{"title":"Intracranial Hypotension and Bilateral Isolated Cortical Vein Thrombosis: A Case Report and Literature Review","authors":"Andreas Filippaios","doi":"10.31579/2690-4861/316","DOIUrl":"https://doi.org/10.31579/2690-4861/316","url":null,"abstract":"Background: Intracranial hypotension (IH) is a clinical manifestation of low cerebrospinal fluid pressure with reports of isolated cortical venous thrombosis (ICVT) rarely described. Case Presentation: A 23-year-old man presented with postural headache followed by blurry vision and horizontal binocular diplopia with extreme gaze. Patient workup showed diffuse thickening and enhancement of the dura suggestive of intracranial hypotension and biparietal superficial cortical vein thrombosis was noted on imaging. The patient was started on folate and therapeutic anticoagulation and his condition improved. Conclusion: Patients with IH are at risk for CVT due to the dilation of the venous sinuses of the brain. Changes in the characteristics of the headache of the patient or the onset of new neurological symptoms should warrant further investigation. {"title":"Gold-Induced Cytokine (Goldic®) – A Promising Treatment in Patients with Grade 4 Knee Osteoarthritis: A Case Study","authors":"Pooja Pithadia","doi":"10.31579/2690-4861/318","DOIUrl":"https://doi.org/10.31579/2690-4861/318","url":null,"abstract":"Background: Knee osteoarthritis (KOA) is one of the most common degenerative diseases that can lead to disability and pain. The degenerative nature of this condition cannot be reversed or healed by any currently available treatments. Recently, there has been an upcoming interest in treating degenerative tissue disorders with minimally invasive autologous blood products. Autologous blood was preconditioned with gold particles to encourage the production of various proteins in patients’ blood. This current study investigated the safety and efficacy of pre-conditioning autologous blood with gold particles (GOLDIC®) in patients with severe knee osteoarthritis (KOA). Case presentation We report a case in which four intra-articular GOLDIC® injections were used to treat knee osteoarthritis. After a week, physiotherapy was recommended. Before and three weeks after the injections, she was assessed for her weight, the Visual Analog Scale (VAS), and the Western Ontario and McMaster University Osteoarthritis Index (WOMAC) score. GOLDIC® treatment resulted in significant weight, VAS, and WOMAC score improvements without serious side effects. Conclusion Due to the one-time blood harvesting and only four injections, the initial results demonstrated that the treatment plan is safe, less expensive, patient-friendly, and more successful at achieving compliance. This GOLDIC® treatment can significantly decrease pain and improve knee capability with overall satisfaction over a significant period. The ancient Greeks were also aware of aloe’s medicinal effects as both Pliny (23-79 AD) and Dioscorides (1st century AD) wrote of aloe’s ability to treat wounds and heal infections of the skin. Aspirin having various function like anti-inflammatory, analgesic, antipyretic etc. Aspirin loaded nanosuspension was prepared to enhance the bioavailability of the drug. Nanosuspension is defined as a finely dispersed solid particle dissolved in aqueous and organic medium for oral, tropical, parenteral or pulmonary administration. The size ranges about 200nm to 600nm.Aspirin loaded nanosuspension was prepared by probe sonication method. The probe sonication method is used to reduce the particle size and to increase the solubility of the drug. Total 5 Nano formulations were prepared in which NS1 shows 100% of drug content. Invitro Drug release was performed using Franz diffusion cell in which NS5 formulation shows 94.7% drug release in a time period of two hours and 30 minutes. Entrapment efficiency was performed using Ultracentrifugation method kept for 40 minutes at 17,000 rpm speed. International guidelines also reported that early mobilization is safe and feasible, starting in the ICU setting. Therefore, this clinical literature review aimed to designate the indications of physical rehabilitation programs and early mobilization benefits to add to the existing literature the necessary up-to-date recommendations and share the experience of a tertiary healthcare facility in dealing with different stages of post-COVID-19 patients.
