Unfortunately, diagnosis often occurs at a late stage, and standard treatment involving radical surgery results in more than half of patients relapsing locally or with distant metastases. Median survival rarely exceeds 12 months, and in metastatic setting, 5-year survival is less than 10%. Given that standard treatment is not very efficacious, the emphasis needs to be on prevention. Dietary factors have an important impact on gastric carcinogenesis, especially in case of the intestinal type of gastric adenocarcinoma. Solmaz","doi":"10.19080/ctoij.2021.19.556010","DOIUrl":"https://doi.org/10.19080/ctoij.2021.19.556010","url":null,"abstract":"Purpose: This study aims to evaluate the prognostic significance of the aspartate aminotransferase (AST) / alanine aminotransferase (ALT) (De Ritis) ratio in diffuse large b-cell lymphoma ( DLBCL). Material and Method: This retrospective study analyzed newly diagnosed DLBCL patients between December 2003- November 2016 in our hospital. We used the university archive to analyze patient information. De Ritis ratio were calculated using data from the blood samples at the time of diagnosis. We analyzed the effect of the De Ritis ratio on the overall survival (OS) of 225 patients with DLBCL. Results: The mean age of the patients was 60.3(18–95) years, and 57.7 % of them were males. We performed ROC curve analysis to calculate optimal cut off points for the De Ritis ratio at the time of diagnosed. The median De Ritis ratio was 1.30( range 0,26 - 5,84 ). Based on the cutoff points for the De Ritis ratio, patients were separated in two groups: high De Ritis ratio group ( ≥1.30, n: 101 ) and low De Ritis ratio group ( <1.30, n:154 ) ( with sensitivity of 59%, specifity of 40%). We found a significant relationship between OS and De Ritis ratio. In Kaplan Meier analyses, the group with a higher De Ritis ratio had a more unfavorable prognosis for OS ( p=0.027). Conclusion: The De Ritis ratio may provide a cost- effective and sustainable marker for DLBCL patients. The most common metastases in hypo pharynx cancer are seen to lungs, bones and liver. However, here we present a rare case of carcinoma hypopharynx with skeletal metastases to rectus abdominis muscle. Patient was a documented case of multiple metastases to bone, adrenals and skeletal muscle as per whole body PET-CT scan. Skeletal muscle metastases are rare and has no specific pattern of occurrence. The neoplasm is uncommonly delineated within extracranial or ectopic sites. Thus, ectopic meningioma infrequently emerges within extracranial or extra-osseous region of the head and neck or maxilla and mandible and may manifest as a cystoid lesion [1]. Meningioma is appropriately established with ultrastructural examination and cogent immunohistochemistry. The morphology and immunohistochemistry of ectopic meningioma is identical to the frequently discerned intracranial meningioma. The reason for this includes mild nature of symptoms and thus less emphasis given to them by the patient as well as treating physician. It is important to do thorough examination and early investigations as this may point towards the diagnosis of a specific inflammatory, infectious, or neoplastic disorder. Moreover, in countries with a very high burden of tuberculosis (TB) like India, it becomes pivotal to first rule out extrapulmonary lymph node TB. Also, the failure to recognize and treat the specific condition can significantly add up to the morbidity and the resultant mortality. Clavicular swelling or fractures can occasionally be a presentation of solitary plasmacytomas but, multiple myeloma involving clavicle is extremely rare. 10-40% cases of multiple myeloma are asymptomatic and incidentally detected. Different sites are involved in the mechanism of chemotherapy induced peripheral neuropathy and considered multifactorial. 68% of cases develop CIPN during the first month following the start of chemotherapy, 60% of cases develop CIPN within 3 months after chemotherapy and only 30% after 6 months. CIPN is caused by chemotherapeutic agents which include taxanes, platinum analogs, and vinca alkaloids. The clinical presentation of CIPN includes multiple symptoms that may cause functioning impairment and may require reduction of the dose of chemotherapy. CIPN considered a common sequel of different agents of chemotherapy and may last from months to years after chemotherapy completion. CIPN can be diagnosed by a detailed history and clinical examination. Clinical examination of a patient with CIPN can be done by the use of nerve conduction studies. Antidepressant includes tricyclic antidepressants and serotonin-noradrenaline reuptake inhibitors are common drugs used for CIPN. Material and method: The study was performed after institutional ethical committee clearance.35 consecutive eligible patients treated between January 2015 to May 2016 were included in the study. PETCT was acquired at baseline and post neoadjuvant chemotherapy, prior to surgery. The SUV max of tumor and involved nodes at baseline and post neoadjuvant chemotherapy was compared to pathological tumor size and nodal status. Response was assessed using PRECIST and RECIST criteria. Statistical methods used for analysis were software SPSS version 13, calculations for Sensitivity, specificity, positive predictive value and negative predictive value. Results: The median age was 51.3 years (range 33 to 68 years). The patients were generally healthy with ECOG PS 0-1. FDG PET tumor to HPE correlation -sensitivity, specificity, PPV, NPV SENSITIVITY 88.8% SPECIFICITY 50% PPV 85.7% NPV 57.1% our results show the high sensitivity and low specificity of PETCT in detecting residual tumor. Overall PCR was in 22.6% (8/35). Pathological Complete Response Rates In The Axillary Node Luminal wise:Total:23/35 – 65.7%. Luminal A PCR Rate -14.4%; Luminal B PCR Rate-8.5%; TNBC PCR Rate-31.4%; HER 2-11.4%. For example, targeting the EGFR mutation has changed the overall prognosis in NSCLC. However, resistance to treatment has emerged and many canonical mechanisms have already been described. NF1 mutation causes partial or total loss of function of neurofibromin, which activates several intracellular pathways (MAPK / ERK, PI3K / AKT, TGF -β / Smad), producing cellular proliferation, migration, apoptosis resistance and genetic instability, leading to loss of EGFR expression.
