In cases when the delivery time of the donor organ is more than 4 hours, the risks of the donor organ ischemic injury are multiplied. Transportation of the recipient to the location of the potential donor in such cases is a forced measure, as it increases the risk of adverse cardiovascular events in the recipient during transportation. Providing medical care to a recipient in such circumstances may be difficult. However, these measures are fully justified by the reduced risk of postoperative complications associated with the time of graft ischemia. The article describes a case of heart transplantation in the conditions of transporting the recipient to the donor. Heart transplantation was performed in a patient with NYHA functional class IV heart failure, refractory to conservative therapy. The postoperative period was uneventful. The time of cardiopulmonary bypass was 90 minutes, the total ischemia time was 180 minutes. On the 3rd day, the patient was transported to the specialized centre to continue treatment and rehabilitation. On the 21st day the patient was discharged in satisfactory condition for outpatient treatment. Materials and methods. The study included 30 patients with coronary heart disease who underwent coronary artery bypass grafting with application of 2–3 aortocoronary anastomoses with CPB. According to the anesthetic management, all patients were divided into two groups: the first group (13 patients) – low opioid anesthetic scheme; control group (17 patients) – a standard scheme of anesthetic management. The determination of the level of аnnexin V in the blood was carried out before CPB and after bringing the sternum by enzyme-linked immunosorbent assay. Results. The low-opioid scheme of anesthetic management was associated with significantly (by 1.5 times) lower level of аnnexin V and by 28.38 % lower level of interleukin-6 at the end of the surgery as compared to the standard scheme. A significant negative correlation (r = –0.117, p = 0.523) was found between the levels of аnnexin V and interleukin-6. One-way analysis of variance showed that patients who had low cardiac output syndrome in the postoperative period had a significantly higher level of аnnexin V after CPB (p = 0.001). Conclusions. The use of multimodal low-opioid anesthesia is characterized by relative safety, a sufficient level of analgesia and lower level of аnnexin V compared to the control group. From the standpoint of current guidelines, the addition of second-line drugs, in particular ranolazine, to beta-blockers and/or calcium antagonists is considered during anginal attacks due to incomplete revascularization, to prevent myocardial damage during percutaneous coronary interventions, and in cases when revascularization is not possible. The results of many clinical studies have proven the antiischemic effect and safety of ranolazine after coronary artery stenting. Potential advantages of ranolazine compared with other second-line antianginal drugs are the absence of significant changes in hemodynamic parameters, good tolerability and proven antiarrhythmic effects. Materials and methods. Ninety patients with ischemic heart disease who underwent planned stenting of the coronary arteries were studied. The patients who underwent surgery were evenly divided into three study groups, depending on mode of the intraoperative analgesic sedation and the approaches to anesthesia. The first comparison group consisted of patients who received slow intravenous administration of diazepam and fentanyl solutions. The second comparison group consisted of patients with balanced administration of fentanyl and propofol solutions to provide analgesic sedation at the level of conscious anesthesia. The study group consisted of patients with a personalized approach to the administration of opiates, namely, we used the original fentanyl test described in the article. Analgesic sedation at the level of conscious anesthesia (ІІІ by Ramsey) was maintained by propofol infusion. Results. The usage of a personalized scheme of fentanyl administration for stenting of the coronary arteries, compared to the standard sedation using combination of diazepam and fentanyl, was accompanied by better indicators of intraoperative blood saturation with oxygen and carbon dioxide (respectively (103.67 ± 22.05) and (39.64 ± 6.85) mm Hg in group 1, (105.70 ± 31.64) and (37.68 ± 7.11) in group 2 and (109.42 ± 34.36) and (36.25 ± 6.52) mm Hg in patients of the 3rd group), lower blood pressure after surgery ((127.85 ± 9.87)/(79.64 ± 8.62) mm Hg in patients of group 1, (129.48 ± 8.73)/(81.05 ± 7.92) mm Hg in group 2 and (131.15 ± 10.64)/(82.68 ± 9.72) mm Hg in group 3), lower level of stress markers (blood cortisol during surgery in patients of the 1st group (8.83 ± 4.58) mmol/L, in patients of the 2nd group – (7.73 ± 2.79) mmol/L, in patients of the 3rd group – (7.55 ± 4.35) mmol/L), as well as lower frequency of detecting episodes of perioperative pain of various origins. Conclusions. A method of personalized anesthesia was elaborated, based on individual scheme of fentanyl administration («fentanyl test») during coronary artery stenting. Its usage is safe (due to the optimal parameters of gas exchange and hemodynamics and fewer side effects, such as nausea and residual sedation) and effective (due to the lower level of stress markers and less frequent complaints of pain of various origins) than when the routine technique was used. However, due to the anatomical characteristics of the aorta about 40 % of patients cannot be candidates for еndovascular aneurysm repair (EVAR). Anatomical features that are difficult or unacceptable for stent-graft placement include short or no proximal neck, angular, tapered neck, and vessel diameter that exceeds the existing capabilities of endoprostheses. In cases where traditional surgical correction is not an acceptable option, various technological methods and equipment are used. The observation included 16 consecutive patients with abdominal aortic aneurysm, where EVAR was performed. When analyzing the anatomical characteristics of the aneurysmal sac, 7 (43.7 %) patients were classified as patients with an unfavorable proximal neck of the aneurysm, the so-called «hostile neck». Two EVAR interventions were complex (2 patients with parallel grafts), which made it possible to achieve an increase in the proximal infrarenal zone implantation up to 16–20 mm, and adjuvant or additional endovascular procedures – in our case, implantation of Aptus Heli-FX endoancors in 5 patients. The comparison group consisted of 9 patients with standard aneurysm neck, where standard EVAR procedures were performed. The main anatomical difference in the groups was the length of the aneurysm neck – 9.9 mm and 36.1 mm in groups I and II, respectively (p = 0,0003). EVAR in the groups were carried out without significant complications or fatal cases; in the first group, the duration of the operation (p = 0.01), the amount of contrast (p = 0.03) and the fluoro time (p = 0.01) were significantly increased than in the 2nd group. The postoperative period did not differ between the groups. Conclusions. The use of contemporary technologies allows to significantly expand the indications for EVAR in patients with unfavorable anatomy. One of the options for the surgical treatment of this pathology is septal myoectomy, which has been the gold standard for decades. However, despite this, surgical treatment is intended for young patients with a low risk of postoperative complications, while patients with concomitant diseases and a higher surgical risk require alternative treatment. Today, alcohol septal ablation is considered an effective, minimally invasive method for treating hypertrophic obstructive cardiomyopathy in patients with a left ventricular outflow tract gradient ≥ 50 mm Hg. The article presents the experience of using alcohol septal ablation in 57 patients with obstruction of the left ventricular outflow tract. Undoubtedly, these two techniques changed the development of the cardiovascular field of medicine for many decades. The spread of aortic stenosis, as the most common acquired heart disease, allows us to understand the importance of the latest methods of its correction. The growing prevalence of use and changes in indications for this operation require the collaboration of specialists in various specialties: cardiologists, cardiac surgeons, anesthesiologists, resuscitators, perfusiologists, rheumatologists, electrophysiologists and others. To implement the method in medical institutions, separate teams of doctors «Heart Team» are created that not only participate in the selection of patients, but also participate in the implementation of TAVI.
