{"title":"[Recommendations for first-line immuno-hematology examinations by medical laboratories].","authors":"Patrick Joubaud, Jean-Marc Giannoli","doi":"10.1016/j.tracli.2024.10.004","DOIUrl":"10.1016/j.tracli.2024.10.004","url":null,"abstract":"<p><p>The main objective of these recommendations for good practices is to supplement the regulatory provisions of the decree of May 15, 2018 which sets the conditions for performing erythrocyte immunohematology tests in medical biology, in order to clarify their application for medical laboratories which conduct first-line immunohematology tests: erythrocyte phenotyping and detection of anti-erythrocyte antibodies. These recommendations cover all phases of a biological examination (pre-analytical, analytical, and post-analytical) as defined in the Public Health Code (Article L6211-2). Four levels of recommendations have been established: ● Recommended practices: complying with the requirements of the NF EN ISO 15189:2022 standard and ensuring optimal medical service delivery. ● Acceptable practices: situations which deviate from the best practices in immunohematology but still ensuring satisfactory patient management as well as transfusion and obstetrical safety. ● Non-recommended practices: not complying with the requirements of the NF EN ISO 15189:2022 standard and not allowing satisfactory patient management, but ensuring transfusion and obstetrical safety. ● Unacceptable practices: presenting a risk for transfusion or obstetrical complications. {"title":"Assessment of machine learning classifiers for predicting intraoperative blood transfusion in non-cardiac surgery.","authors":"Insun Park, Jae Hyon Park, Jongjin Yoon, Chang-Hoon Koo, Ah-Young Oh, Jin-Hee Kim, Jung-Hee Ryu","doi":"10.1016/j.tracli.2024.10.006","DOIUrl":"10.1016/j.tracli.2024.10.006","url":null,"abstract":"<p><strong>Background: </strong>This study aimed to develop a machine learning classifier for predicting intraoperative blood transfusion in non-cardiac surgeries.</p><p><strong>Methods: </strong>Preoperative data from 6255 patients were extracted from the VitalDB database, an open-source registry. The primary outcome was the area under the receiver operating characteristic (AUROC) curve of ML classifiers in predicting intraoperative blood transfusion, defined as the receipt of at least one unit of packed red blood cells. Five different machine learning algorithms including logistic regression, random forest, adaptive boosting, gradient boosting, and the extremely gradient boosting classifiers were used to construct a binary classifier for intraoperative blood transfusion, and their predictive abilities were compared.</p><p><strong>Results: </strong>337 (5%) patients received intraoperative blood transfusion. In the test-set, the logistic regression classifier demonstrated the highest AUROC (0.836, 95% CI, 0.795-0.876), followed by the gradient boosting classifier (0.810, 95% CI, 0.750-0.868), AdaBoost classifier (0.776, 95% CI, 0.722-0.829), random forest classifier (0.735, 95% CI, 0.698-0.771), and XGBoost classifier (0.721, 95% CI, 0.695-0.747). The logistic regression classifier showed a higher AUROC compared to that of a multivariable logistic regression model (0.836 vs. 0.623, P < 0.001). Among various parameters used to construct the logistic regression classifier, the top three most important features were operation time (0.999), preoperative serum hemoglobin level (0.785), and open surgery (0.530).</p><p><strong>Conclusion: </strong>We successfully developed various ML classifiers using readily available preoperative data to predict intraoperative transfusion in patients undergoing non-cardiac surgeries. Platelet apheresis (PPH) performed with apheresis equipment has increased rapidly in recent years. Leucocyte-reduced platelet apheresis (LRPH) can reduce the risk of platelet refractoriness and febrile nonhemolytic transfusion reactions (FNHTRs) for transfusion. Accordingly, this study aimed to investigate and compare the platelet metabolic and functional responses between PPH performed with Haemonetics and LRPH performed with Trima Accel cell separator.\u0000\u0000\u0000METHODS\u0000The qualities of platelets collected through PPH and LRPH were evaluated in terms of visual appearance, morphology, platelet-aggregation changes, metabolic activities, and bacterium-screening test during 5-day storage. Statistical analyses included two-sample t-test and generalised estimating equation(GEE) method.\u0000\u0000\u0000RESULTS\u0000During 5-day storage in LRPH, residual leucocytes were all <1.0×106, and the parameters of platelet function were as follows: platelet aggregated to agonists such as adenosine 5'-diphosphate (ADP) and collagen, and the extent of shape change and pO2 showed no statistically significant difference between PPH and LRPH. The hypotonic shock reaction (HSR) on days 0, 1, and 3 were significantly higher in LRPH than in PPH (71.78±6.92 vs. 64.10±7.42; p=0.002; 71.53±8.98 vs. 62.96±9.84; p=0.007; 68.05±7.28 vs. 57.76±6.80; p<0.0001, respectively). Values of mean platelet volume (MPV) were statistically larger in PPH than in LRPH on days 0, 1, and 3. On day 5, the swirling score was higher in LRPH than in PPH. The mean lactate levels had no statistically significant difference between PPH and LRPH. Moreover, no growth was observed through bacterium-screening test conducted on 40 samples.\u0000\u0000\u0000CONCLUSION\u0000Comparison of LRPH and PPH products collected from the Trima Accel and Haemonetics automated blood-collection systems, respectively, revealed that both products possessed good platelet qualities even though additional processes are needed to reduce leucocytes.
