Le CHU Sainte-Justine a vu ainsi l’implantation de la pharmacie clinique et son développement, lequel a abouti depuis quelques années à l’émergence de soins pharmaceutiques spécialisés adaptés aux besoins des équipes de soins et aux profils des patients. Le service de gynécologie-obstétrique, appuyé notamment par la création d’une chaire pharmaceutique, a bénéficié des activités du pharmacien clinicien qui est devenu rapidement un membre incontournable pour une prise en charge thérapeutique efficiente. Nous présentons, au travers d’un cas clinique d’une patiente enceinte exposée à l’azathioprine dans le cadre de son lupus érythémateux, des exemples d’interventions pharmaceutiques pouvant être effectués par le pharmacien clinicien dans le cadre de sa démarche de soins auprès des patients et des professionnels de santé.</p></div><div><p>Beginning in the 1990s, clinical pharmacy has greatly contributed to the development of hospital pharmacy practice in North America. Clinical pharmacy was implemented at CHU Sainte-Justine to promote rational and appropriate use of drug therapy. The department of obstetrics-gynecology, supported notably by the creation of a pharmaceutical Chair, has benefited from the services of a clinical pharmacist since 1998. Cependant, peu de recommandations existent concernant la délivrance des produits sur le terrain et les expériences antérieures locales ont montré un temps de mise à disposition inapproprié aux besoins.</p></div><div><h3>Objectif</h3><p>L’équipe médico-pharmaceutique lyonnaise a conceptualisé un système de kits permettant, pour chacun d’eux, la prise en charge d’un patient pour un geste vital donné. L’objectif est d’évaluer la faisabilité et la durée de préparation de 30 kits lors de l’arrivée de l’équipe pharmaceutique au niveau du poste médical avancé.</p></div><div><h3>Résultats</h3><p>Un premier exercice a permis de tester ce dispositif auprès de dix pharmaciens et 15 préparateurs mobilisables répartis en six groupes. Le temps moyen de préparation des kits était de 45<!--> <!-->minutes.</p></div><div><h3>Discussion - Conclusion</h3><p>Des évolutions en termes de procédures, d’organisation, de matériels et de composition des malles sont apportées. Cette organisation sur le terrain présente un double intérêt : optimiser les temps de réponse par l’anticipation des besoins (objectif fixé à 30<!--> <!-->minutes) et limiter, au démarrage, une désorganisation éventuelle toujours préjudiciable.</p></div><div><h3>Introduction</h3><p>Emergency stocks (including PSM, the Mobile Emergency Units in French) have been placed in specific hospitals for many years, thus improving casualties’ assistance in case of disasters. However there are few recommendations about pharmaceuticals release on the ground and the previous local experiences showed an inappropriate time for the needs.</p></div><div><h3>Objective</h3><p>Therefore, the medical pharmaceutical team in Lyon worked out and set up a kit-system, each of those enabling assistance of one patient for one given vital gesture. The objective is to estimate the feasibility and the duration of preparation of 30 kits from the arrival of the pharmaceutical team to the Mobile Emergency Unit.</p></div><div><h3>Results</h3><p>A first exercise enabled to test this device with 10 pharmacists and 15 pharmacist assistants who can be mobilized, divided into six groups. The average time of 45<!--> <!-->minutes was necessary to set up the kits together.</p></div><div><h3>Discussion - Conclusion</h3><p>Modification of process, organization, material and composition of trunks were implemented.
