Yarustovsky","doi":"10.21320/1818-474x-2020-s1-9-120","DOIUrl":"https://doi.org/10.21320/1818-474x-2020-s1-9-120","url":null,"abstract":"In the process of developing the recommendations, the publications of the official websites of the Russian Federation, the electronic databases of the RSCI, PubMed, MEDLINE, EMBASE and the Cochrane Central Register of Controlled Trials (CENTRAL) were analyzed by the developers independently of each other. The date of the last search query was November 1, 2021. To develop the recommendations for the Guidelines, documents were used that directly describe the features of the management of patients with a new coronavirus infection COVID-19 (recommendations and guidelines - 35; randomized clinical trials and Cochrane Reviews - 23; observational and comparative studies - 134; other documents, notes and comments - 72). Compared to the previous, 5th, version of the recommendations, 35 provisions have been corrected in 10 sections. The provisions of the current version of the recommendations highlight the specifics of anesthesia, intensive care, rehabilitation, resuscitation measures, manipulation, transportation, prevention of the spread of COVID-19 in the implementation of these activities. Methods of protecting personnel from infection with COVID-19 during manipulations, anesthesia and intensive care are considered. The features of respiratory support, extracorporeal detoxification, extracorporeal membrane oxygenation, thromboprophylaxis, drug interactions are described. The features of the management of pregnant women, children, patients with concomitant diseases, the principles of the formation of stocks of drugs and consumables are considered. It is in this category of patients that the likelihood of developing perioperative cardiac complications is highest, which requires the use of cardioprotective measures to reduce the risk of their occurrence. One of the options for cardioprotection may be the use of drugs aimed at improving metabolism and maintaining adequate energy supply to the myocardium. Such metabolic agent is exogenous phosphocreatine. This article provides an overview of studies on the use of exogenous phosphocreatine as a cardioprotector in patients undergoing noncardiac surgery. The publications were searched for in PubMed / Medline, Embase, e-library, China National Knowledge Infrastructure, WANGFANG DATA. It has been shown that the use of phosphocreatine (PhC) in abdominal surgery, oncosurgery, during operations in traumatology, thoracic surgery, transplantation, as well as in intensive care units for sepsis and septic shock is associated with a decrease in the risk of cardiac complications and a reduction in mortality. Обзорная статья предлагает прежде всего знакомство с ключевыми научными направлениями, по которым на данный момент развивается теория ПОН (исследование значимости аларминов, митохондриальная дисфункция, барьерная недостаточность, иммунологическое и неврологическое сопряжение, формы программированной гибели клеток, индуцированная иммуносупрессия, разрешение воспаления). The authors did not evaluate the effect of the urgency o f spinal decompression on the severity and duration of systemic hypotension in this category of patients previously, as well as the presence of a connection between systemic hypotension and the outcome of the injury. Objective . Впервые РФС был описан более 70 лет назад, но до сих пор мало исследован. Основа патогенеза РФС — выраженные водно-электролитные и метаболические нарушения, провоцируемые возобновлением питания, на фоне истощения запасов калия, магния, фосфора, приводящие к полиорганной недостаточности. Гипофосфатемия является основным диагностическим критерием РФС, при этом у пациентов отделения реанимации и интенсивной терапии (ОРИТ) существует множество других причин гипофосфатемии, что значительно затрудняет диагностику. Большинство исследований, посвященных РФС, проведены среди пациентов с нервной анорексией. В ОРИТ встречается около 34 % случаев РФС, но до сих пор все рекомендации по ведению этих больных были перенесены из практики лечения нервной анорексии и были основаны на мнениях экспертов. За последние годы появилось несколько крупных работ, которые доказали эффективность гипокалорийного подхода в лечении РФС у пациентов в критических состояниях.
