Arthroscopy is considered the most valid method for evaluation of cartilage lesions. The best way for grading, measure and dimension is direct visualisation. Methods : Patelar cartilage lesion was analysed macroscopically during 176 total knee replacement. The grading of cartilage lesion, by direct visualisation, indentified six diferents groups. Results: Patellar articular cartilage was damaged in all 176 cases. There was no evidence of association between grade of arthrosis, gender, side or age. There is an association between tricompartmental arthrosis, varus deformity, obesity and grade of patelar arthrosis. Conclusion: All classifications systems regarding the femoropatellar cartilage lesions are unrealistic. It is a rare entity with limited literature. We present a case of lipoma arborescens in 17-year-old boy who presented with complaints of mono-articular swelling of left knee for 2 years with history of antecedent trauma. MRI showed frond like masses present in supra and infrapatellar region with high signal intensity on T1W and Intermediate signal intensity on T2W, consistent with fat. He was managed with open synovectomy which showed yellowish white polypoidal and villous masses of adipose tissue. Masses were sent for histopatholgy which showed hypertrophic villous projections of fat lined by synovial cells. First description of Alkaptonuria was done by Garrod in 1908, that also recognized it to follow classic Mendelian recessive inheritance.1 The genetic defect is mapped to the HGD gene and more than 90 different mutations have been identified to date.2 Alkaptonuria is caused by homogentisic acid oxidase enzyme deficiency, that results in high homogentisic acid levels. It will eventually result in black deposits in skin, sclerae, connective tissues and urine. Ochronosis, dark pigmentation of connective tissues, is the musculoskeletal manifestation of this disease.3 It can lead to early degeneration of cartilage, leading to secondary osteoarthritis,4 by a mechanism that has not been understood.2 A worldwide review conducted in 2004 found that approximately 600 cases of ochronotic arthropathy have been reported since 1962.1 The diagnosis is often delayed because of its low prevalence and nonspecific early symptoms.4 The diagnosis occasionally is discovered intraoperatively in patients undergoing joint replacement surgery through the observation of the characteristic bluish-black pigmentation of the tissue surrounding the joint.2 Ochronotic arthropathy manifests first in the spine. Subsequent degeneration is on the knee (most commonly affected, in 64% of cases), hip, and shoulder. Depending on the number of spinal segments involved and the degree of degeneration, symptoms can range from mild muscular aches to severe neural compression that can compromise movement and sensation severely up to a point of disability. Treatment usually follows a hierarchy of steps or algorithm ranging from rest and medical treatment to extensive major surgery. The gait of a person is a peculiar kind of two-legged movement, in which the support for one leg cyclically changes the biaxial period, and then the support to the other leg. Gait is figuratively described as “controlled fall”. At each new step, the person bend over and begins to fall, which is hampered by the advanced leg. After it touches the earth, the weight of the body is transferred to it, the knee bends, and erects, returning the body to its original position.
