The high photon collection efficiency of TB PET translates into superior sensitivity and signal-to-noise ratio compared to conventional scanners, enabling shorter acquisition times and lower administered doses while significantly enhancing diagnostic performance. In the era of precision medicine, patient selection for radioligand therapy and treatment response assessment are expected to benefit from accurate lesion detection, dual-tracer studies, the use of short- and long-lived radiopharmaceuticals, tracer kinetic analysis, and frequent follow-up monitoring. These advantages are made possible by long-axial field-of-view scanners and further maximized through TB PET/CT.</p>","PeriodicalId":94168,"journal":{"name":"PET clinics","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2026-04-07","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147641053","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This review explores emerging biomarkers that may complement or expand beyond PSMA, including gastrin-releasing peptide receptor, fibroblast activation protein, prostate stem cell antigen, CD46, delta-like ligand 3, glypican-3, B7-H3, 6-transmembrane epithelial antigen of the prostate-1, trophoblast cell surface antigen-2, CEACAM5, and vicrostatin. Its high accuracy in detecting primary, recurrent, and metastatic disease guides personalized treatment strategies and enhances interventional procedures like biopsies and ablations. Advances in novel radiotracers and hybrid imaging modalities further improve diagnostic specificity, especially in complex and pediatric cases. Integrating PET-CT with genomic data and artificial intelligence (AI)-driven tools promises to advance personalized medicine, enabling tailored therapies and better outcomes. PET-CT, particularly with Gallium-68-DOTATATE, allows earlier and more precise detection of lesions compared to conventional imaging. It identifies somatostatin receptor-positive tumors, aiding in staging, interventional procedures, and guiding peptide receptor radionuclide therapy. The complementary use of fluorodeoxyglucose-PET for predicting tumor biology and managing aggressive, poorly differentiated tumors, helps provide a comprehensive framework for personalized mNETs management. Finally, PET-CT's role in patient diagnosis, staging, surveillance, and management by the interventional oncologist is outlined in detail.</p>","PeriodicalId":94168,"journal":{"name":"PET clinics","volume":" ","pages":"e27-e34"},"PeriodicalIF":0.0,"publicationDate":"2025-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"145067133","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
