Velly (Professeur des Universités, praticien hospitalier, chef de service)","doi":"10.1016/j.emcchi.2005.09.002","DOIUrl":"https://doi.org/10.1016/j.emcchi.2005.09.002","url":null,"abstract":"<div><p>After more than half a century where pneumonectomy was the only curative procedure for bronchial carcinoma, this intervention has become a procedure to be avoided as far as possible, due to its high associate in-hospital mortality (about 6% to 7% in specialized centers, up to 12% after the 3<sup>rd</sup> postoperative month)<span><sup>1</sup></span>. A better selection and preparation of the patients and significant technical improvements have stabilized the risk. An increased risk should be predicted however, related to the population ageing and to the more frequent combination of chemo- or radiotherapy with surgery. Any thoracic surgeon should be aware of the strictness of pneumonectomy procedures and rules for adequate decision making, successful surgery performance and management of the postoperative course. In this chapter, the authors describe the detailed gesture to perform surgical cervico-thoracic approaches and propose a synthesis of the different advances published in that matter. The cervico-thoracic approach of Cormier-Dartevelle-Grunenwald, the cervico-manubrial approach, the two combined video-assisted mediastinoscopy and thoracoscopy approaches are described. Selecting the better approach should take into account the situation of planned surgery or emergency, the need for combined access of both cervical and mediastinal or axillary regions or 2-stage approaches. Any of such approaches allows combined access of the three cervical, axillary, and thoraco-mediastinal regions. Such approaches are sometimes more time-consuming than the planned surgery act itself and they need thoracic surgery expertise.
