Recent studies have shown that physical therapists (PT) can reliably measure the glenohumeral\u0000joint space using ultrasound imaging (USI) and operator experience or well defined training protocols influences the reliability. Few studies have\u0000reported the reliability of medial knee gap measurements with USI.\u0000Objective: Evaluate inter- and intra-rater reliability of a novel approach to measuring knee gapping using experienced and trained novice clinicians.\u0000Design: Inter-rater and intra-rater reliability study\u0000Setting: Physical therapy program\u0000Participants: Novice researchers were three upper-year Doctor of Physical Therapy students. Experienced researchers were two PT certified in\u0000MSK USI with eight years of experience.\u0000Methods: A total of 166 images of two subject’s medial knee joint gaps were captured in resting by a single experience researcher. Three novice\u0000researchers, who underwent a three-hour training protocol and two experienced researchers measured the images individually. Measurements were\u0000taken using standardized bony landmarks and internal calipers. All researchers were blinded to the subjects and results.\u0000Main outcome measures: Data was analyzed using two-way ICC mixed-model single measurement, SEM run for all researchers compared with\u0000experienced researcher one.\u0000Results: An excellent degree of reliability was found for both intra-rater and inter-rater measurements for Novice One and a good degree of\u0000reliability was found between Experienced One and Novice Two and Three.\u0000Conclusion: Both experienced and novice researchers attained a clinically significant ICC when compared to Experienced One. Akin","doi":"10.36879/jcmi.18.000106","DOIUrl":"https://doi.org/10.36879/jcmi.18.000106","url":null,"abstract":"Purpose: To evaluate the performance of maximum likelihood (ML) estimation of diffusion kurtosis (DK) imaging parameters in the prostate and\u0000compare the estimated parameters to those measured using least squares (LS) estimation.\u0000Materials and methods: The institutional review board issued a waiver of informed consent for this Health Insurance Portability and\u0000Accountability Act (HIPAA)-compliant, retrospective study of forty-two patients (median [Md] age=61 years; range: 43-74 years) who underwent\u0000magnetic resonance imaging (MRI) between September and October 2016. Diffusion-weighted MRI (DW-MRI) at nine b-values (0-2000 s/mm2\u0000)\u0000were acquired using a 3-T whole-body MRI unit (Discovery MR750; GE Medical Systems, Waukesha, WI) equipped with an eight-channel phased\u0000array coil for signal reception. Diffusion coefficient (D) and kurtosis (K) were estimated from the normal appearing prostate peripheral zone and\u0000prostate cancer regions of interest (ROIs). The parameters were estimated by fitting the measured MR signal intensities as a function of b-value,\u0000using LS and ML algorithms. An estimate of the noise was obtained on the b=0 images in an artifact-free ROI in the rectum. Simulations were also\u0000carried out to assess the properties of the two estimators in a range of signal-to-noise ratios.\u0000Results: For benign ROIs, the mean D ± standard deviation, (1.88±0.52)×10-3 mm2\u0000/sec, and mean K (0.79±0.20), measured using LS estimation,\u0000differed significantly from the mean D (1.96±0.48)×10-3 mm2\u0000/sec and mean K (0.68±0.21), measured using ML estimation (P<0.001 for both). For\u0000malignant ROIs, the mean D (1.48±0.38)×10-3 mm2\u0000/sec and mean K (0.94±0.20), measured using LS estimation, differed significantly from the\u0000mean D (1.54±0.36)×10-3 mm2\u0000/sec and mean K (0.81±0.19), measured using ML estimation (P<0.001 for both). Simulations demonstrate that ML\u0000minimizes the bias estimate of DK parameters within the signal-to-noise ratio range of 5-15.\u0000Conclusion: By incorporating the noise level, the ML estimation increases the accuracy of DK parameter estimation. Orbital fractures can have important consequences and are called blow-out, when\u0000there is a collapse of the floor or medial wall of the orbit, causing loss of ocular content. They are classified into two types: pure and impure. The risk of serious complications following colonoscopy is usually low. Hernial complications are rare after colonoscopy, and are\u0000probably promoted by an increased abdominal pressure and patient’s physical constitution. Inguinal hernia usually includes intestine and not\u0000parts of the urinary tract. In literature there no studies reporting cases of bladder herniation after a colonoscopy procedure. We presented a case of\u0000an 84-years-old man admitted to our emergency department reporting scrotum edema after a colonoscopy procedure; the abdominal computerized\u0000tomography scan showed a bladder herniation through the inguinal canal into the scrotum.
