The main objective of this study was to analyze the predictive variables for complete tumor response after neoadjuvant therapy.</div></div><div><h3>Methods</h3><div>Data from 146 breast cancer patients treated with primary systemic therapy at La Ribera Hospital between 2018 and 2022 were reviewed. Statistical methods were employed using SPSS 23 software to identify factors predicting tumor response to treatment.</div></div><div><h3>Results</h3><div>Among the patients analyzed, 22.6% presented complete response according to magnetic resonance imaging, and 31.5% showed no residual tumor in the surgical specimen. In the univariate analysis, variables associated with higher presence of residual tumor included lymphovascular invasion (<em>P</em> = 0; 95%CI: 2.361−45.390), luminal B subtype (<em>P</em> = .039; 95%CI: 1.028−5.826), and sentinel node involvement or positive axillary nodes in axillary lymphadenectomy (<em>P</em> = .031; 95%CI: 1.006−72.361). Conversely, tumors smaller than 2 cm (<em>P</em> = .002; 95%CI: 1.567−7.535) and the Her2+ subtype (<em>P</em> = .004; 95%CI: 0.086−0.667) were associated with a higher likelihood of no residual tumor. In the multivariate analysis, lymphovascular invasion was the only statistically significant factor associated with the presence of residual tumor (<em>P</em> = .021; 95%CI: 0.037−0.760).</div></div><div><h3>Conclusions</h3><div>Lymphovascular invasion was identified as a predictor of lack of response to neoadjuvant treatment. Additionally, the Her2+ subtype demonstrated the best response to preoperative therapy. Studies like ROLARR have shown no significant differences, often due to biases related to the surgeons’ experience.</div><div>The treatment of rectal cancer is complex, and robotic surgery promises to facilitate embryological dissection, potentially improving oncological and functional outcomes. However, solid evidence is still scarce. The advantages of robotic surgery for surgeons include better ergonomics, precision in movements, and the possible connectivity between systems for telemonitoring. This study aimed to identify preoperative risk factors for bowel resection in acute irreducible inguinal hernias.</div></div><div><h3>Methods</h3><div>A retrospective cohort study included patients from 2 university hospitals who underwent emergency surgery for acute irreducible hernias between January 2010 and December 2018.</div></div><div><h3>Results</h3><div>Out of a total of 652 patients, 15% required intestinal resection; females, older individuals, and those with comorbidities were more likely to undergo resection. Multivariate analysis identified patients with femoral hernia (OR 2.272; 95%CI 1.275–4.047; <em>P</em> = .005) and preoperative intestinal obstruction (OR 8.071; 95%CI 4.331–15.043; <em>P</em> < .001). Patients needing resection experienced higher postoperative complication rates and longer hospital stays.</div></div><div><h3>Discussion</h3><div>Femoral hernia and preoperative intestinal obstruction were independent predictors of bowel resection in acute irreducible hernias. It was described for the first time by Antonio Toesca in 2015. In Spain, it is currently carried out in very few places. The results of this technique have been reported in multiple studies with low conversion and complication rates. It is indicated in patients with genetic risk and in patients with early breast cancer presenting no evidence of skin, chest wall or nipple-areolar complex involvement. In order to advocate the robotic approach, we give a step-by-step explanation so that it can be reproduced safely.
