Angelico","doi":"10.15761/mcrr.1000124","DOIUrl":"https://doi.org/10.15761/mcrr.1000124","url":null,"abstract":"Spur cell anemia (SCA) is a form of hemolytic anemia historically associated with alcoholic liver cirrhosis. The name of the disease comes from the spike-like projections on the erythrocyte membrane. These are due to a disorder in the cholesterol metabolism causing increased cholesterol to protein and cholesterol to phospholipid ratios in the red cell membrane, making the cells more prone to trapping and destruction by the spleen [1]. One of the first cases of SCA associated to post-alcoholic liver cirrhosis was reported in the New England Journal of Medicine in 1964 [2]. Several investigators have reported the association between alcoholism and SCA, but recent reports demonstrated that the entity might be present in liver disease regardless of etiology. It was reported that the comparison of spur cell rate in patients with alcoholic cirrhosis versus remaining etiologies did not exhibit any difference [3]. The presence of SCA in liver cirrhosis has been associated with advanced disease and poor prognosis [4]. Spur-cell morphological abnormality is reversible in a non-cirrhotic microenvironment and it vanishes after liver transplantation [5]. In this study, the main outcome was to investigate the appropriate dosage of intermittent bolus injection from epidural block (0.2%ropivacaine 200mL+fentanyl 200-400μg/kg). The definition of “Appropriate” was we had no side effects such as neurologic complication such as numbing and hypotension, and to keep the patient’s good pain control controlled below resting Numerous Rating Scale (NRS) three until 24 hours after surgery (ICU periods). This study started from 2ml6ml/60 minutes (minimum dose) according to our protocol. In 2ml protocol, one patient cleared our protocol, however two patients couldn’t clear in terms of NRS. In 3ml protocol, we could keep the good pain control based on the definition of “effectiveness” (NRS, bolus request, the necessity of increased pain analgegics and no deleterious events). 3mL of intermittent epidural contrast for the open hepatectomy was identified by our clinical study in order to control the postoperative pain. Even though the exact etiology is not clearly understood, it is believed that upregulation of postsynaptic dopamine receptors after chronic dopamine blockade and neuronal oxidative damage may be implicated. The symptoms may be lifelong in some patients. Risk factors include advanced age, female sex, type of antipsychotic agents and routes of administration, pre existing movement disorders and general health of the patient. Multiple agents including vitamin B6, branched-chain amino acids, Ginko biloba, medications including beta blockers, ondansetron and benzodiazapines have been tried in the treatment of tardive dyskinesia without much success. The newly approved medications valbenazine and deutetrabenazine offer hope to these patients who otherwise had to live with this socially and functionally disabling disorder. Methods : Literature review was conducted using keywords tardive dyskinesia, risk factors, pathophysiology, treatment, Valbenazine, and deutetrabenazine. Search engines used include Pubmed, Cochrane Review, PsycINFO, and Psychiatry Online. Results : We have summarized the history, pathophysiology, risk factors, and management of TD including the recently approved medications. Conclusion : Tardive dyskinesia is a disabling, long term side effect of antipsychotic use. Many risks factors predispose patients to the development of symptoms. Until recently, there were no FDA approved treatments. It is a disorder of fat metabolism with deposition of multiple, symmetrical and painless non-encapsulated lipomas. Depending on the type of Madelung’s Disease, the adipose tissue deposition can be seen in the neck, upper extremities, trunk and hip area. About 90% of the cases described in the literature have been associated with chronic alcoholism and tobacco use and in a population of European/ Mediterranean descent, reporting up to 1 in 25,000 cases in Italy to have the condition [2,3]. Rare cases have also been reported in Indians and one case in an African American male [4,5]. Diagnosis was clarified with a biopsy of the frontal bone which revealed multinucleated cells with granulomas. Negative S100 and CDA1 immunohistochemical markers helped exclude. Also, drainage of a paracostal soft tissue abscess reported acid-fast bacilli seen in the Ziehl-Neelsen stain of pus. Bone marrow biopsy and aspirate were both normal. Tuberculosis is one of the most frequent differential diagnosis, but still, the clinical and radiological manifestations overlap between these two diseases. A high index of suspicion is necessary to clarify the diagnosis and offer timely treatment. Case report An 8-year-old, apparently previously healthy boy, presented to the emergency room with 3 months of a left paracostal abscess and a frontal subcutaneous nodule, accompanied by weight loss and intermittent non quantified fever. Skull radiography revealed osteolytic lesions of the frontal bone (Figure 1) and chest X rays showed osteolytic lesions of the left tenth and eleventh ribs, without pulmonary compromise. A 3-phase bone scintigraphy, reported abnormal cranial, costal and vertebral (T2, T4 and T9) capitation (Figure 2). Suspecting Langerhans cell Histiocytosis (LCH), a biopsy of the frontal bone along with drainage of the paracostal soft tissue abscess were performed. The Ziehl-Neelsen stain of pus drained from the paracostal abscess reported acid-fast bacilli (Figure 3). After growth in culture, a rapid test (SD BIOLINE TB Ag MPT64 Rapid TEST) confirmed M. tuberculosis complex, later identified as M. tuberculosis by PCR (GenoType Direct mycobacterial assay). Frontal bone biopsy revealed multinucleated cells with granulomas. S100 and CDA1 immunohistochemical markers to exclude LCH came back negative. The bone marrow biopsy and aspirate were both normal, as well as a negative 4th generation HIV test. No different organic compromise was identified except the multiple bones previously mentioned.
