The principal indication for surgery is the psychological impact for the young adult. The modelling technique referred as osteochondroplasty with stabilization by sliding splint staplers can be selected in the three following types of deformations: <em>pectus excavatum</em>, <em>pectus carinatum</em> and <em>pectus arcuatum</em>. Such procedure requires good anatomical knowledge of the thoracic malformation because it induces major modifications of the chest resulting from various osteotomies and chondrotomies. These latter interventions are aimed at constructing a real sternochondrocostal shutter which is stabilized afterwards by osteosynthesis using Borrelly’s splint staplers; these staples are positioned, after correction, behind or in front of the breastbone depending on the type of malformation. The constricting membrane is the last stage of loculated and complicated pleural effusions: haemothorax, empyemas, pleural tuberculosis, various other pleural diseases and sequellae of earlier collapsotherapeutic procedures. Decortication typically involves removal of the entire complicated pleural disease, freeing the visceral pleura (decortication <em>strico sensu</em>) and the parietal pleura (pleurectomy). After decortication the lung can expand again and improved functioning can be expected. Multiple technical procedures are available. When the pleural thickening is induced by underlying lung disease, it may be necessary to resect the underlying diseased parenchyma during the same intervention. Topical silicones have also become an integral part of the postoperative therapy for scar modification. In the most resistant cases, treatment is dependent on the various aetiologies that disturb the normal management of the chyle within the thoracic duct. In fact, chylothorax results from a pleural chylous leak issuing from the thoracic duct or from one of its tributaries, or from those collateral lymphatic vessels that allow chyle derivation in case of abnormal function. This leak may be acquired following trauma, thoracic surgery, or various diagnostic or therapeutic medical procedures. It may appear spontaneously, due to rupture of chyle overloaded lymphatic vessels, with chyle back flowing into them from a pathological or malformed thoracic duct. Chylothorax is easily diagnosed by thoracocentesis but its mechanisms (status of the thoracic duct, valve insufficient tributaries, chyle derivation collaterals) may be studied only by lympho-angiography. Standard treatment is first medical: drainage, suppression of all oral fat intake, re-nutrition, treatment of the medical cause (such as cancer, thrombosis…) and use of products aimed at reducing chyle production. In case of therapeutic failure, surgery is required. Lympho-angiography is of paramount importance among the surgical strategy. Surgery may act on the leak itself : ligation (or embolization) of the thoracic duct, sutures of the leaking collaterals, or various other procedures. While many researchers are attempting to replicate endogenous structures to create new tissues, numerous barriers must be overcome to create complex, vascularized, patient-specific tissue constructs for replacement and repair. Although multi-step, multi-component tissue fabrication requires an amalgamation of ideas, for clarity we will limit this review to recent developments in the application of natural and synthetic bioabsorbable scaffolds.
