Some have even compared its effectiveness to corticosteroid injections [4]. To bring more attention to this new technique, the term Glucopuncture was introduced in January 2021. Glucopuncture is a medical procedure which applies isotonic glucose injections for the management of non-rheumatic musculoskeletal conditions [5]. The technique consists of multiple injections of G5W (Glucose 5% in Water) in the region of complaint. Intradermal injections are given for pain modulation, intralesional injections are given in, for example, muscles, tendons, and ligaments to stimulate tissue repair. One can also apply these injections adjacent to peripheral nerve endings [6] or in the epidural space [7]. Invasive pulmonary aspergillosis occurs almost exclusively in immunocompromised patients and may be associated with pneumopericardium. We present a 12-year-old female with acute lymphoblastic leukemia who developed multifocal pulmonary Aspergillus terreus after induction chemotherapy and prolonged granulocytopenia. Despite aggressive antifungal therapy, she developed pneumopericardium, which progressed to pneumothorax and pulmonary hemorrhage, resulting in death. To our knowledge, no reports involving the sequential development of these three findings are reported to date. {"title":"Is there a role for the Girdlestone resection arthroplasty in modern orthopedic trauma surgery? A preoperative functional evaluation and a one year postoperative follow up was performed using the Merle d’Aubigne Score. Despite the unavailability of osteosynthesis or prosthetic implant a good functional result was achieved. The established surgical procedure of Girdlestone resection arthroplasty represents an effective treatment option for patients with severe fractures of the proximal femur with a high risk of infection, who live in low socio-economic and medically underserved areas. This procedure avoids secondary surgery, chronic infections, fistulas and repeated hospitalization. Susanna","doi":"10.15761/mcrr.1000174","DOIUrl":"https://doi.org/10.15761/mcrr.1000174","url":null,"abstract":"Elevated intra-ocular pressure (IOP) and mean IOP are considered the main risk factors for the development and progression of glaucoma. However, some patients still progress with IOP apparently in the target range. This observation has been explained on the basis that other non-IOP dependent risk factors are contributing to the glaucoma pathogenesis in these individuals. An alternative explanation is that progression occurs at least in - part due to high IOP peaks not detected during routine eye examinations. Several studies have demonstrated that peak IOP may be a better predictor of glaucoma progression. IOP peak assessment has been used recently to verify if the peak pressure of a given patient is in target range, to evaluate glaucoma suspect risk, the efficacy of hypotensive drugs and to detect early loss of IOP control. These are important aims to be addressed in glaucoma management. Several methods have been described to assess IOP peaks. The costs and labor involved in this make the determination of the 24-hour IOP or contact lens-sensor are difficult if not impossible in all patients. Recently the water drinking test (WDT) has been used as a surrogate marker for outflow reserve to detect IOP instability and to estimate IOP peak pressure. {"title":"ANTIPROLIFERATIVE PROPERTIES OF ETHANOLIC AND AQUEOUS GRAVIOLA LEAF EXTRACTS ON TONGUE SQUAMOUS CELL CARCINOMA CELL LINE-25.","authors":"Ricardo A Velázquez-Aponte, Céline Cassé","doi":"10.15520/jmcrr.v3i08.201","DOIUrl":"https://doi.org/10.15520/jmcrr.v3i08.201","url":null,"abstract":"<p><strong>Background: </strong><i>Annona muricata,</i> commonly known as Graviola, soursop or guanabana, is an evergreen tree native to the tropics with a long history of use in ethnomedicine in indigenous communities in Africa and South America. Its active phytoconstituents have provided medicinal benefits against various ailments and diseases such as arthritis, parasitic infection, hypertension, fever, or diabetes. Studies conducted <i>in vitro</i> and <i>in vivo</i> have concluded that Graviola phytocomponents have anti-cancer and anti-tumor properties. One of the characteristics of cancer cells is their uncontrolled proliferation rate. In that sense, molecules that inhibit cell proliferation offer potential therapeutical benefits.</p><p><strong>Methods: </strong>We prepared ethanolic and aqueous extracts from dried Graviola leaves and tested their respective antiproliferative activities on tongue Squamous Cell Carcinoma cell line-25. We treated the cells with increasing concentrations of the extracts for 24 h. The respective doses leading to a 50% inhibition of cells growth (GI<sub>50</sub>) were determined.</p><p><strong>Results: </strong>Our results showed that the ethanolic extract was 4 times more active in inhibiting the growth of Squamous Cell Carcinoma cell line-25 than the aqueous extract (respective GI<sub>50</sub> of 61.7 μg/mL, and 274.6 μg/mL).</p><p><strong>Conclusion: </strong>We hypothesize that some organic compounds involved in the antiproliferative/cytotoxicity of Graviola leaves were selectively extracted by Ethanol. Future plans include characterizing those bioactive compounds and assessing their activity on Squamous Cell Carcinoma cell line-25 vs. non-cancerous oral cells. The assumption in this type of analysis is that if an identical nucleotide sequence codes for an identical peptide sequence in two different proteins in two different viruses; then, this is evidence of “descent from a common ancestor”. This is the Darwinian assumption and is the most common method of analysis. Just recently, for example, the following sequence VLLFLAFVV was identified as a common epitope in the Envelope protein (E) of both SARS and COVID19 [3] and they suggest this as the epitope for the basis of a vaccine. Their work is quite pretty. However, we feel that this may not be the best epitope on which to base a vaccine. We have two reasons. However, we report here an exceptional case of long term evolution of this malignancy. The patient was a 73-year-old man who was operated of a sarcomatoid carcinoma of the pancreas 10 years ago. He received adjuvant chemotherapy with gemcitabine. Our patient was followed-up regularly. A routine check-up revealed that the patient was free of tumor recurrence and metastasis during 10 years. After this period of recovery, the patient presented a colonic obstruction due to a metastatic disease . Indeed, the early stage and the radical treatment may enhance the prognosis of sarcomatoid carcinoma.
