Martín Comín","doi":"10.1016/S1578-200X(10)70001-2","DOIUrl":"https://doi.org/10.1016/S1578-200X(10)70001-2","url":null,"abstract":"<div><p>The efficacy of carotid endartectomy (CE) in cerebrovascular accidents produced by carotid stenosis can be evaluated with the cerebral perfusion with single photon emission tomography, using statistical parametric mapping (SPM).</p></div><div><h3>Material and methods</h3><p>Twelve patients with significant carotid stenosis who underwent endartectomy were included in the study. All underwent a cerebral perfusion study with <sup>99m</sup>Tc-ethylene dicysteine diethyl ester (ECD) at baseline and after the endartectomy. Using SPM, the baseline/post-surgery study was compared independently with the control group made up of 20 patients and the corresponding parametric statistical mappings were obtained. Changes in extension (k<sub>E</sub> or voxel number) and intensity (change in the T value) of the significantly hypoperfused zones and direction of these changes were evaluated.</p></div><div><h3>Results</h3><p>In the group of 12 patients, improvement of the post-surgical cerebral perfusion was observed in 5 patients with an average 50.56% decrease in the extension of the hypoperfused zones and average 30.9% decrease of intensity. Four patients showed an average 85.53% increase in the extension of cerebral hypoperfusion and of 34.21% in intensity. The multidisciplinary team must do it with rigor and demonstrate that they have fulfilled some essential criteria.</p></div><div><h3>Objective</h3><p>To evaluate the quality of the procedure in the Breast Pathology Unit of Hospital Puerta del Mar, using the self-evaluation guidelines of the Spanish Society of Senology and Breast Pathology.</p></div><div><h3>Material and method</h3><p>These guidelines establish some objective and measurable evaluation criteria related to the aspects that intervene in the SLNB. Each criterion is associated to 4 levels of quality standards. There are 3 essential criteria of mandatory fulfillment to implement SLNB in the clinical practice. They evaluate care experience (6 criteria), operative aspects (18 criteria, 3 essential) and scientific activity (8 indicators).</p><p>The initial validation phase included 50 patients and the application phase 120 patients (February 2009).</p></div><div><h3>Results</h3><p>Three essential criteria related to the detection of the sentinel node (98%), accredited sensitivity (90%), and mean number of axillary sentinel nodes (2.1) were fulfilled. The application of self-evaluation guidelines in our Unit obtained a global score of 39 out of 71 points, quality level II being the most prevalent.</p></div><div><h3>Conclusion</h3><p>During the validation phase, our unit fulfilled the criteria established to implement the procedure. García-Dorado","doi":"10.1016/S1578-200X(10)70046-2","DOIUrl":"https://doi.org/10.1016/S1578-200X(10)70046-2","url":null,"abstract":"<div><h3>Objective</h3><p>The purpose of this study was to analyze left systolic ventricular function and myocardial perfusion characteristics between short one day exercise-rest and long two days gated SPECT (Single Photon Emission Computed Tomography) protocols in patients with ischemic cardiomyopathy (ICM).</p></div><div><h3>Methods</h3><p>A prospective study of 40 patients (59.6<!--> <!-->±<!--> <!-->8.9 years, 3 women) with IHD (left ventricular ejection fraction (EF) ≤<!--> <!-->40%) was performed with myocardial perfusion gated SPECT. From 5 to 10 days after a one-day exercise-rest study (gated SPECT-1), patients were called back for a second rest study (gated SPECT-2) in order to compare EF and differences in perfusion summed rest score (ΔSRS=SRS1-SRS2) and summed difference score (ΔSDS=SDS1-SDS2) between both protocols.</p></div><div><h3>Results</h3><p>Between rest-gated SPECT-1 (short protocol) and rest-gated SPECT-2 (long protocol) EF increased (34% vs 37%, P<!--> <!-->=<!--> <!-->0.008) in 26 patients (65%), and in 11 patients (27.5%) the increase was ≥<!--> <!-->5%. There were no significant differences in clinical and coronary angiography variables between patients with and without increase of the EF ≥<!--> <!-->5%. In the multivariate analysis, ΔSRS (95% CI: −1.1 to −29.2) and ΔSDS (0.179–1.236) were predictors for this EF increase between both studies.</p></div><div><h3>Conclusions</h3><p>Exercise-rest short protocol can underestimate EF in patients with ICM. His risk was stratified using the myocardial perfusion gated SPECT, showing mild inferior ischemia. Thus, medical therapy was optimized and the patient was discharged. He continued with exertional dyspnea so a coronary CT angiography was performed. It revealed severe lesions in the proximal RCA. SPECT-CT fusion images correlated the myocardial perfusion defect with a posterior descending artery from the RCA, in a co-dominant coronary area. Subsequently, cardiac catheterism was indicated for his treatment.</p><p>The current use of image fusion studies is limited to patients in whom it is difficult to attribute a perfusion defect to a specific coronary artery. To solve this problem, available softwares include myocardial edge delimitation.</p></div><div><h3>Purpose</h3><p>To evaluate the influence of ECA in automatic myocardial edge detection under normal conditions and with abnormal perfusion and also evaluate the reproducibility of semi-automatic processing.</p></div><div><h3>Methods</h3><p>A total of 100 MPSs, 50 with ECA, were analyzed. Each subgroup included 25 cases with perfusion abnormalities. The cases were processed automatically and by 4 independent operators with different levels of experience. Commercial QGS and QPS softwares were used with tools to mask and relocate the left ventricle area. Functional parameters (final diastolic and systolic volumes and ejection fraction) and perfusion parameters such as the reversibility perfusion score and rest perfusion defect extension were analyzed. The data were compared with Pearson's correlation and Student's test.</p></div><div><h3>Results</h3><p>Interobserver correlation significantly worsened with the presence of ECA and was moderately affected by perfusion abnormalities. More experienced observers presented better correlation. Reproducibility was greater for the functional perfusion parameters, independently of the observer's experience.</p></div><div><h3>Conclusions</h3><p>ECA significantly affects automatic edging delimitation, affecting the MPS values. Interobserver reproducibility with manual processing was more altered regarding funtional parameters than in the perfusion scores. Perfusion abnormalities did not interfere with software reproducibility, and when present, better correlation was found. Martín-Comín","doi":"10.1016/S1578-200X(10)70010-3","DOIUrl":"10.1016/S1578-200X(10)70010-3","url":null,"abstract":"<div><h3>Objective</h3><p>To analyze the presence of anxiety in patients referred to a Nuclear Medicine Department (NMD).</p></div><div><h3>Material and methods</h3><p>A total of 148 patients were included: 67 were referred for radioiodine therapy, 48 with hyperthyroidism (HT), 19 with differentiated thyroid carcinoma (DTC), and 81 were referred for detection and biopsy of the sentinel node in breast cancer (BC).</p><p>The following documents were filled out: personal data, a state-trait anxiety inventory (STAI), a scale of pre-disposing factors causing anxiety and an information questionnaire.</p><p>Anxiety-predisposing factors and the influence of the information on the presence of anxiety were studied.</p></div><div><h3>Results</h3><p>HT patients: 47% had anxiety in the moment of the visit that was not related to the level of information received. The factor that worried them the most was the radioiodine administration. Being the first visit to a NMD significantly influenced (<em>p</em> < 0.05) on the presence of anxiety.</p><p>DTC patients: 42% had anxiety in the moment of the visit not related to the level of information received. The factor that worried them the most was the illness itself. No factor had a significant influence on the presence of anxiety.</p><p>BC patients: 53% had anxiety in the moment of the visit that was not related to the level of information received. What worried them the most were the results. Having anxiety and/or depression significantly influenced (<em>p</em> < 0.05) the presence of anxiety.</p></div><div><h3>Conclusion</h3><p>The quantity of information given before a procedure in a NMD does not influence the presence of anxiety. A very intense somatostatin avid abdominal lesion was located and radioguided surgery was proposed. Prior to the procedure, we performed a SPECT-CT allowing detection and localization of the lesion anatomically, thereby facilitating the performance of the radioguided procedure.
