Lower limbs are the most frequently involved, most of the time at the popliteal level; other locations include head and neck veins, and abdominal and thoracic veins. Various clinical presentations are reported in the literature. Venous aneurysms can be diagnosed as a subcutaneous or a mediastinal widening mass, as an incidentally findings on imaging study, or during work-up for abdominal or chronic venous diseases work-up. Although their natural course is usually benign, depending on their location, venous aneurysms have the potential for serious complications and may present initially as a pulmonary emboli, a thrombosis, or a rupture aneurysm. Thromboembolic complications are common in aneurysms that involve the deep vein system, particularly in popliteal aneurysms. The management of venous aneurysms remains controversial. Indication for surgery should consider potential thromboembolic complications or risks of rupture. Except for neck venous aneurysm, most venous aneurysms should be surgically treated to avoid potential morbidity or even death. Pulmonary emboli, venous thrombosis, external compression, bleeding, pain, swelling, or undefined mass are common indications. Numerous types of surgical repair have been described and the most common procedures are tangential excision with lateral venorrhaphy or excision with interposition grafting. The risk of recurrence is about 30%. Surgery is indicated in case of persistence despite drainage or incomplete lung reexpansion, first recurrence, or bilateral pneumothorax. Surgical management consists in resection of bullous area and pleurodesis. The techniques used for pleurodesis are the chemical pleurodesis, the mechanical abrasion or the parietal pleurectomy. Except for the rare cases of pseudo-tumours and “medical tumours”, surgical treatment is the main common perspective in this heterogeneous group. Chest wall surgery has long been considered as associated with very bad prognosis. These considerations have changed. Surgical procedures have become safe and effective. They do not inflect anymore the prognosis by themselves provided basic principles are respected. A detailed preoperative assessment has to be performed. It widely contributes to the development of an operative strategy. Afterwards, resection and wall reconstruction constitute an essential two-part surgical procedure. The risk for diaphragmatic rupture is 3-fold higher in case of lateral impact than after frontal impact. The diagnosis of diaphragmatic rupture remains problematic due to the poor specificity of clinical signs. Radiology shows the elevation of abdominal viscera rather than the discontinuity of the diaphragm. So, its identification is frequently missed in the acute phase, and it may be discovered later. Once the diagnosis has been established, the surgical treatment consists of a suture by an abdominal approach, often preferred to the thoracic approach which has only few indications. The use of autologous adipose transplant has been reported for the first time at the end of the nineteenth century. During the twentieth century, concepts and techniques have considerably evolved, and a renewed interest for adipocyte grafting is observed these last years, particularly after the description of the Lipostructure<sup>®</sup> by Coleman. This is a recent technique of adipocyte grafting that requires the utilisation of a specific material and a rigorous methodology. Several published works have shown the atraumatic character of this technique and the survival of the transplanted adipose tissue. Such demonstration and the good results that were obtained have promoted the utilisation of this technique for the restoration of all volume defects. In addition to the description of the properties of adipose tissue, history, and current technique for adipocyte grafting, the current applications of adipocyte grafting are listed and illustrated. Indications for face, thorax, breast, and limbs are described. This a simple, efficient, and reproducible technique that currently appears to be the best means for restoration of volume defects.
