An above elbow tourniquet is applied. A drip stand is then attached to the table at the level of the shoulder on the surgical side. The limb is prepared and draped, such that the sterile sheets completely cover the drip stand. Chinese finger traps are then placed upon the index and middle fingers. A four-inch crepe bandage is securely tied in to a loop and then hung from the drip stand. The finger traps can then be suspended from this so that the elbow is approximately six-inches from the arm table (Figure 1). The remainder of the four-inch crepe bandage is looped around the upper limb and tied underneath the arm table in to another loop, from which counter-traction weights can be suspended (Figure 2). We recommend sufficient counter traction such that the metacarpophalyngeal (MCP) joints of the index and middle fingers are both palpably and visibly distracted (Figure 3). This should enable adequate joint distraction, in addition to being able to manipulate the wrist in both coronal and sagittal planes as required during arthroscopic assessment. The set-up and traction is now complete, providing excellent access for portal placement and subsequent safe arthroscopy. Ayed","doi":"10.15406/mojor.2019.11.00469","DOIUrl":"https://doi.org/10.15406/mojor.2019.11.00469","url":null,"abstract":"First described in 1983 by Graham RV Hughes,1 antiphospholipid syndrome (APS) is a clinical-biological entity characterized by the association of thromboembolic events, classically recurrent, with obstetrical complications, and the permanent and significant presence of autoantibodies against the phospholipids of cell membranes.2,3 It is a rare entity that may be primitive or secondary to other autoimmune diseases, in particular systemic lupus erythematosus. Alsulaimani","doi":"10.15406/mojor.2019.11.00465","DOIUrl":"https://doi.org/10.15406/mojor.2019.11.00465","url":null,"abstract":"Patellofemoral pain syndrome (PFPS) is a common cause of knee pain in young people, especially who are reguarly active in sports involving running and jumping.1,2 PFPS usually involves pain in the anterior part of the knee or pain beneath or sides the patella, resulting from conditions such as Osgood-Schlatter, patella synovitis, patellar instability, patellar hypomobility, and chondromalacia.3 This pain and dysfunction results from excessive load or prolonged repetitive in the patellofemoral joint (PFJ).1 Another common reason for PFS pain are decrease strengthening of hip abductors and increase power of adduction muscles, knee ligaments injuries, and foot issues.1 Symptoms are commonly aggravated by walking up or down stairs, climbing or descending hill, squatting, lunge, running, biking and sitting with knee 90 degree for long time.1–4 Detailed subjective and objective examination sometimes involves imaging is necessary for diagnosis.2,4 To determine the exact source of pain leg strength, alignment, Qangle, and knee ROM, as well as check patella functions such as tracking, mobility, and normal anatomical features should be assessed.2–4 This research focuses on patellofemoral stability and mobility. Stability of the patellofemoral joint involves active and static stabilizers that control movement of the patella within the trochlea groove, known as patellar tracking, that are affected by stabilizing forces such as the patellar tendon, quadriceps tendon, and the nearby soft tissues such as iliotibial band. Loads on the patella during walking vary between one third and one half of body weight, approximately three times the body weight during walking up stairs, and with squatting reach up to seven times the body weight. The prevalence of this disease is between the third and fourth decade of life. The SVP was described by Chassignac in 1852 as a nodular lesion in the sheath of a flexor tendon of the hand.1,2 In 1864, its localized form in the knee was described.
