Introduction: Cervical carcinoma is the fourth most common gynecologic cancer. Screening assays that include the conventional cytology (CC) have a sensitivity and specificity far from optimal; however, liquid-based cytology (LBC) may overcome some limitations. The established problems with the functioning of various parts of the immune system (IS) in this pathology dictate the need to include immunomodulatory therapy into the complex. The developed program of combined immunotherapy for immunocompromised girls allows reducing the severity and duration of exacerbation of NCVV, their frequency against the background of a significant reduction in ARVI incidence. Positive clinical effects were observed with underlying restoration in IS functioning. A protective effect was obtained (observation in a catamnesis for 1 year): the duration of a clinically safe period increased from 6 to 11-11.5 months per year. Introduction Vulvovaginitis takes the first place in the structure of gynaecological disorders in girls of primary school age and preschool age, with the peak share at the age of 2-7 years old. Acute and chronic genital inflammations in childhood may later cause serious infections and inflammations in women of various ages and result in reproductive disorders in women of childbearing age. D","doi":"10.15744/2454-3284.4.202","DOIUrl":"https://doi.org/10.15744/2454-3284.4.202","url":null,"abstract":"Case report: A 26-year-old woman who was previously diagnosed as immature, right ovarian teratoma, was presented to Hangzhou obstetrics and gynecology hospital. The patient had previously undergone a laparotomy and right salpingo-oophorectomy with partial omentectomy 14-year before. Initial laboratory tests showed the serum markers: cancer antigen (CA)-125 was 44.5IU/ml (increased) and CA199, AFP levels were normal. Sonographic examination revealed a complex mass, measuring 12.6cmx10.5cm x8.5 cm, behind the uterus. Magnetic resonance imaging (MRI) findings were in favour of teratoma, originated from the right adnexa, occupying pouch of Douglas. The patient underwent exploratory laparotomy with a right intraligamentary teratoma cystectomy along with residual omentectomy,andrandom peritoneal biopsies. Among the various histotypes associated with hereditary ovarian cancer, high-grade serous epithelial ovarian carcinoma (HGSEOC) is the most predominant and women with inherited mutations in BRCA1 have a lifetime risk of 40-60%. HGSEOC is a challenge for clinical oncologists, due to late presentation of patient, diagnosis and high rate of relapse. Ovarian tumors have a wide range of clinical presentations including development of ascites as a result of deregulated endothelial function thereby causing increased vascular permeability of peritoneal vessels. The molecular mechanisms remain elusive. Studies have shown that fallopian tube cancers develop in women with BRCA1 gene mutations more often than previously suspected. Recent studies suggest that many primary peritoneal cancers and some high-grade serous epithelial ovarian carcinomas actually start in the fallopian tubes. In this article we have addressed the molecular pathway of a recently identified potential biomarker Ubc9 whose deregulated expression due to BRCA1 dysfunction can result in HGSEOC with peritoneal permeability and formation of ascites. We also discuss the role of downstream targets Caveolin-1 and Vascular Endothelial Growth Factor (VEGF) in the pathogenesis of ascites in ovarian carcinomas. Finally we hypothesize a signaling axis between Ubc9 over expression, loss of Caveolin-1 and induction of VEGF in BRCA1 mutant HGSEOC cells. We suggest that Ubc9-mediated stimulation of VEGF as a novel mechanism underlying ovarian cancer aggressiveness and ascites formation.
