Léger","doi":"10.1016/j.emcaa.2004.09.002","DOIUrl":"https://doi.org/10.1016/j.emcaa.2004.09.002","url":null,"abstract":"<div><p>The laser Doppler (LDP), which utilises the Doppler effect, is the only method that allows a continuous assessment of the microcirculation perfusion. The measurement can be made at a unique point with the standard LDP, or on a surface of tissue using LDP– imaging (LDPI). The analysis of the signal allows measuring the perfusion and analysing the vasomotion. Tests of provocation are often used to standardise the measures. The fields of application of LDP are numerous: physiology, pathology, and pharmacology. Raynaud's phenomenon, peripheral arterial disease, ulcers, burns, cosmetics, plastic surgery, and numerous dermatologic pathologies are the most frequent applications. This locates superficial varicosities, sites of reverse flow and provides information on the disease stage and on the deep venous network. The aim of surgery is to eradicate reflowing vessels and superficial varicosities with concomitant sparcing of normal saphenous vessels. High ligature and stripping is the basic approach for long and short saphenous vein incompetence. Concerns about nerve injuries and cosmetic reasons suggest the selection of minimal invasive procedures such as selective stripping and invagination techniques. These basic methods may be associated to Muller's varicectomies and perforator interruption. The essential role of high ligation has been demonstrated in prospective studies for incompetent long and short saphenous veins; conversely, stripping of the whole saphenous trunk remains controversial. It seems to improve the results when the long saphenous vein is enlarged and submitted to significant reverse flow. Post operative recurrence rate is reduced by preventing de novo formation of a saphenofemoral junction. Patient selection is based on clinical data and D-dimer test criteria. The ultrasound examination is perfomed by a well- trained specialist allowing the identification of thrombosis when present (sensitivity and specificity close to 95%), as well as its origin in some patients. In several cases, the Doppler ultrasound may detect another disease likely to play a role in the symptomatology. When gangrene is present the arterial origin is beyond doubt. The diagnosis is more difficult when cutaneous ulceration appears. Venous or arterial disease, microcirculation abnormality must be ruled out.
