{"title":"Predictors of elevated intraocular pressure after intravitreal injection of anti-vascular endothelial growth factor: a prospective observational study","authors":"Jolly Tsui, Ivan H. W. Young","doi":"10.12809/hkjo-v25n1-290","DOIUrl":"https://doi.org/10.12809/hkjo-v25n1-290","url":null,"abstract":"Purpose: To investigate the patterns and predictors of intraocular pressure (IOP) changes after intravitreal injection (IVI) of anti-vascular endothelial growth factors (anti-VEGFs).Methods: This study enrolled 32 men and 16 women (mean age, 65.3±12.3 years) who underwent IVI of anti-VEGFs between January and March 2020 in our department. IOPs were measured using Goldmann applanation tonometry. Potential predictors included age, sex, lens status, axial length, history of glaucoma, number of previous IVIs, diagnosis, and post-injection vitreous reflux.Results: The respective mean IOP was 16.2 mmHg, 32.7 mmHg, 21.7 mmHg, and 18.3 mmHg at baseline and at 5, 15, and 30 minutes after IVI. IOP elevation of ≥15 mmHg was observed in 48% of eyes at 5 minutes after IVI; all spikes resolved and the IOP reduced to <21 mmHg within 60 minutes. Previous IVI number (r=0.346, p=0.016) and baseline IOP (r=0.304, p=0.04) were associated with IOP at 5 minutes after IVI. Baseline IOP was associated with IOPs at 15 and 30 minutes after IVI (r=0.488-0.573, p<0.001). In multivariate regression analysis, the previous IVI number (b=0.55, p=0.04) was an independent predictor of IOP at 5 minutes after IVI.Conclusion: Transient but substantial IOP elevation shortly after IVI of anti-VEGFs was positively correlated with the number of previous IVIs; this could be used to stratify patients for IOP spike prophylaxis, especially those with advanced glaucoma at risk of further optic nerve damage secondary to acute ocular hypertension. After initial 1.5T magnetic resonance imaging, a double primary of left lacrimal gland adenoid cystic carcinoma with tricompartmental (frontal bone, lacrimal and temporalis fossa) involvement was suspected. However, subsequent high-field multi-parametric 3T magnetic resonance imaging suggested a solitary metastasis with a subperiosteal location and an uninvolved lacrimal gland. {"title":"Hyperbaric oxygen therapy for central retinal artery occlusion: experience in Hong Kong","authors":"Lai Ting Yip, S. Au, C. Ko","doi":"10.12809/hkjo-v24n2-281","DOIUrl":"https://doi.org/10.12809/hkjo-v24n2-281","url":null,"abstract":"Objective: To review medical records of patients who underwent hyperbaric oxygen therapy (HBOT) for central retinal artery occlusion (CRAO).Methods: Medical records of patients who underwent HBOT for CRAO between November 2018 and December 2019 were reviewed. The first emergency HBOT was at 2.8 atmosphere absolute for 90 minutes with staged decompression. Subsequent sessions were at 2.4 atmosphere absolute twice daily or daily. Patients were followed up daily at the eye clinic. HBOT lasted for 5 days or 10 treatment sessions if there was visual improvement on day 3. Treatments were discontinued if patients had no visual improvement or were unable to tolerate the treatment or experienced major adverse effects, or when the patient was confirmed to not have CRAO.Results: Of 31 patients who underwent HBOT, 25 with CRAO (17 men, 8 women; aged 44 to 89 years) were included. Mean onset-to-door time was 3.3±4.2 hours, and mean onset-to-HBOT time was 13.3±7.4 hours. Mean number of HBOT sessions was 7.9±2.7. Mean change in visual acuity (VA) was -0.43 LogMAR (p=0.003). At the end of treatment, 84% had VA of 0.1 (1.0 logMAR) or worse and 64% had VA of finger counting (1.7 logMAR) or worse. No factors were associated with VA improvement including age, onset-to-door time, onset-to-HBOT time, number of HBOT sessions, or pre-HBOT VA.\u0000Conclusion: HBOT for CRAO shows promising visual outcomes. They occur in anatomically predisposed eyes similar to acquired lower lid epiblepharon. Chong","doi":"10.12809/hkjo-v24n2-288","DOIUrl":"https://doi.org/10.12809/hkjo-v24n2-288","url":null,"abstract":"Objective: To report the long-term efficacy and safety of simultaneous correction of upper eyelid cicatricial entropion and dermatochalasis.\u0000Method: A retrospective non-comparative chart review of consecutive patients with severe upper eyelid cicatricial entropion and dermatochalasis who underwent surgical correction from July 2012 to July 2014 at a tertiary referral centre by a single surgeon.\u0000Results: Twelve eyes of 10 patients aged 80±7 (range, 65-92) years old received upper eyelid blepharoplasty, tarsal margin rotation, and graded posterior lamellar super-advancement. All eyes resumed normal upper eyelid margin position with no recurrence over a mean follow-up of 48.3±18 (range, 9-70) months. Complications including suture granuloma (n=1), transient lagophthalmos (n=1), mild lid notching (n=2) and residual peripheral asymptomatic trichiasis or distichiasis (n=3) were managed conservatively successfully.
