The combination of finite element analysis (FEM) and compositional fluid dynamics (CFD), known as FSI, is widely regarded as the most advanced tool available for analyzing pressures and stresses within an aneurysm. However, it is also recognized as being both costly and time-consuming. In general, the accuracy of the solutions obtained using this method is heavily reliant on the quantity and quality of the grid elements employed. An important reason of performing this analysis is to investigate the controversial recommendations of adding blood flow inside the deformable models – which increases the cost and the time of the analysis – against using uniform pressure avoiding the complexity of blood flow analysis. Or using the blood flow analysis in a non-deformable aneurysm shape. The pressure acting on the inner wall is the major determinant of the wall stresses and it leads to wall deformation which will normally affect flow behavior. It is debated that pressure variations, due to fluid motion, can significantly affect wall stress results. And it is also debated that deformation of the aneurysm wall due to acting pressure can affect the blood flow behavior and lead to more or less pressure distribution on the wall. Clinical case: A 41-year-old male patient without comorbidities underwent laparoscopic cholecystectomy, during which bile leak from the accessory duct was detected. Double ligation was performed. During the postsurgical period, the patient presented a good clinical evolution. No evidence of collections, fistulas or any complications. Discussion: Different case series demonstrate that injuries during laparoscopic cholecystectomy are usually recognized during the procedure or more commonly in the immediate postoperative period. In the immediate postoperative period, the presence of bile output through a drain or peritoneal signs should imply a biliary injury. Localized accumulation of bile can produce a sterile bilioma , subhepatic or interhepatophrenic abscess . {"title":"Essential Knowledge to Impart to Young Women about the Specifics of Human Sexuality/ Reproduction","authors":"","doi":"10.14738/bjhmr.105.15687","DOIUrl":"https://doi.org/10.14738/bjhmr.105.15687","url":null,"abstract":"Homo sapiens appeared some 300,000 years ago and our female ancestors delivered obligatorily vaginally prior to the development of medical interventions (such as caesarean sections, a safe procedure only since the 1960’s). Furthermore, women are unique among the 4,300 mammal species suffer from a harmful complication of pregnancy, preeclampsia (especially during a first pregnancy). This paper explains first, concerning physiological vaginal first birth (primiparity) why young women should consider delivering her first baby before the age of 26 (there is a linear association between maternal ages and healthy deliveries). Second, because of the comparatively large human fetal brain, nutritional maternal-foetal exchanges are twice those of other mammals (per body-mass ratios). But, as the human species has retained the hemochorial placenta, that first appeared in mammalian evolution, women face the great challenge of immunological tolerance towards the male partner’s tissues during the nine months of gestation. This major challenge occurs during the first pregnancy and tolerance is related to semen exposure prior to conception.  In subsequent additional pregnancies, immunological recognition is largely achieved through the initial pregnancy. This is why for a first pregnancy (nulliparous woman or multiparous woman with a new male partner), it is preferable to conceive after a delay of 6 months of non-barrier contraception before planning conception. In case of short sexual cohabitation since the constitution of the couple (partial recognition of the male’s tissue), implantation of the embryo may be partially insufficient, leading to complications such as fetal growth restriction and/or early onset preeclampsia. Third, to have a baby neither too small, nor too big, women overweight or obese at the beginning of any pregnancy must gain less weight during the 9 months of gestation as compared to their leaner counterparts. {"title":"Transverse-Sigmoid Colon Synchronous Volvulus in a Patient with Lennox-Gastaut Syndrome: Case Report","authors":"","doi":"10.14738/bjhmr.105.15614","DOIUrl":"https://doi.org/10.14738/bjhmr.105.15614","url":null,"abstract":"Introduction: Lennox-Gastaut syndrome (LSG) is an epileptic encephalopathy characterized by a triad: 1). tonic seizures during sleep with atonic and atypical absence seizures, 2). cognitive and 3). behavioral It is estimated that SLG affects between 1 to 2% of patients with epilepsy. In general, LGS often occurs in young children. The etiology of LGS can be classified as cryptogenic or symptomatic. Symptomatic etiology of LGS can be secondary to hypoxic ischemic encephalopathy, congenital brain malformation, vascular malformation, genetics (tuberous sclerosis), trauma, brain tumor, or perinatal meningoencephalitis. Clinical Case: This is a 29-year-old woman, who has a significant history of prolonged prostration; severe cognitive disorder, Lennox-Gastaut syndrome since childhood. She was admitted to the emergency department with symptoms of abdominal distension of seven days of evolution and generalized pain in all abdominal quadrants. A simple computed tomography of the abdomen was performed, where distension of bowel loops was observed at the expense of mesenteric torsion with data of colonic volvulus with little free fluid. It is evaluated by the general surgery service who determines emergency surgical management by means of exploratory laparotomy, the following findings are addressed and obtained: toxic megacolon from the left end of the transverse colon to the middle third of the sigmoid colon, with multiple necrotic patches, as well as volvulus of the transverse and sigmoid colon, proceeding to perform a left hemicolectomy with colostomy of the transverse colon. Discussion: In children, the incidence is estimated at 2 per 100,000, and it is common in men. LGS is one of the most commonly encountered drug-resistant epileptic encephalopathies, with mortality rates ranging from 3-7%. This figure is predicted to rise further to 3,766 by 2030. The number of falls can be reduced by up to 30% through development of a multi-agency falls pathway focussing on early identification and prevention, and multi-factorial assessment and intervention for people at high risk of falling (1). The role of the health-care assistant (HCA) has developed rapidly in general practice and HCAs can make an increasingly useful contribution to the skill mix in general practice. The project aimed to evaluate; the suitability of HCAs delivering GaitSmart assessments in the GP clinical pathway, GaitSmart protocol for detecting gait deficiency and improving gait and patient reported outcomes (Gait Speed, GaitSmart Score, Falls Efficacy Scale Questionnaire (FES-I) and Edmonton Frailty Score (EFS). The programme produced positive patient outcomes, thus reducing the burden on physiotherapists. Adult women and seniors are most affected and are commonly associated with urinary tract obstruction and infection. Children are rarely affected, as well as bilateral impairment. Clinical Case: A 54-year-old female comes to the emergency room with right lower back pain, nausea, vomiting, and fever. She denies chronic degenerative diseases and refers to herself as a carrier of right renal lithiasis. Surgical History: open cholecystectomy 20 years ago, two cesarean sections 33 and 23 years ago. The current condition began two months ago with pain in the right lumbar region, accompanied by intermittent fever during the day, presenting partial improvement with analgesics and antipyretics. In the last 24 hours, he presented vomiting of gastro biliary content on four occasions and increased lumbar pain. Upon admission, we performed abdominopelvic tomography, in which an enlarged right kidney was observed, with extensive loss of morphology with ectasia throughout the collecting system that extends to the UP junction with the presence of a 20x20 mm stone with a density of 800 HU inside, ureter with slight ectasia along its entire length, identifying the bear claw sign. Due to tomographic findings with obstructive uropathy and in the presence of data of systemic inflammatory response, he decided to perform diversion of the urinary tract with a double J ureteral catheter without success despite several attempts. Discussion: The precise pathophysiology remains incompletely understood. He was born prematurely (29 weeks) after a twin gestation, weighing 1,900 grams, and with severe macroglossia, mild omphalocele, bilateral inguinal hernia, and episodes of transient non-ketotic hypoglycemia in the neonatal period. In addition to the characteristic phenotype of BWS, he had bilateral heterochromia of the iris and hypopigmented skin macules on the skin of the abdomen and back. The genetic study confirmed the existence of an aberrant mutation of the H19 gene in the chromosomal region 11p15.5 and the paternal origin of the genetic alteration (paternal mosaicism). The patient was diagnosed at 5 months of age with hepatoblastoma that was resected at 9 months of age and received neoadjuvant chemotherapy. Their etiopathogenesis is unknown to date; it is believed that they are formed acquired by an increase in pressure within the large intestine. Aim: Present the experience of 10 years in diagnosis and treatment, in a study of four hospital centers in Mexico City. Method: It is a study with a retrospective, longitudinal, observational and descriptive design. In the Colon and Rectal Surgery services, as well as second and third level General Surgery. Results: They were classified into three groups according to incidence/presentation, with a total of 219 records, where there were a total of 123 patients with acute complicated pseudodiverticular disease, 25 cases with chronic presentation were studied, and recurrent lower gastrointestinal bleeding where 71 subjects were reported. Discussion: Treatment in acute and/or chronic complicated pseudodiverticular disease, management is surgical with no option to make a mistake, since morbidity and mortality increase exponentially with the delay of definitively decisive surgical therapy. Additionally, elective surgery reduces, but does not eliminate, the risk of recurrent diverticulitis. Conclusions: Pseudodiverticular disease is a sequela of a commonly undiagnosed underlying disease. Objective: the objective of this study was to investigate worker’s experiences on childbirth complications and the risk factors that affects pregnant women who visit health facilities. The research's driving questions were, \"What are the most frequent problems during childbirth in Mogadishu, Somalia?\" What do medical professionals anticipate as the potential Risk factors of delivery complications? Materials and Methods: The study's cross-sectional assessment of health worker’s experiences on childbirth complications and their risk factors were planned to be gathered from data at a specified group of healthcare facilities in Mogadishu, Somalia, The study's target population consisted of 78 health workers who were asked to respond to childbirth complications; 65 participants were chosen using non-probability methods. The questionnaires were used, and then analyzed in excel and SPSS into frequency table and descriptive statistics (Mean and standard deviations). Results: The significance of discovering the highest percentage that was a Health worker’s experiences on childbirth complication in Mogadishu, Somalia, including \"labor that does not progress (prolonged labor),\" which is 67.7% of respondents who strongly agreed and agreed the existence of this complication at the Mogadishu maternal health center. The second highest percentage was \"water breaking early (preterm PROM),\" with 60% of respondents strongly agreeing or agreeing that this was a common childbirth complication.\". Besides the risk factors with the highest percentage, \"having sexually transmitted diseases and infections\" 61.6% of those polled strongly agreed or agreed. The second question with the highest percentage in risk factors on childbirth complication is \"Lack of near health services facilities,\" which has a score of 58.5%, which indicates the existence of unequally distributed health services in the community. Conclusion: The most frequent birthing complications were water breaking early (preterm PROM) and labor that did not progress (prolonged labor). Additionally to the risk factors with the highest percentages, \"having sexually transmitted diseases and infections\" and \"Lack of near health services facilities,\" which point to the presence of a high percentage of STDI and unequally distributed health services in the community, respectively. {"title":"Gastrointestinal Dependent Stromal Tumor of the Small Intestine (Ileum): Case Report and Literature Review","authors":"","doi":"10.14738/bjhmr.105.15570","DOIUrl":"https://doi.org/10.14738/bjhmr.105.15570","url":null,"abstract":"Introduction: The cells of Gastrointestinal Tumors show characteristics similar to those of the interstitial cells of Cajal, which play a neuromotor role in the regulation of normal intestinal motility. Clinical case: A 57-year-old female patient came to our service for abdominal pain accompanied by abdominal pain in the hypogastrium for 3 months. The pathological anatomy reports a tumor measuring 4 cm by 3 cm by 2 cm, 20 cm from the ileocecal valve. With a spindle cell pattern, in the immunohistochemical study it presents c-kit (+) and CD 34 (+), with a mitosis index < 5 per 50 field, it is considered low risk. He was referred to the medical oncology service, which decided on adjuvant treatment with imatinib mesylate. Discussion: GISTs are usually incidental findings diagnosed during other operations.
