González-González","doi":"10.1016/j.cali.2016.10.002","DOIUrl":"10.1016/j.cali.2016.10.002","url":null,"abstract":"<div><h3>Objective</h3><p>To assess long-term suitability of screening for type 2 diabetes mellitus in women with a previous diagnosis of gestational diabetes in Primary Care. The secondary objectives were to determine if there were clinical factors that modified the usefulness of the screening.</p></div><div><h3>Material and methods</h3><p>An observational cohort type study was performed, which included all patients with the diagnosis of gestational diabetes during the years 2000 to 2009 (n<!--> <!-->=<!--> <!-->470) in the University Hospital Complex of Ourense. The electronic medical records were reviewed to assess the existence of gestational diabetes and the year of the last fasting blood glucose. The mean follow-up time was 12.9 years. The screening for evidence of a fasting blood glucose in the last 3 years was considered adequate. The following variables were analysed: adequacy of screening for type 2 diabetes mellitus, age, body mass index, gestational diabetes in more than one gestation, and rural/urban environment. A descriptive analysis of the data was performed, using Chi2 and Student's <em>t</em>-test to determine differences between subgroups. Statistical significance was considered as <em>P</em><.05</p></div><div><h3>Results</h3><p>The long-term monitoring of these patients was very irregular. Only 67.08% of the study group underwent diabetes mellitus type 2 screening. The level of follow-up was not associated with age, BMI, the place of residence, or the year of diagnosis. Camacho-Pastor","doi":"10.1016/j.cali.2017.02.004","DOIUrl":"10.1016/j.cali.2017.02.004","url":null,"abstract":"<div><h3>Aim</h3><p>To describe the process of implementation of Individualized Care Plan in the Electronic Health Record and its impact on the University Hospital Alcorcón Foundation.</p></div><div><h3>Methodology</h3><p>Working groups of staff nurses who analyzed activities usually performed to create a catalog of diagnoses, outcomes and interventions. A group of referents that refined the catalog to make it manageable was created. A training plan, nursing assessment forms and the Nursing Discharge Report were designed.</p></div><div><h3>Results</h3><p>In February 2016 the new methodology was implemented in inpatient units of adults. Between 74.86 and 88.18% of the patients underwent a care plan with the new methodology. Between 69.41 and 76.25% of patients are discharged with a Nursing Discharge Report accordance with regulations. An increase of 24.1% of patients with Nursing Discharge Report after implantation is observed (<em>P</em> <!-->=<!--> <!-->.000; RR: 1.46; 95% CI 1.36-1.56). A total of 116 nurses has been trained.</p></div><div><h3>Conclusions</h3><p>In the study conditions, the use of nursing taxonomies has generated thinking skills and allowed nurses to issue judgments, ensure quality of care, and implementing interventions with a planned results. Participants: 134 health professionals in contact with the hospital anaesthesia department. Tool: self-administered questionnaire, measuring: accessibility, personal treatment, comfort, scientific and technical quality (scale 1 to 5), overall satisfaction (scale 0 to 10), and suggestions for improvement. A descriptive statistical and correlation analysis were performed, including mean differences (by sex, frequency of contact with the anaesthesia department, and unit), as well as a regression model.</p></div><div><h3>Results</h3><p>The quality of personal treatment received a mean of 4.2 points (SD 0.651), the scientific and technical quality 4.00 points (SD 0.532), accessibility 3.3 (SD 0.795), professional comfort 3.30 (SD 0.988), and patient comfort 2.62 points (SD 1.051). Overall satisfaction obtained a mean of 7.1 points (0 to 10 scale).</p><p>Women and professionals working in units with less than 10 people had a better general evaluation of the anaesthesia department. The worse perspective was that of staff with daily contact with the anaesthesia department. Cerezo-Sanmartin , en representación del Grupo de Trabajo de Atención Primaria del Área VII Murcia Este , Unidad de Diabetes Área VII Murcia Este","doi":"10.1016/j.cali.2016.09.005","DOIUrl":"10.1016/j.cali.2016.09.005","url":null,"abstract":"<div><h3>Purpose</h3><p>The aim of this study was to evaluate the efficiency of a joint intervention that included educational components, self-assessment, and information to optimise diabetes control through the introduction of instant capillary glycosylated haemoglobin (HbA1c) determination in Primary Care.</p></div><div><h3>Materials and methods</h3><p>A multicentre prospective descriptive study was carried out over 3<!--> <!-->years in 10<!--> <!-->Primary Care Centres of the Area VII Murcia East. At the end of the study there were 804 patients with type 2 diabetes (DM2). Patients were divided into 4 groups based on initial values of HbA1c, and if changes in their treatment were needed. HbA1c, body mass index, and blood pressure were monitored. A financial assessment was also performed on the impact of the implementation of a protocol to measure instant capillary</p></div><div><h3>Results</h3><p>A significant reduction was observed in HbA1c values. The initial HbA1c mean value was 7.4<!--> <!-->±<!--> <!-->1.4%, which decreased to a final value of 6.9<!--> <!-->±<!--> <!-->1.0% (<em>P</em><.001). At the end of the study, 71.4% of patients included reached diabetic control objectives. In addition, the financial assessment demonstrated that the implementation of this diabetes control system led to a decrease of the 24.7% in spending on glucose strips after the first year of study in Area VII Murcia Health Service.</p></div><div><h3>Conclusions</h3><p>The introduction of capillary HbA1c determination in Primary Care has demonstrated to improve diabetes control and the efficiency of the health personnel.
