The traditional paradigm of acne treatment has its limitations, such as skin irritation, photosensitivity, concern over antibiotic resistance, and regimen complexity. Given these challenges, discussing alternative options for acne treatment in detail is important. {"title":"CABTREO<sup>TM</sup> (Clindamycin Phosphate, Adapalene, and Benzoyl Peroxide) Topical Gel.","authors":"Aditya K Gupta, Avantika Mann, Kimberly Vincent, William Abramovits","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Cabtreo<sup>TM</sup> (1.2% clindamycin phosphate, 0.15% adapalene, and 3.1% benzoyl peroxide) or IDP-126 topical gel was approved by the US Food and Drug Administration (FDA) in October 2023 for the treatment of moderate to severe acne vulgaris in patients aged ≥12 years. Its effectiveness and safety were evaluated in two phase 3 trials (Trial 1 and Trial 2). In both trials, subjects were randomized into two groups, one received IDP-126 gel and the other received vehicle gel. The primary efficacy end point was treatment success at week 12, defined as subjects achieving at least a 2-grade reduction from baseline in Evaluator's Global Severity Score (EGGS) and a final score ranging from 0 (clear) to 1 (almost clear) as well as reduced counts of inflammatory and non-inflammatory lesions. In Trial 1 (N = 183), treatment success was achieved in 49.6% (61/122) of subjects in the IDP-126 group versus 24.9% (15/61) of subjects in the vehicle group (<i>P</i> < 0.01). In Trial 2 (N = 180), treatment success was achieved in 50.5% (61/120) of subjects in the IDP-126 group versus 20.5% (12/60) of subjects in the vehicle group (<i>P</i> < 0.01). She had achieved all of her developmental milestones, and her parents denied any similar family history. She had only two hair cuts in the past, which were performed to trim long hair over the midscalp to have a uniform length. Hair at the front, back, and sides of the scalp were never trimmed. Examination revealed short, dark, and thin hair, varying in length from a few millimeters to 2 cm, over the frontal, parietal, and occipital areas. Longer hair were present over the midscalp, varying in length from 5 cm to 8 cm. The hairpull test was negative, and dermatoscopic examination of short hair demonstrated tapered ends, rather than cut ends. Microscopic examination revealed no hair shaft abnormality, and a trichogram of 50 parietal hair fibers demonstrated an anagen versus telogen ratio of 68:32. His pre-immunosuppressant work-up was normal. He was prescribed methotrexate prior to switching to cyclosporine with an initial dose of 200 mg daily (2.44 mg/kg/day). His urinalysis was normal 2 months after initiating cyclosporine. After 18 months of cyclosporine therapy, his urinalysis revealed glycosuria (300 mg/dL), even though his fasting serum glucose was 5.2 mmol/L (normal) and there was no indication of any urinary tract infection (UTI). This occurred at a dose of 100 mg every 2 days (0.610 mg/kg/day). Dermatopathology learning is a core requirement for both dermatology and pathology residencies. Many traditional educational methods have been utilized over decades for teaching our residents. In the recent digitized era, technological advances are impacting the study and practice of medicine. Several innovative tools have become available that complement the conventional learning model. This contribution provides guidelines for establishing a robust dermatopathology rotation for residents and insights into the present and future advances in novel educational tools. Dermatopathology is an ever-growing body of knowledge. It takes years to accumulate practical knowledge of this discipline and requires a continuing effort to maintain and keep it updated. This journey of learning starts during the early years of postgraduate medical education. Attaining competency in dermatopathology is vital for both dermatology and pathology residents. The earlier the exposure they receive, the better the foundation and understanding of cutaneous disease processes they develop.
