Aguirre","doi":"10.21037/jlpm-22-47","DOIUrl":"https://doi.org/10.21037/jlpm-22-47","url":null,"abstract":"Background: Cell population data (CPD) are reported as part of leukocyte differentials by Mindray BC 6800Plus analyzer, give information of the size (Neu Z, Lym Z, Mon Z) nucleic acid content (Neu Y, Lym Y, Mon Y) and internal structure (Neu X, Lym X, Mon X) of leukocytes. We evaluated the potential utility of the leukocyte differential and CPD as laboratory indicators for the detection of coronavirus disease 2019 (COVID-19) in patients when admitted to the Emergency Department. Methods: A total of 672 patients with suspected infection were recruited at admission to the Hospital. The study group included 237 (151 COVID-19, 33 other virus, 53 bacterial infections). We applied the unsupervised K-means clustering method and principal component analysis (PCA). A validation group of 435 patients, 268 COVID-19 and 167 non-COVID-19 was used to verify the reliability of our model. Results: The COVID-19 cases presented the typical neutrophilia and lymphopenia, high Mon Z, and intermediate values of Neu X and Neu Y. The study group was classified into two clusters. This model was applied to the validation set;PCA analysis showed that almost 45.71% of the data variability could be explained by the two clusters. Cluster 1 had higher neutrophil counts, Neutrophil Lymphocyte ratio, Neu X, Neu Y and Mon Z, and Cluster 2, higher lymphocyte counts (P<0.05). Cluster 1 which included 91.4% of the COVID-19 patients and 60.5% of non-COVID-19 patients were assigned to Cluster 2, notably 100% of other viral infections. Conclusions: Leukocyte count (WBC) differential and CPD seem reliable parameters for the initial evaluation of patients with fever of unknown origin. By means of K analysis, the patients can be classified into distinct groups according to the etiology of the infection. © Journal of Laboratory and Precision Medicine. Ndrepepa","doi":"10.21037/jlpm-22-68","DOIUrl":"https://doi.org/10.21037/jlpm-22-68","url":null,"abstract":"Aspartate aminotransferase (AST) to alanine aminotransferase (ALT) ratio (De Ritis ratio) has been used as a marker of alcohol-related liver disease, liver fibrosis and muscle disease. This article reviewed the literature on the association of De Ritis ratio with cardiovascular disease (CVD). Recent studies support an association between elevated De Ritis ratio and prognosis of patients with diabetes mellitus, cancers, diseases characterized by multiorgan failure, CVD, stroke and corona virus disease (COVID)-19. Elevated De Ritis ratio may indicate increased cardiometabolic risk associated with overt or occult hepatic and extrahepatic diseases and may be a metabolic trait indicating abnormalities at the level of basic metabolism that foresees the development of future metabolic diseases. De Ritis ratio correlates positively with age, female sex, C-reactive protein and impaired renal function and inversely with diabetes mellitus, obesity and metabolic syndrome. Epidemiological studies suggest an association of elevated De Ritis ratio with CVD and strongly support an association between elevated De Ritis ratio and increased risk for all-cause and CVD-related mortality. The strength and direction of the association between De Ritis ratio and cardiovascular risk factors cannot explain the association between De Ritis ratio and CVD or CVD mortality. Elevated De Ritis ratio may represent cardiometabolic risk that is not mediated (or poorly mediated) by traditional risk factors and may be seen as an emerging nonstandard marker of cardiometabolic risk. De Ritis ratio requires standardization in terms of reference range and interpretation. Future epidemiological, clinical and laboratory (biochemical) studies are required to further investigate De Ritis ratio as a marker of cardiometabolic risk and CVD. © Journal of Laboratory and Precision Medicine. By the end of the article, the reader will be able to order and interpret appropriate investigations when faced with a patient with hyponatraemia or hypernatraemia. Methods: A narrative, focused literature review was performed using PubMed, OMIM and Google during September 2021 to January 2022 to identify references published from database inception to January 2022; reference lists from these articles were also used. Language was restricted to English. Key Content and Findings: Sodium is an important electrolyte, and its blood concentration is tightly controlled by the renin angiotensin aldosterone system (RAAS) and the regulation of antidiuretic hormone (ADH) secretion. However, sodium imbalance can occur in many disease states and can lead to significant morbidity and mortality, therefore finding the primary abnormality is important. A laboratory approach to investigate the cause of hyponatraemia and hypernatraemia is presented. It should be noted that sodium results are particularly susceptible to being falsely low or high in many circumstances, and the validity of sodium results should always be questioned before clinical investigation. Conclusions: Diagnostic flow charts are presented and the limitations of the laboratory tests discussed.
