Although contraception reduces these risks, data about contraception practices among Syrian child brides is sparse. This cross sectional, descriptive pilot study examined contraceptive knowledge, attitudes, barriers, needs and practices among young Syrian brides. A convenience sample of female Syrian refugees aged 13-25 who had married before the age of 18 was recruited through a civil society organization in Lebanon. Among the 32 participants, there were significant knowledge gaps and negative attitudes towards contraception, with approximately one fifth of participants (18.8%) unaware of contraceptive methods and 84.4% unaware of emergency contraception. Negative attitudes towards contraception were common, including beliefs that it was physically harmful (47.0%), contradicted religious views (43.8%), and lacked support by husbands (50%). The majority of participants (53.1%) had never used contraception with the most common reason being fear of side effects (47%). Approximately one-third (30%) of participants with two or more children reported sub-optimal birth spacing of less than a year and almost one-quarter of participants (24.1%) reported a history of terminating a pregnancy. Notably, one-fifth of participants (20.8%) had an unmet need for contraception, and unwanted pregnancies were common among women who were currently (42.9%) or previously (48.1%) pregnant. Results from this small convenience sample of Syrian child brides in Lebanon identify an urgent need to further explore contraception use among this population and to inform interventions for increased contraception usage to decrease adolescent pregnancies and improve maternal and child health.","PeriodicalId":92345,"journal":{"name":"Current opinion in gynecology and obstetrics","volume":"14 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-05-02","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"89186885","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Fetal weight was evaluated with Estimated Fetal Weight (EFW) formula, with the measurement of the Abdominal Circumference (AC) and with the assessment of the Abdominal Circumference corrected with the constant “c” (ACc).Results: 1030 patients with single pregnancy were included, 67.28% of them were primiparous, average Body Mass Index (BMI) at birth was 27.37 and macrosomia was found in 77 (8,08%) fetuses. EFW showed a sensitivity of 61.53%, a specificity of 90.72%, a positive predictive value (PPV) of 27.58% and negative predictive value (NPV) of 97.46%, to recognize birth weight of more than 4000 g. AC greater than 375 mm presented a sensitivity of 53.24%, a specificity of 93.21%, a PPV of 41.41% and a NPV of 95.67%, to detect macrosomic fetuses. ACc greater than 375 mm showed a sensitivity of 66.23%, a specificity of 88.65%, a PPV of 34.35% and a NPV of 96.68%, to predict macrosomia. The mean absolute error when the neonatal weight was greater than 4000 g, with EFW was 9,23%, with AC were 11,02% and with ACc was 9,32%.Conclusions: In this study was shown that either EFW, or AC-only, or ACc are useful tools to detect fetal macrosomia. The measurement of AC and Acc seem to be the easiest methods to learn and to be used every time. During the first stage of labor, ultrasound scan associated to clinic evaluation, provides further elements to physician, to allow a better management of delivery. The finding of a NPV greater than 95% with the three methods, leads to exclude macrosomia with a good approximation, even in case of clinic hypothesis.","PeriodicalId":92345,"journal":{"name":"Current opinion in gynecology and obstetrics","volume":"61 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-04-16","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"81263291","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

To date, many studies inepithelial ovarian cancer (EOC) have reported on the association HER-2/neu, p53 proteins and steroid hormones and their respective receptors with prognosis and/or the carcinogenesis process, but no definitive conclusion has been reached.Objectives: To assess the proteins c-erbB-2, p53, Ki67 and receptors of estrogen (ER) and progesterone (PR) of EOC, with regard to clinical stage findings and its effect on survival.Methods: 125 patients with a diagnosis of EOC treated by primary surgery and chemotherapy have participated. A surgical stage was noted and analyzed the correlation with c-erbB-2, p53, Ki67, ER and PR. Immunohistochemical analysis, using the anti-c-erbB-2, p53, Ki67 monoclonal antibodies, the antibody cod PR clone PgR and code ER-6-F11 Anti human estrogen. The c-erbB-2 study was complemented by genetic amplification and was reported univariate and multivariate analysis.Results: Age 55.7 ± 16; 50.2% with residual disease (< 2 cm); initial (54.6%) and advanced (45.4%) stage. Univariate analysis showed positive staining for c-erbB-2, p-53, Ki67, PR and ER. The patients with negative receptors had a significantly shortened survival time (p = 0.01) than patients with positive receptors. Multivariable analysis revealed only clinical FIGO stage as an independent prognostic of overall survival (p = 0.002). Other variables like c-erbB-2, p53, Ki67, and ER were not significantly related to survival.Conclusions: We concluded that patients with negative PR had a significantly shortened survival time than patients with positive receptors. The overexpression of markers c-erbB-2, p53, Ki67, and ER, were not significantly related to survival in EOC. Only the FIGO stage was achieved to be an independent predictor of overall survival. They should be evaluated together with the patient’s clinical status and other prognostic factors.","PeriodicalId":92345,"journal":{"name":"Current opinion in gynecology and obstetrics","volume":"93 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-04-08","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"79516759","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

As a result, PTB has a significant health and financial impact on health systems, families and societies. Its aetiology is not fully understood, but in most cases it is multifactorial, with several maternal, paternal, and epidemiological factors associated with increased risk. Other factors include parental ethnicity, maternal age and body mass index, socioeconomic status, and where the families live. This review examines the influence of ethnicity as an individual risk factor for PTB. It also explores its influence on the epidemiology of PTB and demonstrates that data on certain ethnicities are lacking, despite the fact that these ethnic clusters are within the very ‘high-risk groups’ that are adequately represented in some Western societies. This review examines the influence of ethnicity as an individual risk factor for PTB and also explores its influence on the different epidemiological aspects. A thorough revisit of the ethnic epidemiology unveiled other unnoticed risk factors that if addressed appropriately prematurity can be prevented. Moreover, certain ethnicities were not within the attention of researchers, despite the facts that they are very ‘high-risk groups’ and are also adequately represented in some Western societies.","PeriodicalId":92345,"journal":{"name":"Current opinion in gynecology and obstetrics","volume":"74 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-03-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"86368312","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Here we review the related research about ANG in reproduction. ANG was detected in different levels of follicle except in primordial stage in bovine and human and reveals that it may play role in follicle development and mature. During pregnancy, large amount of ANG dramatically expressed in maternal, amniotic fluid or fetal placenta, it may play a role in embryo implantation, the growth and development of fetus. The occurrence of reproduction related diseases such as infertility, endometriosis, ovarian hyperstimulation syndrome, gestational hypertension, fetal growth retardation and etc seems related with ANG. However, detailed research on the mechanism of ANG in related diseases is lack. Therefore, the further study about the role of ANG is of great significance for the understanding ANG function and may bring promising treatment of fertility, reproduction related diseases.","PeriodicalId":92345,"journal":{"name":"Current opinion in gynecology and obstetrics","volume":"29 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-02-20","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"78217626","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

They are usually stage I tumors and have a relatively good prognosis. Surgery is the mainstay of primary therapy. Recent studies showed that a minimally invasive laparoscopic approach is an option for selected patients with adult granulosa cell tumors. We report the case of a 91-year-old woman with an 18 cm solid adult granulosa cell tumor treated with laparoscopic surgery. The patient complained of pelvic discomfort and was found to have a solid ovarian tumor. The tumor was expected to be an early-stage sex cord-stromal tumor such as a granulosa cell tumor or thecoma because of the pelvic magnetic resonance imaging findings and abnormal hormonal blood test results. She underwent laparoscopic bilateral salpingo-oophorectomy. We successfully performed laparoscopic surgery in an elderly woman with an 18-cm solid adult granulosa cell tumor without scattering and spillage of the tumor, using a Vagi-Bag. She had no perioperative complications and recovery was rapid. She was doing well and disease-free at 8 months of follow-up.","PeriodicalId":92345,"journal":{"name":"Current opinion in gynecology and obstetrics","volume":"3 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-02-07","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"89830419","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Over 3% of all embryo transfers done in the United States involve GCs. Recent review has revealed high perinatal risks associated with assisted reproductive technologies (ART) for GCs. Compared to spontaneous conception, which the GC is familiar with, GCs using IVF have increased risk for twins and triplets of 20 and 46 fold, respectively. The resultant perinatal mortality rates are increased 4-6 fold. Costs in the first year of life for multiples are 5 to 20 times greater than for singletons. As GC cycles continue to increase, we must develop strategies to reduce perinatal risks for GCs and their offspring.Recent findings: Recent advances in IVF with pre-implantation genetic testing allow detection of euploidy for all 24 chromosomes. Elective single embryo transfer has reduced the risk of multiple pregnancy, preterm delivery and perinatal morbidity for women across the age spectrum.Summary: Elective euploid single embryo transfer, through the use of PGT, allows for very high pregnancy rates while reducing perinatal morbidity to natural levels. Both multiple gestation and miscarriage are dramatically reduced. Implementation of this technology in GCs will greatly enhance the safety of pregnancies, protecting this vulnerable population of altruistic women.","PeriodicalId":92345,"journal":{"name":"Current opinion in gynecology and obstetrics","volume":"19 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-01-22","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"86808176","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
