{"title":"Application of Caprini risk assessment model in the implementation of VTE individualized protection scheme in COPD patients","authors":"Yanna Wang, Zhan-bin Luo, Haiying Quan","doi":"10.3760/CMA.J.CN221370-20190220-00386","DOIUrl":"https://doi.org/10.3760/CMA.J.CN221370-20190220-00386","url":null,"abstract":"Objective \u0000To explore the application effect of Caprini risk assessment model in patients with chronic obstructive pulmonary disease (COPD) undergoing individualized protection of venous thromboembolism (VTE). \u0000 \u0000 \u0000Methods \u0000A total of 60 cases of COPD patients admitted to the department of respiratory medicine of the hospital from January to June 2017 were selected as the control group. Caprini risk assessment and VTE risk classification were conducted within 24 hours upon admission, and conventional protective measures were adopted.60 patients who visited the hospital from January to June in 2018 were selected as the intervention group. Caprini risk assessment and VTE risk classification were conducted within 8 hours upon admission. Individualized protection scheme was implemented on the basis of routine care.The differences between the two groups were compared and analyzed. \u0000 \u0000 \u0000Results \u0000The incidence of PTE/DVT on day 14 in the intervention group was 3.34%, significantly lower than that of the control group (16.67%), and the difference was statistically significant (P<0.05). The levels of D-D and Fbg in the intervention group were lower than those in the control group, and the difference was statistically significant (P<0.05). The levels of PT, aPPT and TT in the intervention group were higher than those in the control group, and the differences were statistically significant (P<0.05). The length of stay in the intervention group was (16.2±3.2) days, significantly shorter than that in the control group (19.4±4.1) days, and the difference was statistically significant (P<0.05). The patients were divided into observation group and control group with 50 patients in each group according to the odd and even number of inpatient beds.The control group used routine methods to judge the patients' activity ability and perform routine activities on a daily basis.The observation group divided the activity ability of ICU patients into levels A, B, C, and D according to the factors affecting the early activities of ICU patients with severe pneumonia, and developed the corresponding level of activity programs. By daily assessment of the level of patients, the corresponding activity programs were adopted for activities.The Barthel index was used to judge the activity ability of daily life, and the differences between the two groups were compared.Meanwhile, the use time of ventilator, blood oxygen saturation at the time of discharge, average length of stay in ICU, and patients' satisfaction with nursing work were compared between the two groups. \u0000 \u0000 \u0000Results \u0000There was no significant difference in Barthel index score between the two groups(P>0.05). There was a statistically significant difference between the Barthel index score at the time of discharge and that at the time of admission(P<0.05). Meanwhile, the success rate of first aid, the success rate of transport and the emergency diagnosis rate were recorded.Anxiety and depression levels were assessed in both groups, and nursing satisfaction was investigated. \u0000 \u0000 \u0000Results \u0000The difference of response time, electrocardiogram time, total emergency time and emergency admission time between the experimental group and the control group was statistically significant (P<0.05). At the same time, the emergency diagnosis rate, transport success rate and first aid success rate of the experimental group were significantly increased compared with the control group, and the SAS and SDS scores of the experimental group were significantly reduced compared with the control group after nursing, the differences were statistically significant (P<0.05). After intervention, the scores of social function, psychological function and physical function of the two groups were significantly improved, and the scores of each dimension of the experimental group were significantly higher than that of the control group (P<0.05).
