Bello, Ibrahim Kalle Kwaifa","doi":"10.4314/sokjmls.v8i3.11","DOIUrl":"https://doi.org/10.4314/sokjmls.v8i3.11","url":null,"abstract":"Endothelial dysfunction in diabetes manifests in part as reduction of nitric oxide (NO) bioavailability, which leads to inadequate relaxation of the vascular smooth muscle and a disparity between the vasoconstrictive and vasorelaxant intracellular pathways which favours a rise in vasoconstriction. In this current work, we evaluated serum Nitric oxide (NO) concentration and the activity of Nitric oxide synthase (NOS) in streptozotocin-induced diabetic male and female rats' serum. Our results showed initial and final fasting blood glucose concentration 5.30±0.16mmol/l and 5.24±.015mmol/l versus 5.68±0.18mmol/l and 5.43±0.15mmol/l in male and female controls. Among the diabetic rats, the initial and final fasting blood glucose concentration was 17.50±1.91mmol/l and 15.68±2.84mmol/l versus 20.50±4.76mmol/l and 19.40±4.13mmol/l in male and female rats respectively. NO concentration was 106.12±7.23μmol/l and 131.81±12.54 μmol/l in male and female control rats compared to 52.38±3.01μmol/l and 65.29±16.19 μmol/l in male and female diabetic rats respectively. Serum activity of NOS were 133.72±10.92μIU/l and 156.06±18.22 μIU/l in control male and female rats compared to 98.01±1.48 μIU/l, 78.89±8.39μIU/l in diabetic male and female rats respectively. The difference in both NO concentration and NOS activity was significant between diabetics and non-diabetic rats as well as between male and female diabetics rats (p< 0.05). Treatment approaches vary, including traditional and conventional methods. This study in Ekpoma, Nigeria assessed biomarkers in fracture healing through a cross-sectional random sampling. Blood samples from 60 subjects were analyzed for hydroxyproline, creatinine, calcium, alkaline phosphatase, and C-reactive protein. The aim was to evaluate treatment effectiveness and improvement in patient outcomes by monitoring bone remodeling. Statistical analysis utilized is the SPSS software version 21.0 software (SPSS Inc., Chicago, IL, USA). Biochemical marker analysis revealed that serum alkaline phosphatase activity, calcium, and creatinine did not significantly differ between subjects without fractures and those with fractures (p>0.05). However, hydroxyproline levels exhibited a significant difference (p<0.05), with higher values observed in subjects with fractures. Additionally, C-reactive protein levels showed significant variations (p<0.05), indicating increased inflammation in fracture patients. High-sensitive C-reactive protein levels also displayed significant differences (p<0.05). Further analysis comparing male and female subjects without fractures and those with fractures revealed no significant variations in serum alkaline phosphatase, calcium, and creatinine levels (p>0.05). However, hydroxyproline levels demonstrated significant variations (p<0.05) between males and females in the fracture group, suggesting gender-specific differences in bone metabolism. C-reactive protein and High-sensitive C-reactive protein levels exhibited significant variations (p<0.05) between males and females in the fracture group. Furthermore, a comparison between patients on traditional and conventional treatment methods indicated significant variations in serum alkaline phosphatase activity and calcium levels (p<0.05), suggesting distinct effects of the treatment modalities on these markers. However, no significant differences were observed in creatinine, C-reactive protein, and high-sensitive C-reactive protein levels (p>0.05). Traditional and conventional fracture treatment methods may affect biochemical markers differently, with gender-specific variations in hydroxyproline, C-reactive protein, and high-sensitive C-reactive protein levels. This may occur for a few hours or some weeks, making many of them to require hospital admission and blood transfusion, with inevitable financial implications on care givers. Due to its tissue specificity, cardiac troponin I and T have been claimed to be important in detecting cardiac damage even in the presence of elevated total creatine kinase (CK) and CK-MB that can likely occur in exercise-induced skeletal muscle damage. The significance of cardiac assessment in sickle cell patients was emphasized in a previous report which showed that evidence of mortality related to cardiovascular causes was often encountered during autopsy. This study aimed at accessing serum cardiac Troponin I (cTnI) in adult patients with sickle cell disease at University College Hospital, Ibadan. After written consents were obtained, 131 individuals above 18 years were recruited, 95 of which were sickle cell disease subjects and 36 were non-sickle cell disease controls. Mean age was 35.4years±SD=7.54years with age range of 19 to 56years for the sickle celled subjects and 36.8years±SD=9.11years with age range of 19 to 58 years for controls. Male to female ratio was approximately 2:1 for control and 2.4 :1 for sickle cell subjects. A total of 4(4.2%) sickle cell patients had abnormal troponin I level. There was a significant difference in mean troponin I level between sickle cell subjects and control, but there was no significant association between troponin I and haematological parameters of the sickle cell patients.
