{"title":"Rare Case of Sarcoidosis Presenting As Bilateral Iridocyclitis","authors":"","doi":"10.33140/jocr/02/03/00001","DOIUrl":"https://doi.org/10.33140/jocr/02/03/00001","url":null,"abstract":"Background: A case of acute onset bilateral iridocyclitis is reported in a previously undiagnosed and asymptomatic case of sarcoidosis. Case: A 39 years old male presented with pain and redness in the right eye for one day. Slit lamp examination revealed granulomatous iridocyclitis in the eye with three hundred sixty degree posterior synechiae. While under investigation for the cause, on fourth day of initial presentation, left eye also developed similar granulomatous iridocyclitis, being symptomatic only overnight. All laboratory investigations, radiological evaluation and biopsy led to the final diagnosis of sarcoidosis. Conclusion: Sarcoidosis is a multisystem granulomatous disorder of unknown etiology which may present as an acute uveitis or chronic form. Subjects with Ocular\u0000deviation between 10 to 40 prism diopters, Corrected distance Visual Acuity should be greater than 6/18 and Age\u0000should be between 10 to 40 years of age were included in the study. Visual Acuity was assessed with Log Mar chart.\u0000Results: 30 subjects were included in the study. Out of that, 16 subjects were in the age group of 11-20 years, 12\u0000subjects were in the age group of 21-30 years and 2 subjects were in the age group of 31-40 years. 60% subjects were\u0000Female and 40% subjects were Male. The mean visual acuity was considered in each amount of deviation. If a cancer patient has got some orbital lesion everyone thinks about orbital metastasis. But a cancer patient sometimes has other orbital tumors or inflammatory orbital lesions. The aim of the work is to identify distinctive features of the orbit metastatic tumor for differential diagnosis with other tumors and inflammatory lesions in the orbit. Material and Methods: Retrospective, non-comparative, chart review of 81cancer patients. 74 patients (10 men and 64 women aged 18 – 87 years old, median 45 years old) had orbital metastases, 5 patients had the second malignant orbital tumor - primarily multiple malignant tumors (1 man and 4 women aged 55 – 78 years old, median 61 years old), and 2 men (64, 66 years old accordingly) had inflammatory orbital lesions. All the patients underwent complete ophthalmological and general examination. The orbital lesions were studied morphologically. Results: Distinctive features of the orbit metastatic tumor are the following: not rapid but gradual development of symptoms. As a rule there is a single well-delimited tumor in the orbit. Metastatic tumor is mainly localized under the upper wall of the orbit in the anterior or middle part of the orbit. Due to the localization of the tumor, exophthalmoses develop with displacement of the eyeball (mechanical strabismus). The main symptom of the orbital metastases is the limitation of the eye movements. The symptoms of orbital non-Hodgkin’s lymphoma are identical to the symptoms of orbital metastases. Acute orbital Inflammation, simulating metastases, had atypical erased symptoms. Conclusion: Only a comprehensive assessment of anamnesis data, clinical symptoms and the results of morphological examination of biopsy sample allows establishing the correct diagnosis of the orbital metastatic tumor. Examinations of the anterior segment spotting unilateral irrido-trabecular dysgenesis of the right eye with abnormal visibility of the Schwabe line corresponding to a posterior Embryotoxon and associated angular abnormalities. The eye tone measured with Goldman tonometer showed 14mmHg in the right eye and 15mmHg in the left eye. {"title":"A Prospective Trial Determining the Relationship between Corneal Hysteresis, Corneal Corrected Intraocular Pressure, and Optical Coherence Tomography Parameters in Open Angle Glaucoma Suspects","authors":"Ian McWherter","doi":"10.24966/ocr-8887/100044","DOIUrl":"https://doi.org/10.24966/ocr-8887/100044","url":null,"abstract":"Background: Glaucoma has been associated with low corneal hysteresis, a characteristic that may provide additional information in identifying those who are considered glaucoma suspects. The current study aimed to determine the relationship between corneal hysteresis, corneal compensated intraocular pressure, and GCC analysis in patients who are labeled clinically as open angle glaucoma suspects. Methods: Participants were categorized into 2 groups, control (n = 14) or suspect (n = 27), based on clinical examination. Corneal hysteresis and optical coherence tomography were performed and analyzed for any correlation between these two groups. Results: Correlations between corneal hysteresis and parapapillary retinal nerve fiber layer thickness or corneal hysteresis and ganglion cell complex thickness in the macula were weak to moderate and ranged from r = -0.340 to r = 0.316. The correlations between corneal compensated intraocular pressure and parapapillary retinal nerve fiber layer thickness or corneal compensated intraocular pressure and ganglion cell complex thickness were also weak to moderate and ranged from r = -0.489 to r = 0.396. Conclusion: In the current study, corneal hysteresis and corneal compensated intraocular pressure did not provide a definitive relationship when trying to identify patients as glaucoma suspects. The relationships between corneal hysteresis and retinal nerve fiber layer thickness or corneal hysteresis and ganglion cell complex thickness were weak to moderate. Although wearing contact lens is safe and does not require expertise but it\u0000can cause serious eye infections and corneal ulcers, sometimes resulting into loss of vision. The study is designed to\u0000com-pare and observe the contact lens hygiene compliance and self-management behavior with a focus on the contact\u0000lens wearing habits, cleaning and maintenance of contact lens by different methods and self-prescription between the\u0000medical and non-medical students of the Saudi Arabia.\u0000Methods: Five hundred young contact lens wearers with an average age of 18-22 years were selected conveniently\u0000from the student population of Princess Nourah Bint Abdulrahman University, Riyadh. After taking informed consent\u0000from the participants, their level of contact lens hygiene compliance and self-management was assessed by using a\u0000peer-reviewed questionnaire.\u0000Results: The mean (±SD) age of the participants was 20.87± 1.696 years. Out of 500 students 37.8% were medical\u0000and 62.2% were non-medical students. 56% students were wearing contact lens for the cosmetic reasons while 43.6%\u0000students were using it for the correction of their myopic refractive error. Most of the students were using daily wear\u0000soft contact lenses (96.6%) by self-prescription (51.4%) and majority of them were buying them from general retail\u0000store (83.6%) instead of some proper optician. The self-management behavior was statistically significant among nonmedical students (p=0.026). There was no significant difference between the two groups regarding the compliance of\u0000the contact lens hygiene but the knowledge and awareness about the risks and complications was statistically high in\u0000the medical students (p= 0.028). Most of the students in our study had rated them as an average wearer.\u0000Conclusion: This study concludes that self-management with the contact lens use is very common among the nonmedical students of Saudi Arabia.
