At present, the treatments of endometrial cancer focus on surgical treatment, followed by radiotherapy, chemotherapy and endocrine therapy. The progress of surgical method is transferring from traditional laparotomy, laparoscopy, single port laparoscope to Da Vinci robotic surgery system. Different surgical options should be personalized according to the patient’s situation. Sexual dysfunction affects a large proportion of the female population and appears to be associated with distorted genital anatomy although what women perceive as normal varies vastly depending on cultural and social beliefs. Cosmetic gynaecology office procedures are simple, quick, and effective solutions to improve sexual function as well as body image with minimal interventions and minimal side-effects. In this narrative review, we present these widely used minimally invasive aesthetic gynaecology interventions, focusing on their efficacy, and reported complications. In addition, Assisted Reproductive Techniques (ART) offers a variety of therapeutic approaches that aim towards fertility preservation and hormonal restoration after treatment. Ovarian Tissue Cryopreservation (OTC) and Ovarian Tissue Transplantation (OTT) have been proposed as an efficient alternative to reserve fertility and hormona function in cases of prepubertal young girls and women needed to be treated immediately with radiation and/or chemotherapy. In this narrative review, current scientific evidence is summarized in order to explore OTC/OTT. {"title":"Bilateral Krukenberg Tumor with Undetermined Origin during Pregnancy","authors":"J SiufiNeto, W. In 80% of the cases, Krukenberg tumors are bilateral and the prognosis is poor, especially during pregnancy, which can mimic symptoms and delay diagnosis. We present the case of a 25-year-old woman with a gestational age of 15 weeks diagnosed with bilateral ovarian tumors in a routine ultrasound. Investigation with colonoscopy, esophagogastroduodenoscopy and breast ultrasound was negative. The MRI showed two expansive formations in the pelvic-abdominal region. The right-sided mass measured 12.6 x 12.8 x 14.0 cm with a volume of 1174 cc and the left-sized mass measured 6.0 x 6.8 x 5.1 cm with a volume of 108 cc. Neither side showed signs of invasion of adjacent structures. It was also observed a marked thickening of the greater omentum, minimal thickening of the iliac peritoneal surface, and a large amount of free fluid in the abdominal cavity. Ca – 125 levels were higher than 251, with normal Ca 19-9 and CEA levels. A diagnostic laparoscopy was firstly performed and further bilateral adnexectomy with omentectomy. Pathology report revealed both ovaries infiltrated by epithelioid cells with irregular nuclei consistent with signet-cells. Immunohistochemistry was positive for cytokeratin 20 (CK20), CDX2 and focally positive for cytokeratin 7 (CK7). Based on these findings, the final pathology report was mucinous adenocarcinoma with “signet ring” cells (Krukenberg tumor) favoring gastrointestinal tract origin (CK20, CDX2 and CK7 positive). Due to the need for systemic treatment, pregnancy was interrupted at 31 weeks and a C-section was performed, still showing ascites and multiple tumor implants. A viable female infant was born, weighting 1,715 g with Apgar scores 9 and 10. The infant was admitted to the neonatal intensive care unit for further care and was discharged home after five weeks in good conditions. In this setting of forced displacement, child marriage, insecurity, and limited access to sexual and reproductive health services can lead to increased rates of adolescent pregnancy, which have been linked to exacerbated maternal morbidity and mortality. Family planning can help to delay childbirth, increase time between pregnancies, and empower women to make their own reproductive health choices. To date, there is limited research on the knowledge of, and attitudes towards, family planning among Syrian refugees in Lebanon.Objective: Identify knowledge, attitudes, and perceptions towards family planning among Syrian refugees with the overarching goal of informing response strategies to improve sexual and reproductive health for displaced Syrian families in Lebanon.Methods: A thematic qualitative analysis of focus group discussions conducted in Lebanon in January 2017 by the ABAAD Resource Center for Gender Equality. The sample of 99 participants included Syrian women, girls and men.Results: While contraceptive use was generally deemed acceptable by women and girls, husbands’ and mother-inlaws’ attitudes towards fertility influenced their decisions about its use in practice. Additionally, reliable family planning services and sexual and reproductive health education were perceived as seldom available to Syrian refugees in Lebanon. Participants suggested that family planning awareness programs were needed for both parents and girls.Discussion: Changes at the policy, service, community, and individual levels are required to increase knowledge regarding and access to family planning services for Syrian refugees in Lebanon. It affects up to 15% of women in the reproductive age group and is often associated with pelvic pain and subfertility. Different theories explain how retrograde menstruation gives rise to endometrial deposits at ectopic sites, and how several other factors are involved in the progression of the disease. Its final diagnosis is established through direct visualization at laparoscopy or surgery followed by histological confirmation. Available epidemiological reports along with histopathological observations and molecular studies shed interesting light on the pathogenetic basis of different variants of the disease like deep infiltrating endometriosis and ovarian endometriosis. Evidence also suggests that endometriosis is a neoplastic condition which serves as a precursor of ovarian and endometrial cancers. In the present review, we have attempted to take a stock of the current epidemiological and molecular knowledge regarding endometriosis associated cancers and develop a model of functional network with interactions among critical genomic factors regulating cellular processes leading to fibrotic reaction. It is being conjectured that cyclic bleeding associated with inflammatory and oxidative stress followed by repeated tissue injury and repair along with recurrent estrogenic stimulation and ovulatory events in the pelvic environment bring about the complex phenotype of atypical endometriosis and associated neoplasm with a trade-off malignant transformation in high risk population. {"title":"Unusual Early Cerebral Metastasis of Ovarian Carcinoma as Initial Presentation","authors":"Q. In general, the outcome for ovarian carcinoma with brain metastases is dismal as most of these patients usually present late with metastases elsewhere.Materials and methods: A retrospective review of four cases with cerebral metastases as the initial presentation of ovarian carcinoma in patients ages 25 to 45 years. Three patients received hormonal therapy for infertility. All cases presented initially with symptoms and signs suggestive of increased intracranial pressure with unknown primary pathology. CT and MRI scans showed intracranial enhancing lesions in different locations. Full malignancy work up showed a primary lesion arising from the pelvis with no other lesions detected.Results: Three patients underwent laparotomy for ovarian carcinoma resection once their neurological condition optimized, one case died early before she had laparotomy. All cases underwent Craniotomy for excision of the solitary metastatic lesion. In all cases, the histopathology demonstrated metastatic ovarian carcinoma. Subsequently, patients received chemo-radiotherpy. Although treatment all cases died within two years.Conclusion: It is unusual for ovarian carcinoma to present initially with CNS involvement. Unlike other primary tumours, ovarian carcinoma does not have a predilection for the central nervous system. However, CNS involvement occurs at late stage of the disease. Although multimodal treatment the outcome still poor. Failure of tocolysis is a common situation in obstetrics with its share of risks and complications for the fetus.Objective: To identify the determinants of tocolysis failure in two Hospitals in Yaounde.Patients and methods: We carried out a case-control study over a 6 months period at Yaoundé Gyneco-Obstetric and Pediatric Hospital (YGOPH) and Yaounde Central Hospital (YCH). We compared each case (patient with failure of tocolysis after 48 hours) with 3 controls (patients with successful tocolysis after 48 hours). The association degree between two variables was measured by calculating the odds ratio (OR) with their confidence interval.
