h3><p>To describe the case of two siblings with severe protein-calorie malnutrition due to abuse by starvation.</p></div><div><h3>Cases description</h3><p>The two patients were simultaneously referred to the Hospital Municipal, where they were admitted to the Pediatric Gastroenterology clinic of a university hospital for diagnostic investigation of the cause of severe malnutrition and screening tests for Celiac Disease, Cystic Fibrosis and Environmental enteropathy among others. The exams were all normal, and after detailed research on the interactions of this family, we reached the conclusion that the malnutrition was due to abuse by starvation. The intervention was performed by medical students dressed as clowns and included magic tricks, juggling, singing with the children, making soap bubbles and comedic performances. The intervention time was 20min. Vital signs were assessed in two measurements with an interval of 1min immediately before and after the interaction. Non-verbal communication was observed before and during the interaction using the Non-Verbal Communication Template Chart, a tool in which non-verbal behaviors are assessed as effective or ineffective in the interactions.</p></div><div><h3>Results</h3><p>The sample consisted of 41 children with a mean age of 7.6±2.7 years; most were aged 7–11 years (n=23; 56%) and were males (n=26; 63.4%). There was a statistically significant difference in systolic and diastolic blood pressure, pain and non-verbal behavior of children with the intervention. {"title":"Relation between safe use of medicines and Clinical Pharmacy Services at Pediatric Intensive Care Units","authors":"Lucas Miyake Okumura , Daniella Matsubara da Silva , Larissa Comarella","doi":"10.1016/j.rppede.2016.04.001","DOIUrl":"10.1016/j.rppede.2016.04.001","url":null,"abstract":"<div><h3>Objective</h3><p>Clinical Pharmacy Services (CPS) are considered standard of care and is endorsed by the Joint Commission International, the American Academy of Pediatrics, and the American College of Clinical Pharmacy. In Brazil, single experiences have been discreetly arising and the importance of these services to children and adolescents care has led to interesting results, but certainly are under reported. This short report aims to discuss the effect of implementing a bedside CPS at a Brazilian Pediatric Intensive Care Unit (PICU).</p></div><div><h3>Methods</h3><p>This is a cross-sectional study conducted in a 12 bed PICU community hospital, from Campo Largo/Brazil. Subjects with<18 years old admitted to PICU were included for descriptive analysis if received a CPS intervention.</p></div><div><h3>Results</h3><p>Of 53 patients accompanied, we detected 141 preventable drug-related problems (DRPs) which were solved within clinicians (89% acceptance of all interventions). The most common interventions performed to improve drug therapy included: preventing incompatible intravenous solutions (21%) and a composite of inadequate doses (17% due to low, high and non-optimized doses). Among the top ten medications associated with DRPs, five were antimicrobials. By analyzing the correlation between DRPs and PICU length of stay, we found that 74% of all variations on length of stay were associated with the number of DRPs.</p></div><div><h3>Conclusions</h3><p>Adverse drug reactions due to avoidable DRPs can be prevented by CPS in a multifaceted collaboration with other health care professionals, who should attempt to use active and evidence-based strategies to reduce morbidity related to medications.</p></div>","PeriodicalId":101120,"journal":{"name":"Revista Paulista de Pediatria (English Edition)","volume":"34 4","pages":"Pages 397-402"},"PeriodicalIF":0.0,"publicationDate":"2016-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1016/j.rppede.2016.04.001","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"55274909","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
