They represent about 1-2 % of all bony injuries in children [2]. Seventy percentage of femoral fractures involve the shaft [3]. The cause of these injuries varies with age; in preschool children the most common mechanism is a fall from a height. In children aged 4-12 years accidental injury during sport is the predominant cause [4]. A vast majority of femur fractures in children heal without any longterm sequelae [4]. With better understanding of biology of fracture healing and with advances in fixation methods and operative techniques, there has been a general trend toward operative stabilization of femoral shaft fractures in children. Operative treatment options for femoral fractures in children include plating, rigid intramedullary nailing, flexible intramedullary nailing, and external fixation. However, Martinez [5] reported excessive shortening and angular deformity in around 43 % of their patients treated by an early spica cast. Hughes [6] studied the impact of immediate spica casting on patients and families following femoral shaft fractures. They found that the greatest problems encountered by the family in caring for a child in a spica cast were transportation, cast intolerance by the child, and keeping the child clean [4-6]. Although most authors still recommend conservative methods of treatment in pediatric femur fractures in preschool children, there is still confusion regarding the optimal method for children beyond this age group [7]. In this study we compare two main methods in the treatment of isolated femoral fractures in four and five years old children which are intramedullary nailing and spica casts to assess the best method of management of femoral fractures in this age (Table 1). It is currently practiced in more than 200 countries throughout the world and became an Olympic discipline for the first time at the 2000 Games in Sydney. TKD is a full-contact martial art characterised by its emphasis on dynamic attack and kicking techniques requiring rapid reactions, speed, agility and endurance. TKD is a popular sport in Germany, where the number of active athletes has risen considerably in the last few years. Injuries to the hamstring group of muscles generally occur in the two-articulated sections following sudden over-extension of the muscles with a maximum degree of hip flexion and knee extension [1]. So far, sprint sports such as American football, in which the rapid changeover from muscle contraction to relaxation occurs, were known to be high-risk sports [2]. In TKD such movement patterns during explosive high kicks are especially important, as attacks to the opponent’s head are only permitted with the feet and generally score highly. The frequency of general injuries to the hip is approx. 11 % [3-5] and to the lower extremities 44.5 % [6]. To date there is no explicit data on the frequency and patterns of hamstring injuries in TKD, as information on the occurrence of TKD injuries in the literature is unspecific [3,4]. It usually occurs as a result of blow to the body, a fall, or accident. Traumatic injuries account the majority of the fractures, although some fractures are secondary to a disease (pathological fractures). The highest incidence of fracture in males is between the age group of 15 to 24 years and in women it is 65 years or above [1]. Majority of patients with injuries of the musculoskeletal system will be managed by bed rest, immobilization and rehabilitation itself without requirement of surgical intervention. Immobilization is achieved by orthopedic appliances. These appliances produce skeletal changes by applying force with the help of plaster cast and braces [2]. The patient with plaster cast may develop cast syndrome due to obstruction of superior mesenteric artery and it can be prevented if symptoms are recognized quickly and treatment instituted in a timely manner. Expected complications of plaster of Paris cast are impaired blood flow, nerve damage, tissue necrosis, infection, cast syndrome and also other complications may arise due to prolonged immobilization like hypostatic pneumonia, foot drop, renal calculi, stiffness of joints, constipation, pressure ulcer etc [3]. A study was conducted to assess the knowledge on fracture among 127 osteoporosis patients with recent fragility fracture at Hamilton Health Sciences hospital. The results showed that, majority of patients were unaware of important risk factors related to fracture. The study concluded that education initiatives are essential in improving knowledge among individuals in fracture clinics, acute care and rehabilitation settings, as these areas are an ideal place to communicate information [4]. Majority of musculoskeletal disorders are relatively long time in nature. The care and management of this condition needs a specialized orthopedic nursing practice5. After going through the related literature it was found that there were no studies either in India or abroad specifically related to management of orthopedic appliances. Hence the researcher felt with an objective to evaluate the effectiveness of structures health education programmed on management of orthopedic appliances among fracture patients. They refer to this cross theoretical hybridization as the behavioural theory of corporate governance. Within that framework, they distinguish two main mechanisms that impact on behaviour and actions of actors involved in governance processes in corporations, i.e. the socially situated and socially constituted agency [1,2]. PAT, as an under socialized and actor centric theory, concentrates on examining patterns, according to which individuals voluntarily, however rationally, realize their own goal agendas. They are motivated by self-interest and differential personal risk preferences, as well as are subject to informational and incentive constraints. In effect, PAT governance mechanisms tend to be formal in nature. They take a form of either incentive for managers as agents or means of monitoring/controlling them. They are construed to provide safeguards against such actions of managers, who driven by their self-interest may be potentially deviating from the desired organizational and/or societal outcomes. This unfolds by aligning managerial interests with those of shareholders or disciplining managers as agents. Infuse the agency relationships with the social context. They emphasise that corporate leaders do not operate in the social vacuum. On the contrary, they act in the socially constructed and interpreted reality.
