The procedure consist of the stenting of the entire length of the carotid lesion and, for most operating centres, the use of cerebral protection. Preoperative therapy by aspirin and clopidogrel is mandatory as this reduces the rate of postoperative acute in-stent thrombosis. In order to ensure high rates of technical success and low rates of neurological events, carotid stenting should not be performed in patients with adverse anatomical features and the endovascular procedure guidelines should be strictly applied. Safety as regards administration of local anaesthetics (LA) imposes the knowledge of their side effects, particularly the toxic effects, and a strict respect of the operating procedures, whatever the technique. The use of needles with short bevel is recommended. The LA amounts will have to be adapted to patient's weight in order to avoid accumulated doses close to the toxic amounts (multiple injections, or anaesthetic procedure on several sites). The technique of the pudendal block is based on an access of the nerve at the end of Alcock's canal. It may be performed an isolated infiltration of the pudendal nerve or a block of this nerve by neurostimulation. When available, the graft must be anatomically and functionally perfect; this is a condition for the recipient's survival. Consequently, sampling and preservation must be performed with extreme caution. In 9/10 cases, grafts are removed from a donor in whom brain death is established and in 2/3 cases, several organs are sampled: heart, lungs, liver, kidneys, bowel, bone, cornea and sometimes skin. Whenever possible, maximum number of organs must be removed, in order to reduce the cruel and fatal situation of patients waiting for transplantation. The procedure is complex but well-organized; it necessitates very rapid intervention of several operating teams. Every surgeon must be perfectly aware of all steps of the procedure, as he or she will sometime encounter the necessity of sampling. The human context must be taken into account and the surgeon should keep in mind that although the sampling procedure is carried out on a deceased subject, its aim is to preserve or to keep living another one. Recent imaging techniques (CT-Scan and MRI) are the best diagnostic tools and are crucial when selecting the therapeutic strategy. Endoscopic surgery of paranasal sinuses has become the procedure of choice for benign tumours of the nasal cavity and paranasal sinuses. How the concept of endonasal surgical treatment can be applied to a malignant tumour and in particular to an ethmoidal adenocarcinoma? Lateral rhinotomy for ethmoidal adenocarcinoma allows removing the lateral mass of the ethmoidal bone and preserving the walls of the lateral masses. Endoscopic surgery outcomes are of the same quality than carcinologic results. Transfacial approach may be necessary when tumour extends to the surrounding structures. Nose and paranasal sinuses tumours can be successfully treated by endonasal approach. Traditional lateral rhinotomy approach of nasosinusal cavities remains the standard therapy for malignant tumours. If the tumour is very small-sized and located within the ethmoid sinuses, an endoscopic approach may be considered.
