The patients required nipple elevation of 3-6 cm and had unpleasing result after circumareolar mastopexy were treated with the technique of circumvertical lift with breast suspension and internal shaping with inferior dermal-glandular flap which pedicle was on the inframammary crease and tack technique for removing excessive skin precisely. \u0000 \u0000 \u0000Results \u000033 cases had their drainage tubes removed at the 2~3 days and suture removed at the 12~14 days postoperatively. All had primary healing of incision and no complications such as necrosis of NAC and dehiscence of incision occurred. Elastic bra was asked to wear for three to six months and then stable shape was achieved. Follow-up lasted for 6-60 months. They were randomly divided into two groups (group A and group B) and both underwent splitface therapy. Each group was randomly divided into two subgroups. Group A was administered by microneedle + PRP (A1) on one side and by microneedle + saline (A2) on the other. In group B, one cheek was treated with ID PRP (B1) and the other received ID saline (B2). Each patient received overall 3 treatments per session with 28 days intervals. Patients were followed-up six months after their last treatment and clinical evaluation was assessed by 2 independent plastic surgeons and patients themself. \u0000 \u0000 \u0000Results \u0000The results were obvious difference between each group. The score of evaluation by physicians and patients in A1 group (2.565±0.504) point showed significantly better results compared with other groups. B1 group (1.820±0.592) point was significantly higher than B2 group (1.345±0.483) point (P 0.05). The total effective rate in A1 group was 100%, which was higher than B1 group (43.75%), A2 group (81.25%) and B2 group (25%), with statistical significance (P<0.05). \u0000 \u0000 \u0000Conclusions \u0000The current study introduces the combination of autologous PRP and microneedle technique as an effective and safe modality in the treatment of facial post acne scars with short recovery time and easy operation. Any concerns of immunogenic reactions or disease transfer are eliminated because PRP is prepared from autologous blood. The perforator flap was designed according to the perforator location detected by Doppler before surgery. The flaps were harvested on the pedicle of perforators and dissected on the deep layer of deep fascia (including part of the myolemma). The areas of flap ranged from 5.0 cm × 10.0 cm to 19.0 cm × 22.0 cm. The donor site was sutured directly or local flap was transferred to repair. \u0000 \u0000 \u0000Results \u0000Flaps survived well in 59 of the 62 cases. The tip of flap was necrotic and healed after dressing change in 1 diabetic patient. The distal part of flap was necrotic on cuticular layer and healed after dressing change in another patient with scrotal defect. One patient had flap necrosis 10 days after operation and was repaired twice. The results were satisfied after following-up from 3 months to 2 years for 17 cases and from 6 months to 2 years for 25 cases. Fibrosarcoma protuberant and squamous cell carcinoma recurred in 1 case each, but the function and appearance were satisfactory after primary repair. \u0000 \u0000 \u0000Conclusions \u0000The procedure of preserving Choke vessel area can improve the blood supply of perforator flap and obtain larger survival area and survival quality. Then the ADSC was stored in 10% PRP, 10% HS, and 0.9% Nacl at room temperature (RT, about 26 ℃) for 2 hours. After that the proliferation ability, survival rate and differentiation ability of the cells were detected. Finally storage media of ADSC was determined. \u0000 \u0000 \u0000Results \u0000Compared with 0.9% NaCl, ADSC showed a better proliferation and higher differentiation ability at 10% PRP and 10% HS; besides, the survival rates of two groups were higher than the group of 0.9% Nacl. The facial morphology and the simulated effect were ontained. All patients were female and they were 18 to 39 years old. The average age was 26 years old. The outline and form were normal. We discussed with patients to confirm the ideal nasal 3D simulated effect. In the process of operation, we were guided by the data of soft measures and 3D printing model. \u0000 \u0000 \u0000Results \u0000Among 25 patients who werefollowed up postoperatively. We got intuitive, accurate 3D reconstruction graphs and simulated surgery procedures in all cases. Being applied to actual surgery procedure, 23 patients were satisfactory with postoperational results. Two patient's nasal tip dropped. Meng, Yong Xu, Hui Wang, Ying-fan Zhang, Yao-zhong Zhao","doi":"10.3760/CMA.J.ISSN.1671-0290.2019.04.006","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1671-0290.2019.04.006","url":null,"abstract":"Objective \u0000To present our experience and techniques with the use of autologous costal cartilage grafts in Asian rhinoplasty, and to report the surgical results and complications in 86 consecutive rhinoplasty cases. \u0000 \u0000 \u0000Methods \u0000All operations were performed by the first author (Liu AT) with open approach, costal cartilages and perichondrium were used to reconstruct the nasal tip projection according to the tripod theory in rhinoplasty, after removing the previous injection material, L-shaped implant or hypertrophic scar tissue in the tip. Medical charts and operative records were reviewed retrospectively to summary the complications. Nasal dorsum augmentation was done by costal cartilage or I-shaped allograft, sometimes with anterior sheath of rectus abdominis. Patients' subjective satisfaction of the postoperative nasal appearance was self-evaluated with grading (1 worse, 2 no change, 3 improved, and 4 much improved). \u0000 \u0000 \u0000Results \u0000From September 2015 to March 2017, 86 patients underwent rhinoplasty at our hospital. The postoperative follow-up duration was 6 to 20 months. Overall, functional and aesthetic outcome was satisfactory in most patients, and the mean score by the patients' self-evaluation was 3.3±0.6. A cutaneous excision (6 mm diameter) was made on the back of each animal, close to the cervical area. The dorsal skin of every rat was allocated to three groups which were treated with physiological saline, human recombinant epidermal growth factor (rhEGF), and CDPs, respectively. After making a rat model of skin incisions, we observed the wound healing process, took photos of the wounds under a digital microscope, and use sulfuric graph paper to record the size of every wound. At the 3rd, 6th, 9th, 12th day after modeling, 6 rats were killed, and mRNA expression of K10, K14, and EGF was detected in the skin tissues using a RT-PCR technique. \u0000 \u0000 \u0000Results \u0000At the 6th and 12th day after modeling, there were significant differences between the experimental group and the blank control group (P<0.05). The follicular units were arranged irregularly, 40-55 FUs/cm2, with 30°-45° angle from the forehead skin and 15°-20° angle from the temporal skin. The finest hairs were selected and arranged in hairline margin. \u0000 \u0000 \u0000Results \u0000All the patients were followed up for 10-24 months. 95 patients were very satisfied, 25 patients were satisfied, and no patients were unsatisfied. 9 patients complained low density in the middle of the hairline and they had another surgery to improve the density. 3 patients's hairline was regular, and another graft was done to correct the unnatural appearance. 6 patients's hairs of the hairline margin were thick, but no further treatment was done. 1 patient underwent hair transplantation again, because of the still high hairline. 6 patients's transplanted hairs appeared curly, and the curly degree was decreased after 1-2 years. The density of the transplanted hair was (44.7±8.1) FU/cm2. The density of 9 cases who complained low density in the middle of the hairline, was (36.9±3.1) FU/cm2. The hair diameter of the hairline edge was (57.5±13.5) μm. The hair diameter of 6 cases who felt the hair thick was (70.2±5.1) μm. Xu","doi":"10.3760/CMA.J.ISSN.1671-0290.2019.04.019","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1671-0290.2019.04.019","url":null,"abstract":"Objective \u0000To analyze the abnormalities of local chemokines in patients with vitiligo and to explore the effect of tacrolimus on the secretion of chemokines in keratinocytes. \u0000 \u0000 \u0000Methods \u0000Blister fluids of 50 patients with vitiligo were collected, including lesion areas and normal areas. Luminex was used to analyze the concentration of local chemokines in patients with vitiligo to determine whether the chemokines were closely related to vitiligo. The effect of tacrolimus on chemokine secretion of was analyzed by Western blot in HaCaT cells. \u0000 \u0000 \u0000Results \u0000By Luminex analysis of blister fluid, it was found that CXCL9 and CXCL10 were significantly higher in the leukoplakia of vitiligo, and there was a significant difference, compared with the blister fluid in the normal site (P<0.01). IFN-γ significantly stimulated the keratinocyte cell line HaCat to express CXCL9 and CXCL10. After pretreatment of HaCaT cells with 20 mg tacrolimus, the expression of CXCL9 and CXCL10 was significantly decreased, compared with the blank control (P<0.01). \u0000 \u0000 \u0000Conclusions \u0000The leukoplakia chemokines CXCL9 and CXCL10 are highly expressed in vitiligo patients.
