Westphal","doi":"10.5604/17342260.1024761","DOIUrl":"https://doi.org/10.5604/17342260.1024761","url":null,"abstract":"Objective: To evaluate the effect of a multimodular therapy concept (endurance training, strength training, and diet) on proinsulin in patients with metabolic syndrome and non-insulin dependent diabetes. Methods: Prospective cohort study (n = 22), prepost-evaluation. Data evaluated: BMI, HbA1c, proinsulin, body fat, serum glucose. Observation period three months. Results: Proinsulin showed a tendency to normal values (n.s.). When those patients who took metformin (n = 7) only, the effect was significant (P < 0.05). BMI, body fat and body weight decreased significantly (P < 0.001). HbA1c did not change significantly. Conclusions: Although the effect of decrease of body mass may be partially camouflaged because there was an increase of muscle mass (heavier than fat), the concept has been proven to be an effective strategy to decrease several risk factors on metabolic syndrome. Further investigations are necessary to detect subgroups of patients who may have the optimal benefit and more data are necessary to establish an optimized structure of the training for the patients. The validity of a meta-analysis, also known by its critics as numerological abracadabra, is dependent on the arbitrarily defined criteria and discrimination of the analyst, and more importantly, on the quality of the inclusive studies. The focus of this commentary is a meta-analysis by Krieger, who claimed that multiple sets of each exercise are superior to a single set of each exercise for increasing muscular strength. However,  the  determination  of  the  initial  test  load,  and  duration  of  the  test  make  difficult  to  use the  same  in  non ⴀindividualised  environments. Objective: To determine coefficients to estimate the maximum strength (1RM), based on the relationship between muscular  strength,  lean  body  mass  and  total  body  mass. Methods:  Twenty ⴀeight  strength ⴀtrained  and  non ⴀstrength ⴀtrained  men  participated  in  this  study. Muscle  strength  was determined  using  the  1  RM  test  in  the  bench  press,  supported  barbell  row,  45°  leg  press  and  squat  exercise,  while  body composition  was  measured  using  the  skinfolds  method. After  verifying  the  correlations between  muscular  strength  and  body mass  and  composition,  the  coefficients  to  predict  the  maximal  strength  were  calculated  by  dividing  the  value  of  the  1  RM by  the  total  body  mass  and  lean  body  mass  (kg)  and  by  linear  regression  equation  based  in  these  parameters. Results: Significant  correlations  were  found  between  body  mass  and  lean  body  mass  with  muscular  strength  in  all  the exercises  ( r  =  0.47   ⴀ  0.76, P  <  0.05).  The  greatest  correlations  were  observed  between  the  muscular  strength  values  and lean  body  mass. There  was  a significant  difference  between  the  coefficients  obtained  from  trained  and  non ⴀtrained  subjects in  all  the  tested  exercises  ( P  <  0.05). Compared to other activities such as mountain climbing and rock climbing, the risks associated with hiking are perceived as minimal. However, as participation has increased, the research literature has started to document an increasing number of hiking related injuries and illnesses. The case reports in this study identify the factors that contribute to the death of four hikers in different regions of the American wilderness. Environmental factors and factors related to acute bad judgment syndrome played a major role in the death of all four hikers.
