Educative intervention.","authors":"Suemmy Gaytán-Fernández, Rodolfo G Barragán-Hervella, Jorge Quiroz-Williams, Evelyn M Del Valle-Cortés, Isabel O Castillo-Ramírez, Álvaro J Montiel-Jarquín","doi":"10.24875/CIRU.21000797","DOIUrl":"10.24875/CIRU.21000797","url":null,"abstract":"<p><strong>Background: </strong>Developmental dysplasia of the hip (DDH) is a complex clinical entity that is usually underdiagnosed, if not detected and managed early, will turn the affected individual into a disabled being, with negative social, economic and emotional effects.</p><p><strong>Objective: </strong>To determine the capacity for the timely radiographic detection of DDH before and after an educational intervention.</p><p><strong>Method: </strong>An educational intervention is carried out in family medicine resident, where they are given training on detection in DDH radiographic projections. Pre- and post-training evaluation is carried out. Statistical analysis is performed using Student's t and χ<sup>2</sup>, taking p ≤ 0.05 as significant.</p><p><strong>Results: </strong>94 residents participated. In the pre-intervention evaluation, 87.2% had no knowledge of the early detection protocol (p = 0.525). It was observed that 98.9% incorrectly drew the Perkins line (p = 0.427), 96.8% the Hilgenreiner line (p = 0.177) and 87.2% did not consider the data of bilateral dysplasia (p = 0.956). Its surgical treatment has been shown to be cost-effective. Thermal endoablation performed as major outpatient surgery (MAS) is the technique of choice.</p><p><strong>Objective: </strong>To know the satisfaction of patients after MAS and the quality of care provided.</p><p><strong>Method: </strong>Observational, descriptive, cross-sectional study, carried out between January 2019 and March 2020. A survey was carried out on patients who underwent endovascular radiofrequency thermoablation in two hospitals in Spain, consisting of two questionnaires: one ad hoc and one another of satisfaction (SERCAL). Quality indices for MAS were measured.</p><p><strong>Results: </strong>We analyzed 156 episodes. 145 interventions were carried out. We found 4 (2.8%) unscheduled admissions. 3 patients (2.1%) attended the emergency department. There was a 99.3% success rate. 100% of the patients had pre-surgical recommendations and informed consent. We obtained 48 surveys (response rate: 33.1%). All the patients gave a rating greater ≥ 8 in quality of care (x = 9.83) and would recommend this procedure to their relatives (x = 9.71).</p><p><strong>Conclusions: </strong>Endovascular radiofrequency thermoablation performed in MAS has a high success rate and a very low rate of admission and visit to the emergency room. {"title":"Assessing cell chimerism in an acute graft-versus-host disease model established in TLR4 knockout mice.","authors":"Yi Zhao, Xi Huang, Yujie Zhang, Donghua He, Jingsong He, Enfan Zhang, Miao Chen, Qiuyan Liu, Jing Chen, Zhen Cai","doi":"10.24875/CIRU.22000414","DOIUrl":"10.24875/CIRU.22000414","url":null,"abstract":"<p><strong>Background: </strong>Graft-versus-host disease (GVHD) is a major complication after allogeneic hematopoietic stem cell transplantation.</p><p><strong>Objective: </strong>To elucidate the role of Toll-like receptor 4 (TLR4), the major receptor for bacterial lipopolysaccharide, in the development of GVHD, we constructed a GVHD model in TLR4 knockout (TLR4<sup>-/-</sup>) mice and monitored the cell chimerism.</p><p><strong>Methods: </strong>In this study, we used polymerase chain reaction to identify whether TLR4 knockout (TLR4<sup>-/-</sup>) mice were established. Before transplantation, we pretreated mice with irradiation so as to obtain an appropriate irradiation dose. Flow cytometry was applied to measure the chimerism status, the distributions of antigen-presenting cells (APCs), and T-cells in TLR4<sup>+/+</sup> and TLR4<sup>-/-</sup> recipient mice.</p><p><strong>Results: </strong>The general condition of TLR4<sup>-/-</sup> recipients was better than that of TLR4<sup>+/+</sup> recipients, and the TLR4<sup>-/-</sup> recipient mice showed less severe GVHD manifestations than the TLR4<sup>+/+</sup> recipient mice. Risks related to anesthesia and surgery have led a search for alternative treatments. Bipolar radiofrequency (RF) thermotherapy is one of the methods adopted in patients with high surgical risks. The aim of this study is to compare the effect of bipolar RF thermotherapy and transurethral resection of the prostate (TURP) methods on voiding symptoms and on post-operative complication rates especially in patients carrying high surgical risks.</p><p><strong>Methods: </strong>Pre-operative, post-operative 1<sup>st</sup> and 6<sup>th</sup> month International Prostate Symptom Score (IPSS), Qmax, quality of life, prostate volumes, and postoperative complications of the patients underwent TURP and RF for benign prostatic hyperplasia (BPH) were compared.</p><p><strong>Results: </strong>In the RF group, the pre-operative median IPSS was 30, prostate volume 41.5 cc, post-void residual (PVR) 80 ml, and Qmax is 5.85 ml/s.; In the TURP group, these were 29, 40 cc, 85 ml, and 5.3 ml/sec, respectively. {"title":"Gallbladder stones during pregnancy: are we doing the right thing in Mexico?","authors":"Gerardo Chávez-Saavedra, Elia Lara-Lona, Daniel Díaz-Martínez, Laura Villaseñor-López-Velarde, Angélica Espinosa-Hinojosa, Manuel Rivera-Sahagún","doi":"10.24875/CIRU.22000633","DOIUrl":"10.24875/CIRU.22000633","url":null,"abstract":"<p><strong>Objective: </strong>To present the treatment of choice and approach in pregnant and postpartum women with a diagnosis of gallstones in Mexico and to compare it with the recommendations of international guidelines.</p><p><strong>Method: </strong>Observational, descriptive, and retrospective study based on information from the 2019 Dynamic Cubes database of pregnant women diagnosed with cholecystitis and/or cholelithiasis who had undergone cholecystectomy.</p><p><strong>Results: </strong>During 2019, 937 patients with cholelithiasis and cholecystitis were registered, 516 (55%) pregnant and 421 (45%) in puerperium. 91.47% of cases were managed with medical treatment and 8.53% with cholecystectomy, with predominance in the open approach in 63.75% of cases. Animals were divided into two groups the control group and PRP application group. In each group, a transverse incision was made to the uterus to mimic the cesarean section and sutured. Relaparotomy was performed 21 days after the first operation.</p><p><strong>Results: </strong>When the groups were evaluated in terms of inflammation, there was a significant difference between the two groups. When the groups were evaluated in terms of Mason's Trichrome staining and fibrosis, There was a significant difference between groups. When the groups were evaluated in terms of vascular endothelial growth factor-1, there was also a significant difference between the groups. The VMCT flap was used, and a direct pathway into the defect was made through a separate posterior thoracotomy shortening the distance between the flap and the defect.</p><p><strong>Results: </strong>In 80% of the cases, the flap succeeded in solving the fistula and filling the defect, quality of life improved, and the need for oxygen decreased.</p><p><strong>Conclusions: </strong>Management of open window thoracostomy is challenging. Debridement, thoracoplasty, and flap coverage are the mainstream of their treatment, but these patients have scarce available muscle. The VMCT flap represents the major non-affected musculocutaneous unit in the thoracic area after lung surgery. Its dermal component offers a rigid matrix to form a seal over the bronchial stump. Its muscular component adds a good amount of vascularized tissue. {"title":"Clinical and ultrasonographic findings associated with testicular infiltration in pediatric patients with leukemia.","authors":"José L González-Chávez, Edgar Melo-Camacho, Edgar G Lazcano-Rojas","doi":"10.24875/CIRU.22000004","DOIUrl":"10.24875/CIRU.22000004","url":null,"abstract":"<p><strong>Background: </strong>Testicular infiltration is infrequent in pediatric patients with leukemia and can be confused with other testicular conditions.</p><p><strong>Objective: </strong>To analyze the presence of clinical and radiological features suggestive of testicular disease and its histological association with leukemia infiltration.</p><p><strong>Method: </strong>Retrospective and analytical observational study that included patients with diagnosis of leukemia who underwent biopsy for suspected testicular infiltration. The relationship with the variables analyzed were diagnosis, reason for taking the biopsy, ultrasound findings, stage of treatment, induration, increased volume and pain, with testicular infiltration.</p><p><strong>Results: </strong>Eighteen patients were included; 11 of them with microlithiasis, of which one 1 reported infiltration (odds ratio: 0.075; p = 0.026), no association was found between ultrasound findings and the presence of infiltration. Clinical findings were significantly associated with positive biopsies.</p><p><strong>Conclusions: </strong>No risk association was found with the ultrasound findings such as microlithiasis and hypoechoic imaging. The Food Cravings Questionnaire-State (FCQ-S) is the most used instrument for its diagnosis. It is a multidimensional instrument, sensitive and adaptable to contextual and cultural changes.</p><p><strong>Objective: </strong>To standardize the FCQ-S in the adult population of Mexico City.</p><p><strong>Method: </strong>Non-experimental, cross-sectional, at convenience design, with 1059 adults of both sexes, aged 18-84 years. It is highlighted that 71.9% of the participants were women.</p><p><strong>Results: </strong>A reliability coefficient of 0.95 was obtained, the correlations between the items were from r = 0.598 to r = 0.793. With the exploratory factorial analysis, an MKO of sampling adequacy of 0.943 was obtained, and with the Bartlett sphericity test a p = 0.000. The factors explain 78.61% of the total variation of the data. The RMSEA was 0.068, which indicates an acceptable fit. The CFI was 0.974, considered good, and NNFI was 0.969, good fit.
