The availability of noninvasive investigation means, such as Duplex Scan, MRI and Angio-MRI, explain the extent at which these lesions are currently detected. The improvement of anesthetic procedures and the reproducibility of surgical procedures have resulted in improved management and results. The improvement of endovascular techniques has made such treatment commonly considered for proximal stenosis, whereas long-term benefits of endovascular treatment for carotid lesions remain to be demonstrated. Although vasovagal syncope is the most common cause of syncope in young patients, life threatening conditions may manifest as syncope and the main preoccupation of the physician who has to treat a child presenting with syncope is not to ignore a potentially lethal aetiology. Syncopes can be classified into three categories according to their mechanism: autonomic, cardiac, and non cardiac syncopes. The management begins with a good knowledge of the various aetiologies and with a detailed history and physical examination. Magnetic resonance imaging is a valuable tool to rule out other causes of acute spinal cord syndromes and to show the infarct. Most spinal cord infarcts are due to aortic diseases (atherosclerosis, aneurysm, dissection) or are of iatrogenic origin (surgery of aorta and aortography). Spinal hemorrhagic strokes are usually the consequence of arterio-venous malformations or coagulation disorders.
