This study aims to present the functioning of the APQC and analyse its outcomes.</div></div><div><h3>Methods</h3><div>Between 2022 and 2024, 73 patients were evaluated, with a mean age of 72.8 ± 10 years. Of these, 97.3% were ASA ≥ III, and 41.1% had a Clinical Frailty Score ≥4. The evaluation centered on a multidisciplinary committee responsible for determining the patient’s operability and guiding the intrahospital circuit. During postoperative evolution, patient follow-up was carried out by two complementary teams in continuous communication. Failure to Rescue (FTR) was defined as the death of a patient following one or more serious complications.</div></div><div><h3>Results</h3><div>The main reason for including patients in the CSPA was multimorbidity in 53.4% of cases and a specific pathology in 28.8%. In 31.5% of cases, the intervention was ruled out, with one-year survival below 40%. Among the 35 operated patients, the Comprehensive Complication Index (CCI) was 18.034 ± 21.94, the average hospital stay was 14.34 ± 20.15 days, and the readmission rate was 25.7%. The FTR rate was 12.5%.</div></div><div><h3>Conclusions</h3><div>Current data suggest a positive impact of the APQC on the evolution of complex patients. This meta-analysis encompasses all published randomized trials investigating and comparing the impact of indocyanine green (ICG) angiography on this complication. With four studies and 1,109 patients, the intraoperative ICG angiography group demonstrated a significant reduction in the overall rate of anastomotic dehiscence compared to the non-angiography group (7.3% vs. 11.5%; OR: 0.6; 95% CI: 0.4−0.9; p = 0.03). These differences were maintained in rectal surgery subgroup, with no prolongation of surgical time or increase in morbidity and mortality. There were no differences in the left colon surgery group. Historically, its management was rudimentary, with tracheostomies performed in ancient Egypt, and progress remained stagnant for millennia. Significant advances began in the late 19<sup>th</sup> century, followed by notable surgical and anesthetic progress in the mid-20<sup>th</sup> century. The 1952 polio epidemic spurred further development, with tracheal resection and anastomosis as the first major milestone, later extending to the larynx and carina. All significant advances in airway surgery ended in the latter half of the 20<sup>th</sup> century, with minimal progress since. Initial randomized trials, such as COLOR II and COREAN, indicated that laparoscopic approaches offered similar pathological outcomes with better postoperative recovery than open surgery. In contrast, trials like ACOSOG Z6051 and ALaCaRT suggested noninferiority could not be established. Variability in trial outcomes, focusing on either disease-free survival or pathological measures, initially hindered consensus. Long-term analyses have shown no significant difference in disease-free survival between laparoscopic and open approaches. Meta-analyses have reinforced the benefits of laparoscopic surgery, with reduced mortality and similar oncologic effectiveness to open surgery. However, new techniques like transanal TME (TaTME) and robotic approaches have introduced alternatives, though each presents unique challenges, from recurrence rates in TaTME to costs in robotics.
