On the other hand, use of preservatives in glaucoma medications is a common practice in order to maintain their effectiveness for an extended period of time. Anti-glaucoma medications enclose preservatives and the 70% of them contain a very common preserving agent, the Benzalkonium chloride (BAK). In glaucoma medications BAK toxicity includes, loss of tear film stability, direct damages to the cornea and conjunctival epithelium, and possible immune allergic reactions. Other types of preservatives may exhibit less toxicity but are still irritating agents. Of course, concentration and duration of treatment determine the extent of side effects of the preservatives. High content hydrogel contact lenses absorb BAK molecules and release them slowly onto the corneal epithelium over time. So, patients that use contact lenses may wear those better 15 minutes after the medication. The prolonged exposure to BAK is well known to increase symptoms of ocular inflammation. A diminish of patient's life quality is observed and patient compliance to the specific anti-glaucoma medication, is also undermined. Conversely, there are more causes of dry eye in glaucoma patients, as the majority of the glaucomatous patients are elderly. Such factor is the decrease of tear production with increasing of age and numerous changes in the composition of the tear film. At the same time, elderly often suffer from other pathological diseases that encourage the development of dry eye and eye irritations. Adding the anti-glaucoma eye drops containing preservatives along with the constant use of contact lenses, we are led to a serious multifactorial dry eye condition. The mantle cell lymphoma represents 6% of cases of LNH B. We report a patient of 60 years with bilateral exophthalmia revealing a mantle cell lymphoma. Clinical Observation: A man of 60 years ran for ophthalmological emergencies in an array of orbital cellulitis of the right eye. This swelling was that day, painful, inflammatory, axile, irreducible, and non-pulsatile, with limited horizontal movement of the eyeball. In general terms, the patient has axillary lymph nodes and supraclavicular without splenomegaly. A biopsy of the orbital mass produced by trans-conjunctival, concluded in a non-Hodgkin lymphoma B type mantle (CD20 +, CD5 +, cyclin D1 +). The assessment of extension realized in the Hematology Clinic department shows that this is a mantle cell lymphoma NHL B stage (nodes above and below the diaphragm, bilateral eyelid damage, spinal cord and location). After 3 treatments Rituximab-CHOP and DHAP-Rituximab treatments 3, obtaining a complete remission. Discussion: The mantle cell lymphoma represents about 6% of NHL. Its incidence is estimated between 0.07 and three cases per 100 000 population per year. The geographical area and ethnicity shows a higher frequency in Caucasians than in African Americans. Ocular involvement in the NHL is a rare event, as evidenced by the limited number of cases reported in the literature. It may be opening that is the case of our patient, or alter the course of lymphoma known. Conclusion: The eye location of mantle cell lymphoma is a rare entity. The clinical picture of this location can be misleading, particularly that of orbital cellulitis. The tumor biopsy confirms the diagnosis. Material and Methods: Population-based cross-sectional study that included 710 diabetic patients. All patients underwent an eye examination by indirect ophthalmoscope to check for any signs of DR. Participants were also interviewed and examined to determine their demographic characteristics, medical conditions and the realization of previous fundoscopic eye examination. Statistical studies were done with t-Student test, Fisher test or chi-square test. Results: Among 710 screened patients, 112 had some degree of diabetic retinopathy, and the overall standardized prevalence of any retinopathy was 16,3%, including 90 (80,4%) with non-proliferative and (22) 19,6% with proliferative diabetic retinopathy. Conclusions: The prevalence of DR in São José do Rio Preto is 16,3%. The main risk factors associated with DR were time of disease and glicemic control. Type of DM and nephropathy were considered secondary risk factors. Abdullah","doi":"10.33425/2639-9482.1003","DOIUrl":"https://doi.org/10.33425/2639-9482.1003","url":null,"abstract":"Introduction: PAMM, recently emerged retinal pathology involving preferentially deep retinal capillary system (ICP and DCP) where impaired blood flow occurs, and particularly anatomical and physiological watershed zone is affected (INL and OPL). Resultant ischemia is presented clinically with sudden drop of vision and manifested as band like hyperreflective lesions on SS OCT B scans. SS OCTA with Enface and Retinal flow maps have delineated ischemic lesions within middle retina and exhibited characteristic findings that may help in understanding the pathophysiology behind this entity. Purpose: To highlight PAMM diagnostic features on Optical Coherence Tomography Angiography and Enface, and to emphasize on their major role in detecting and assessing this condition. Methods: This is a retrospective study reviewing two clinical cases presented to our facility with sudden decrease in vision in one eye. DRI OCT Triton (Topcon Corporation, Tokyo, Japan) was employed. In addition to generating high quality conventional cross sectional B scan images, Enface C scans and three dimensional angiography images were obtained at different retinal levels. High-resolution color fundus imaging, Fluorescein Angiography (FA) and Fundus Auto fluorescence (FAF), and Enface in conjunction with OCTA were all used in the two cases. Detailed history, full comprehensive Eye exam was done, appropriate thorough systemic workup was performed; baseline blood tests (CBC, KFT, LFT, UA), coagulation profile (INR, APTT, D Dimer) with blood test for sickle cell (SC), and autoimmune profile. Results: In both cases, OCTA and Enface revealed multiple findings of PAMM during the acute phase of presentation. OCTA provided evidence of DCP ischemia by showing perfusion defects, or capillary dropout. Enface C scans visualized fern-like ischemic distribution at the level of DCP. Discussion: The use of OCTA in conjunction with Enface OCT has proved superior to other imaging modalities in identifying and diagnosing PAMM. Carneiro","doi":"10.33425/2639-9482.1001","DOIUrl":"https://doi.org/10.33425/2639-9482.1001","url":null,"abstract":"Introduction: Posterior lenticonus is a rare ectasia of the posterior capsule of the lens and has a prevalence of 1-4/100,000 and is associated with crystalline opacity in 14% of unilateral cases and 9% of bilateral cases. Depending on the degree of opacity and visual impairment, there is a risk of developing amblyopia but the effect on vision is extremely variable. We report two different treatment approaches to the disease. Case Report: The first case is a 10 months male patient with Down syndrome attended for evaluation of cataract with good fixation and no strabismus. Inicially a non surgical treatment was chosen but later he developed an agressive behavior and a posterior lenticone associated with posterior polar cataract was observed, which lead to the decision of a surgical treatment. The second case is a 7 years old patient with history of low visual acuity and posterior polar cataract in the left eye associated with posterior lenticonus discretely displaced superiorly. He was treated with tropicamide and oclusion of the right eye for thirty days. Discussion: The two cases shows how is it possible to manage posterior lenticonus with surgical ou clinical treatments. There is a tendency for patients who underwent cataract surgery to have a better visual acuity improvement compared with patients who did not have surgery, however this data was not statistically significant. In this method, geometrical boundaries of radial retinectomy are set up as to protect blood vessels, papil and macula. So far, out of totally 60 eyes who have received the treatment of this method, 2 eyes achieved 70% vision improvement, 4 eyes achieved 60%, 9 eyes achieved 50%, 16 eyes achived 40%, 14 eyes achieved 30%, 9 eyes achived 20%, and 6 eyes experienced 10% vision improvement.
